We are here to help you make it happen. While it s great to talk about a positive future, we realise that ensuring it happens involves boring paperwork too. t s vital, in fact. We ve tried our best to keep this form simple, but there s no disguising the quantity of boxes to be filled in. t shouldn t take more than 30 minutes to complete, but we suggest you make yourself a cup of tea, perhaps even grab the biscuit tin, and find a comfy place to seat yourself before diving in. A few points to note before you get started: Before your solar installation is commissioned, you will need to make sure you have an energy performance certificate (rating level D or above) f your installation is above 30kW, we will deduct a quarterly charge of 25 +VAT per quarter from your FiT payments to cover the cost of our administration. Businesses buying energy from us, or sites selling their export power to us, will be exempt from this charge Due to Government changes, extensions of an existing FiT accredited system aren t eligible for FiT after the 14th January 2016 Find out more at goodenergy.co.uk/fit We can only pay the Feed-in Tariff (FiT) to eligible generators, so please take care to provide all the information we need to ensure you can be correctly accredited. f you would like any help when completing the form, or have any questions, then please call our FiT team on 0800 254 0000. 3. Generation meter details FiT payments will be based on the readings from your total generation meter. This is the meter that records how much energy your system is producing. Please note, this will be a different meter to the one that measures the electricity you import from an electricity supplier. The details of your total generation meter should be printed on its front. Please copy these details into the boxes below. By ticking this box, hereby declare that it is my intention to use any and all electricity generated by my FiT installation and that fully understand that any electricity generated but not so used will not be eligible for FiT payments. 5. Export meter details Do you have an export meter? (This is a meter that measures the electricity that you are supplying back to the grid.) Please note that if you do not have an export meter and: your installation has a total installed capacity of 30 kw or less, we will estimate the amount that you export - called deemed export. The current figure is 50% of your total generation for solar PV, wind, AD and micro-chp, and 75% for hydro. your installation has a total installed capacity of over 30 kw, you will not be eligible for deemed export payments. f you wish to receive export payments, you will need an export meter. 6. Energy Performance Certificate declarations Solar PV installations only All applications for accreditation of new solar PV installations with an eligibility date on or after 1 April 2012, need to be accompanied by a copy of this document with the relevant section signed and dated. This will then be used by FiTs licencees/ofgem, as appropriate, to determine whether or not the energy efficiency requirement applies to your installation and, if so, whether it has been met. f your application is for a new solar PV installation you must hold an EPC for your property, unless your installation is not wired to a relevant building *. Please read the points below to find out which declaration is applicable to you, tick the relevant one and then sign the corresponding declaration on the following pages. The energy efficiency requirement does apply and an Energy Performance Certificate (EPC) rating of level D or above has been achieved - this was done before the commissioning date. Energy Performance Certificate declaration 1 certify in respect of this application for accreditation that all of the following are applicable: A. the eligible PV installation is wired to provide electricity to one or more relevant buildings * ; B. a valid EPC is enclosed in respect of the building (or one of the buildings) to which the PV installation is wired to provide electricity; C. the enclosed EPC is the most recently issued energy performance certificate in respect of that building; D. the enclosed EPC certifies that the relevant building to which it relates has been assessed as being within bands A-D; ('E' and 'F' relevant if EPC issued in England and Wales only: if energy performance certificate issued in Scotland ignore 'E' and 'F' below and sign the declaration) E.(i) am an owner or tenant of the relevant building to which the enclosed energy performance certificate relates; or (ii) certify that am not a person mentioned in (i) but such a person has co-signed this document; F. : or (where this declaration is co-signed by another person, that person) consent to the disclosure of the enclosed energy performance certificate and / or the information contained within it to one or more FiT Licensees, the Gas and Electricity Markets Authority (Ofgem) and ministerial departments of the government and devolved administrations for retention and use by them for all purposes connected with administering, auditing, reporting on and performing statistical analysis on the Feed-in Tariff scheme for the duration of that scheme. 7. Multi-installation declarations Solar PV installations only All applications for accreditation of new solar PV installations, except extensions, with an eligibility date on or after 1 April 2012, need to be accompanied by a copy of this document with the 5 or 6 signed and dated. This will then be used by FiTs licencees/ofgem as appropriate to determine whether or not the multi-installation rates should apply to your installation. 8. Switching FiT provider This section is only for those already registered for the FiT with another FiT licensee and who wish to switch to Good Energy. By submitting this form to Good Energy, also confirm that if have included personal information about anyone else, have obtained his/her permission to do so and informed him/her that his/her information may be used for these purposes. * REGOs show that the electricity you have generated is from a renewable source. 9

This will include terms and conditions, accreditation details and bank details form. Please check the information carefully and fill in the appropriate documents before returning them to us at the above address within 10 working days. We won t be able start your FiT payments until we ve received your signed T&Cs (you can also view these online at goodenergy.co.uk/generate). By signing below agree to all statements have confirmed above By signing the declaration below you are confirming that you have installed a FiT eligible technology and attached all the required documentation as noted on the front of the form. You must not sign this form and send it until you have had the equipment commissioned.
