We may not have a specialist doctor and may not be able to comply with your needs even if you have arrived at the hospital. Please make sure to contact us before you come in. < Regular consultations > Weekdays 8:45~17:00 < Closed > Saturdays, Sundays, National holidays & New year holidays (Dec. 29 ~Jan.3) Registration for emergency consultations during off-hours, weekends, and holidays is at the off-hour reception counter located on the 1st floor just inside the entrance. HOW TO REGISTER ON YOUR FIRST VISIT: 1.Complete a First-visit registration form beside the General Information Counter. 2.Present the completed form, health insurance card, Iryojukyushasho (medical care recipient certificate for infants, senior citizens, persons with disabilities, etc.), and referral letter, (if you have one) to First-visit Counter 2. A receptionist will give you a waiting number. 3.Your number will be called at counter no. 2 as soon as registration is completed and your patient ID card is ready. Claim your patient ID card. 4.Proceed to the department you are visiting. Hand in your patient ID card, registration slip (green paper) and referral letter (if you have one) to the receptionist at the counter. You are required to present your insurance card and your patient ID card at the Insurance Confirmation Counter 3 (2nd floor Central Reception) for insurance verification whenever you visit a new department. - 1 -

What is Sentei ryoyo fee (sentei means to choose or select, ryoyo means treatment) It is an additional fee charged to first-time visiting patients without a referral letter in large medical facilities (facilities with over 200 beds); This system was established by the Ministry of Health, Labor and Welfare to promote cooperation and division of labor among medical institutions by encouraging patients to make non-emergency or initial visits to smaller clinics and specialized treatments to large medical facilities. Please note that aside from the consultation fee, an additional Sentei Ryoyo fee of \4,320(tax included)is charged to first-time visiting patients without a referral letter. You are also considered as a first-time visiting patient when you visit us without appointment and more than 6 months have passed since your last visit. Moreover, the Oral and Maxillofacial Surgery Department is classified under a different category, and thus, when visiting the Oral and Maxillofacial Surgery Department and another Department at the same time, and for the first time, the sentei ryoyo fee applies separately to each department. HOW TO REGISTER ON YOUR SUBSEQUENT VISIT: (either with appointment or without appointment) 1.You may use a reception machine to register. Insert your patient ID card into the machine. Take the registration ticket. 2. Proceed to the department you intend to visit. At the counter, present your ticket and your patient ID card. If your insurance card has not been verified for more than two months, you need to present it with your patient ID card at the reception counter of the department you are visiting. Also, please inform the staff at the Insurance Confirmation Counter 3 (2nd floor Central Reception) of any change in your status, name, address, phone number or any other details that we should know. AFTER THE CONSULTATION: 1.Hand in your kihon card (order slip) at the reception counter of the department you are visiting. 2.The receptionist will calculate your bill and will call your name when it is ready for payment. You can then make payment through an Automated Payment Machine located on the 2nd floor by inserting your patient ID card in it. 3.If you were prescribed a medication, a prescription exchange stub will be issued to you after completing payment. 4. Proceed to the Prescription Hand-over Counter 6, exchange your stub for your prescription. You can choose any pharmacy outside the hospital to purchase your medicine. 5.If you come here by car, you can get a parking discount by presenting your parking ticket and your payment receipt at the Payment Counter 4. Parking tickets must have the validation stamp to receive any eligible discounts, so do not forget to bring it with you to the hospital. For more details, please refer to page 3. Floor manager at your service ( Also known as FLOOR ASSISTANT ) A floor manager is stationed at the lobby on the 2nd floor from 8:00 a.m. to 12:30 p.m. to point you to the right direction, help you with the registration process, answer inquiries, and etc. Please feel free to ask for assistance whenever you need help. - 2 -

COUNSELING & SUPPORT: (Registration: Weekdays 9:00 to 17:00) We provide confidential counseling services to our patients and their families to help them receive treatment with ease. Please feel free to consult us about illness, financial matters (medical fees, insurance/medical system), or any other concerns that you may have. All consultations are conducted in private rooms. PARKING AREA: B1 Parking Area 7:30~20:30 Exclusively for persons using wheelchairs, persons with disabilities and persons age 75 and older. Closed on Saturdays, Sundays, National holidays and New Year holidays. To get a parking discount: After completing payment, present your receipt and your parking ticket at the Payment Counter 4 (2nd floor Central Reception). The ticket should have a validation stamp at the counter, so please be sure to bring it with you to the hospital. In case you forgot to bring your parking ticket to the hospital and you need to go back to your car to get it, you can have it stamped at the off-hour counter on the 1st floor (located just inside the emergency entrance) instead of having it stamped at counter 4 on the 2nd floor. Families or persons accompanying a patient in an emergency admission, or those who are waiting during surgery are eligible to receive a parking discount. Just present your parking ticket at the nurse station. VISITING HOURS: Our visiting hours are designated to provide an appropriate environment and fulfilling treatment and care for our patients. The Dermatology and Ophthalmology departments are temporarily closed. Consultation schedules may change on an ad-hoc basis. Please check the schedule board beside the general information counter for latest updates. RGMC policies Smoking is prohibited in all areas of the hospital site. Using cell phones for conversations or e-mails in certain areas inside the hospital may interfere with some medical equipments. We request you to use your cell phones only in permitted areas such as below: at the pay phone corner ( 2F 3F Outpatient Departments ) in the waiting lounge ( 2F beside the main entrance ) in the cafe ( 2F ) behind the elevators ( 1F ) There are also Cell phones Permitted Area signs on other floors. Insert your patient ID. Take the printed waiting number and proceed to the medical department that you wish to visit. Go to the Central Reception Area, Counter no. 3 Insurance Confirmation Present your patient ID. You will be given a consultation number (green paper). You will be given a waiting number. Wait until your number is called. New patients are called by numbers. Referred patients are called by names. You will receive your patient ID card and consultation number (green paper). HEALTH INSURANCE CONFIRMATION Please note that we need to check all health insurance certificates once a month. If your insurance has not been verified for more than a month, please present it at the reception counter of the department that you are visiting. Also, please inform the staff at the Insurance Confirmation Counter no. 3, Central Reception Area of any change in your status, name, address, phone number, or any specific details that we should know. HEALTH INSURANCE CONFIRMATION Health insurance information is checked in every department once a month. CHECK-IN ( at the Reception Counter of the department you have booked ) Present your Patient ID, Referral and Consultation number (green paper). BILLING ( at the department you have booked ) After the consultation, hand-in the order slip (KIHON card) to the staff at the counter. PAYMENT ( 2nd floor, Central Reception Area ) You can pay through the Automated Payment Machines or at the counter. (*Your patient Id card or the consultation slip is needed for reference during payment.) If you were prescribed a medication, a prescription exchange stub will be given to you after completing payment. PRESCRIPTION HAND-OVER COUNTER (2nd floor, Central Reception Area) Present your prescription exchange stub at the Prescription Hand-over Counter to get your prescription. You can have it filled at any pharmacy of your choice outside the hospital. TO SCHEDULE AN APPOINTMENT: If you wish to schedule an appointment for a consultation or a medical test, please discuss it with your attending physician first.
