Your PCP may have an established relationship with a certain group of self-referral specialists in HealthSpan s network. You must speak with your PCP and receive an approved written authorization from HealthSpan s Medical Management team before seeking care from a specialist in this section. Failure to do so could result in loss of benefit and increased out-of-pocket cost to you. Please remember that your PCP choice will determine which specialists are available to you.
