SIGNIFICANT EVENTS IN THE HISTORY AND PRE-HISTORY OF BLANCHARD VALLEY HEALTH SYSTEM JUNE, 1891 Findlay s first hospital is incorporated. Located at 1217 North Main Street, it is known as the Findlay Home for Friendless Women and Children. 1893 1895 1949 1950 Hospital moves to the West Park area, near the Findlay Airport. Purchase of French Estate in south Findlay for $10,000 (this is the present site of Blanchard Valley Hospital). Name changed to "Home & Hospital of the City of Findlay." Women s Auxiliary incorporated. Findlay Hospital changes its name to "Blanchard Valley Hospital." JUNE, 1958 New 170-bed hospital completed and dedicated. Financed through local bond issue and contributions. 1963 Nursing home opens in the Donnell Wing with 39 beds, later expands to 43 beds. The nursing home closed in 1977. OCTOBER, 1967 $2 million, 60-bed east wing expansion. Increases Blanchard Valley Hospital capacity to 230 beds. AUGUST, 1972 Emergency department and ambulatory services expansion costs $1,700,000. NOVEMBER, 1974 BVH becomes the first hospital in the nation to guarantee its services. Articles appear in The Wall Street Journal and Ladies Home Journal. JUNE, 1977 Community passes $4 million bond issue for surgery and intensive and coronary care unit expansion.

1977 Same-day surgery program developed. SEPTEMBER, 1979 East tower, surgical suites and special care units dedicated. Construction and equipment cost - $11 million. 1981 BVH begins home health services. JANUARY, 1983 Blanchard Valley Health Association is formed as a parent corporation of Blanchard Valley Hospital and Blanchard Valley Health Services. FEBRUARY, 1983 BVH opens medical center in Carey, Ohio. APRIL, 1984 Hospice of Hancock County joins BVHA. 1984 1986 Ambassador program established Apples replace smoking at BVH. FEBRUARY, 1988 Woman Wise Mammography Center opens. AUGUST, 1988 BVH opens physician s office in McComb, Ohio. SEPTEMBER, 1989 Center for Diagnostic Studies (CDS) opens. Construction cost - $6 million. Building is devoted entirely to outpatient services. APRIL, 1990 Blanchard Valley Radiation Therapy Center opens. NOVEMBER, 1990 BVHA opens the Lake Cascades Professional Office Complex at 300 W. Wallace Street, Findlay. Offices are occupied by several physician specialists. FEBRUARY, 1991 The Health Bank corporate wellness program is launched, providing approximately $45,000 annually in free wellness-related services to area businesses.

JUNE, 1991 Happy 100th Birthday to Blanchard Valley Hospital! Year-long centennial celebration features an on-site museum in the CDS building. SEPEMBER, 1991 Occupational health practice opens in Findlay. Will ultimately become part of BVHA. NOVEMBER, 1991 $1 million renovation and expansion of Obstetrics Department completed. MARCH, 1992 Blanchard Valley Sports Medicine Center opens in the Center for Diagnostic Studies. Appointments by physician and self-referral. MARCH, 1993 BVHA purchases Sherman House, a 71-unit independent living complex in downtown Findlay. OCTOBER, 1993 Blanchard Valley Hospital opens cardiac catheterization lab in the Center for Diagnostic Studies. SEPTEMBER, 1994 Northwest Ohio Health Partnership managed care plan developed. OCTOBER, 1994 Caughman Health Clinic opens with sliding-scale fees based on income. DECEMBER, 1994 Creighton Dialysis Center opens at Blanchard Valley Hospital. Provides hemodialysis and home dialysis services. JUNE, 1995 BVHA launches website www.bvha.org. JULY, 1995 Blanchard Valley Hospital and Bluffton Community Hospital merge to become Blanchard Valley Regional Health Center (BVRHC). JANUARY, 1996 Bridge Hospice Care Center merges with Blanchard Valley Health Services to form entity known as Bridge Home Health & Hospice. FEBRUARY, 1996 On-site rehabilitation services established at Findlay s Whirlpool Corporation.

OCTOBER, 1996 Nurses Plus telephone triage service opens a program developed with Fostoria Community Hospital. DECEMBER, 1996 BVRHC announces record setting year for physician recruitment, with 19 physician specialists joining the active medical staff. JANUARY, 1997 BVRHC announces plans to bring outpatient rehab and sports medicine services to the University of Findlay campus, occupying 4,000 sq. ft. in the Edward and Joyce Brewer Health Sciences Building. JANUARY, 1997 Pain Management Center opens. APRIL, 1997 Plans for Physicians Plus Urgent Care Center announced at press conference. Physicians Plus will be located in north Findlay and feature state-of-the-art computer technology and drive-thru laboratory. MAY, 1997 BVH, in cooperation with Lima Memorial Hospital and Hardin Memorial Hospital, opens new medical building in Ada, Ohio. JUNE, 1997 BVHA purchases Winebrenner Village long-term care facility. Winebrenner becomes a subsidiary of BVHA that is later renamed the Jack Schaefer Retirement Community (and includes Birchaven Village, Birchaven Heights and Birchaven Estates). JANUARY, 1998 Durable medical equipment (DME) business purchased. New business under BVHA will be located at 117 E. Wallace Street. APRIL, 1998 Karrington and Winebrenner Village enter into a 50/50 partnership of the Karrington assisted living facility in Findlay. JUNE, 1998 Physicians Plus Urgent Care/Medical Mall opens. JULY, 1998 BVRHC receives its highest score and first accreditation with commendation rating from the Joint Commission on Accreditation of Healthcare Organizations.

SEPTEMBER, 1998 BVHA and ProMedica are chosen as Lima Memorial Hospital s new partners. A new joint operating company is formed, with 50% ownership by Lima Memorial; 25% ownership by BVHA; and 25% by ProMedica.* (*BVHA exited this partnership in March 2007 by selling its share of the partnership to ProMedica). OCTOBER, 1998 Development of stereotactic breast biopsy and bone densitometry capability. DECEMBER, 1998 First employee attitude survey provided for all BVHA staff. Results to follow in 1999. MARCH, 1999 MIS Department starts preparation for Y2K computer dilemma. APRIL, 1999 BVHA partners with 30 local physicians to build and operate the Findlay Surgical Center, which will be located on the west bank of Lake Cascades. JULY, 1999 BVH Cafeteria is remodeled to provide better and faster service. AUGUST, 1999 Sleep lab opened by Dr. Wahon Aslanian. OCTOBER, 1999 Ottawa Medical & Diagnostic Center opens in Ottawa. JANUARY, 2000 Service Excellence Department developed. APRIL, 2000 Genesis 2000 project for new emergency department, underground parking garage, cardiology services, and outpatient services commences. MAY, 2000 Valet parking provided for patients and visitors JUNE, 2000 Private rooms provided for all oncology patients. FEBRUARY, 2001 First Distinguished Physician of the Year Award goes to William Alcott, MD MAY, 2001 Auxiliary Nearly New Shop Moves to Lima Avenue in Findlay.

JULY, 2001 North Baltimore Medical & Diagnostic Center office building opens. NOVEMBER, 2001 BVRHC Bluffton expansion complete.the $2.85 million renovation project includes a new lobby and registration area, gift shop, expanded cardiopulmonary unit, CT scan and bone densitometry equipment, complementary medicine offices and new physical therapy and occupational therapy areas as well as renovation of the OB department. MAY, 2002 William E. Ruse Center facility opens at the BVRHC Findlay campus. Houses new ER, imaging services, same day surgery, HeartCare Center, helipad, and a third floor for future expansion MARCH, 2003 BVH partners with Hardin Memorial Hospital to open the Valley Health Center, Kenton, Ohio. JUNE, 2003 Opening of Birchaven Retirement Village at EasternWoods. Services include: congregate living, long-term care, Bridge Home Health and Hospice, Adult Day Center and Intergenerational Program. AUGUST, 2003 Opening of new Blanchard Valley Regional Cancer Center (formerly Radiation Therapy Center) at EasternWoods. Board approves construction of a new inpatient pavilion on the Findlay campus. The six-story, 200,000 square foot, $95 million project will include 150 inpatient beds, and also renovation and expansion of the surgery department. MAY, 2004 Bariatric Solutions opens, offering Findlay s first medical and surgical weight loss center. OCTOBER, 2004 Groundbreaking for new inpatient pavilion on the Findlay Campus. NOVEMBER, 2004 Groundbreaking for the new Birchaven Condominium development (Birchaven Estates) at EasternWoods. JULY, 2005 BVH expands its partnership with Hardin Memorial with the inclusion of the Forest Community Health Center, Forest, Ohio. NOVEMBER, 2005 First open heart surgery performed on the Findlay Campus on November 15.

FEBRUARY, 2006 Sixth Distinguished Physician of the Year Award goes to Marv Rower, MD. JANUARY, 2007 BVHA changes its name to Blanchard Valley Health System. Blanchard Valley Regional Health Center changes to Blanchard Valley Hospital and Bluffton Hospital. Address of BVH and BVHS changes to 1900 South Main Street (from 145 W. Wallace Street). FEBRUARY, 2007 The Creighton Dialysis Center moves to the Riverside campus on East Main Cross Street and begins accepting patients on February 12. Inpatients are moved into the Thomas B. and Kathleen M. Donnell Patient Pavilion on February 17. AUGUST 2007 Findlay experiences the worst flood in its history. OCTOBER, 2007 BVHS moves several departments to Riverside Suites location at 1100 E. Main Cross Street from the former VIP Motel location across Main Street from BVH. 2008 Bluffton Hospital celebrates its centennial. FEBRUARY, 2008 Bridge Hospice Care Center opens at BVH. Provides inpatient hospice care. MAY, 2008 Orchard Hall moves into their renovated location on 2 East in the Kose Building at BVH. AUGUST, 2008 Groundbreaking begins for EasternWoods Outpatient Center on the EasternWoods Campus in Findlay. FEBRUARY, 2009 Wound Care Solutions opens at 100 W. Pearl Street, the old location of Bariatric Solutions. Bariatric Solutions moves to 1920 S. Main Street. AUGUST, 2009 EasternWoods Outpatient Center opens on August 31. Offers a full range of outpatient imaging services, an outpatient laboratory, etc.

SEPTEMBER, 2009 Bariatric Solutions closes. APRIL 2010 Bluffton Hospital expansion groundbreaking takes place for a $12 million project that will expand/renovate the emergency department, surgery department and registration area. JUNE, 2010 BVH performs its first robotic surgery using the da Vinci surgical robot. JULY, 2010 BVHS launches a social media presence on Facebook, Twitter and YouTube. AUGUST, 2010 Bridge Hospice Care Center opens in Bowling Green, Ohio, located on the Heritage Corner Health Care Campus. MARCH, 2011 Blanchard Valley Hospital is named one of the 100 Top Hospitals in the nation by Thomson Reuters. NOVEMBER, 2011 Bluffton Hospital is named a Top 100 Critical Access Hospital in the U.S. by the National Rural Health Association. BVHS implements flu vaccination policy for all associates. JUNE, 2012 BVH HeartCare Center renamed the Jane & Gary Heminger HeartCare Center. DECEMBER. 2012 BVHS opens the Neil and Laura Clark Women & Children s Center. 2013 BVHS assumes primary ownership of Hanco Ambulance, Inc. APRIL, 2013 BVHS enters pediatric partnership with Nationwide Children s Hospital. OCTOBER, 2013 BVH forms a Nursing Congress to give front line nurses a voice in change. JANUARY, 2014 BVHS board approves the building of a comprehensive regional cancer center, later named The Armes Family Cancer Care Center.

FEBRUARY, 2014 Hanco opens a second station on the northeast side of Findlay at 1155 Croy Drive. Birchaven Village named one of the Best Nursing Homes 2014 by U.S. News & World Report. BVH Rehab & Sports Medicine renamed Julie A. Cole Rehab & Sports Medicine. MARCH, 2014 BVHS partners with Mid-Ohio Development Corporation to add an apartment complex for Findlay residents on the EasternWoods campus. MAY, 2014 BVHS earns a silver award from the Partnership for Excellence, Ohio s Baldrige Performance Excellence Program. 2015 BVHS begins a transition to a new electronic medical records vendor, Cerner. MARCH, 2015 Blanchard Valley Hospital is named one of the 100 Top Hospitals in the nation by Truven Health Analytics for the third time in the last 10 years. JUNE, 2015 Construction of new Daniel J. and Maria H. Sak Sleep Wellness Center announced, with the old building to be razed. Building opens in November.
