1 How Premier Members access the Verizon Member Agreement from the Premier website. These instructions are designed to connect your organization to the Verizon Wireless Agreement via the Premier Inc website using the MyPremier product path as shown below. To access the MyPremier site, you will need a user name and password for Premier s website, Premierinc.com. If you currently don t have a user name and password, go to Step 1 below. Otherwise, go to step 2 on page 3. Step 1: Go to which is Premier Inc s home page. To register, click on the register link as shown below within the login box. Then, follow the below directions: The form on the following page will populate Helpful hints: When filling out the registration form to secure a User Name and Password for the Premier website, follow these helpful hints: 1) When typing in your company name avoid abbreviating the name. Instead, please key in your complete company name. 2) On the second line of the address, type in your entity code which is your membership number. If you do not know it, call the Solution Center at Optional: In the third address line, please type Verizon. 3) It is only necessary to complete the Personal information and Password Reminder sections. DO NOT complete the following sections: Supply Chain Advisor, Registration Code or the Suppler Authentication for the Product Portal. Doing so, will delay the User Name and Password registration process. Note: Turnaround time to receive a user name and password from Premier, is anywhere from 1 to 3 business days. Once you have the user name & password, go to Step 2 1
2 When typing in your company name, please avoid using abbreviations. Instead, please enter your complete company name. This is also known as your Membership number. If you do not know it, please call the Solution Center at and they can provide that for you. Please notate your PIN as it will not be included in your confirmation . Enter your work phone number. Please do not use any punctuation. You must request three user IDs as your first and second choices might already be used. Do not complete anything below this thick orange line. Do not complete. 3 If you have trouble with the above on line form, which is obtained on line at call Step 2: Once you receive your user name and password from Premier go to enter your user name and password into the login screen and under Product, select MyPremier as shown below. Once logged onto the site, click on the Verizon Wireless logo. This will take you to the Verizon Wireless contract site for eligible Members of Premier to review and accept the terms and conditions of the Verizon Wireless agreement. Please complete the online form and accept the terms and conditions to be connected to the agreement. Step 3: Upon verification and approval from Verizon Wireless, you will receive an executed agreement. Please do not roll the program out to your employees just yet. From the date you receive the executed agreement, it will take up to 3 to 5 business days for Verizon Wireless to prepare your profiles with all the elements within the agreement. Once prepared, Verizon Wireless will notify you via of our readiness to accept orders and register for the discount. For employees to connect to this agreement, the above process needs to be completed first by someone who can read and electronically sign the agreement. Once notified that Verizon Wireless has processed your agreement and is ready to accept orders with the discount, you may then announce this program to your employees. 4 Frequently Asked Questions and Answers In order to receive benefits for your business and employees, a Member Agreement must first be executed between yourself and Verizon Wireless. This agreement states your desire to participate in the program and agree to terms and conditions outlined in the agreement between Premier Purchasing Partners and Verizon Wireless. This site is designed to provide you an easy way to participate. Please follow the directions above under the heading, How Premier members access the Verizon Wireless Member Agreement from the Premier Website. To gain access to the Verizon Wireless contract portal site, go to site. You will need a User Name and Password. The above instructions under Step 1 & 2 provide detail instructions. Who qualifies for this program? Your company may participate as long as your organization is an active member within Premier, is categorized as healthcare, and falls within the group and primary service categories listed within Attachment A at the end of this document. How do I participate in the program that Premier has negotiated with Verizon Wireless? Your organization must first electronically sign Verizon Wireless Member Agreement to participate in this program. It is done online through MyPremier. It is easy! Follow the directions under Step 2, under the above heading, How Premier members access the Verizon Wireless Member Agreement from the Premier Website. What information will be needed to sign the agreement on line through MyPremier? Verizon Wireless will direct you to fill out an online form to capture information about you and your Premier Membership. The required fields are listed below: Your company name and address Federal Tax ID A person s name besides yourself who is authorized to make updates on behalf of your company Your Premier Membership number. If this number is unknown, contact the Premier Solution Center by at SolutionCenter@PremierInc.com Select your company Service Category and Primary Service. There will be a drop down box to make your selection. To review these categories prior to going onto the site, see Attachment A towards the end of this document. Do you have existing lines with Verizon Wireless? Answer, Yes or No What is your account number 4
5 My company does not want to take advantage of the discounts through Verizon Wireless, instead my company wants my employees to have access to the discounts. How do they receive the discount? That is fine. Five corporate subscriber or employee subscriber lines is the minimum number that is required to participate in the program. It can be all corporate subscriber lines, or it can be all employee subscriber lines. It can also be a combination of both. They should both add up to at least five lines. What if I have questions about this process, who do I contact? You may the Verizon Wireless team at What if I have difficulty receiving the user name and password to complete Step 2 above, what do I do? You may contact the Premier Solution at Or, them at SolutionCenter@PremierInc.com. But, first go to the end of this document for review on how to fill out the form to gain access to a User Name and Password. Once the Verizon Wireless on-line form is filled out and the agreement is executed, how long will I need to wait to roll this program out to my employees and begin ordering on behalf of my company for my business subscribers? The program can be rolled out to the employees once Verizon Wireless has notified the person that completed the online process. They will be notified via that Verizon Wireless is ready to accept orders and register your organization for the discount. Verizon Wireless anticipates a 3 to 5 day waiting period. This time allows Verizon Wireless to prepare an individual corporate and employee profile, containing information on your organization, with the highlights of the discount elements of the agreement. This is an important step, because the profiles are how Verizon Wireless will recognize your organization and issue the correct discounts. During this waiting period, you will not be able to order or apply for the discount. If additional information is needed, Premiersales@verizonwireless.com will contact you. Will I be notified once I complete the on-line process? Yes. The person that electronically signed the agreement will be notified at least three times by Verizon Wireless. 1) Within 2 business days Verizon Wireless will the executed agreement to the person that electronically signed it within your organization. 2) They will be contacted within 3 to 5 business days after receiving the executed agreement to be alerted of Verizon Wireless readiness to accept orders and registration activity from your employees seeking the discount. 3) User Names and Passwords for the My Business corporate ordering tool will be ed as well to person that electronically signed the agreement and to the person they listed as an authorized person to activate and order on behalf of their company. 5
6 I currently have a Verizon Wireless sales representative that I work with. Will I continue to work with that person? Or, will I have a new sales representative from Verizon Wireless? You will continue to work with the same Sales Representative that you worked with in the past. They should be familiar with this program. If they are not, they can contact and we will educate them on this program. Once I am connected to the agreement and have general billing questions about my corporate subscriber lines, who do I call? Customer Care Support at is available to serve you. Should your preference be an communication instead, send an to WFMBizSupport@hq.VerizonWireless.com. Your concern should be addressed within 24 hours. To ensure your routes to the correct team managing the Premier program, write the word Premier, then the name of your organization in the Subject line of your . I called Customer Service regarding my corporate subscriber account and they were not able to assist me, what should I do? Verizon Wireless realizes that from time-to-time you may have something that needs special attention or that is not a routine request. For specialized or escalated customer service issues, use the following address: WFMSoBSCPremierAffiliates@hq.VerizonWirelesss.com. For proper routing within Verizon Wireless, in the subject line of the write Premier and then the name of your organization. This address is designed for all Members regarding escalations, concerning a corporate subscriber line of service. How will I order business subscriber lines of service? After the contract is executed and during the implementation process Verizon Wireless will send the My Business order tool link to the person that executed the agreement and it will also be sent to the person that was listed as an authorized contact. The online My Business tool is an easy way to manage your business account and order services on demand. Verizon Wireless is committed to helping you maximize your online experience with My Business Account by providing a variety of training options. Whether you re new to My Business Account or looking to build upon existing knowledge, to register how to use this tool go to Now that Verizon Wireless owns Alltel, can those business and employee subscribers qualify for the discount? To receive the discounts, the Subscribers will need to transfer their lines from Alltel price plans and equipment to Verizon Wireless price plans and equipment. Once that has been accomplished, follow directions provided in this document on how to obtain the discounts. 6
7 Tell me abut the corporate 500 text message package? Corporate subscribers upon request, activating new service or renewing an existing line of service are eligible to receive 500 Text Messages per month at no charge. The overage rate is $.20 per message, per address (sent or received). Prevailing messaging rates apply to all other messages. This messaging package may not be combined with other Text messaging packages, offers or promotions. How will my employees get the discount and/or order services? Employee Subscribers can: Call: Visit: A Verizon Wireless store, (not an authorized dealer) Click: Employee discounts are obtained by following the register your line process. This can be initiated by the employees. To register the employee line of the Employer Go to: Upon being greeted with the following screen below, they will be prompted to enter their work address personal s can not be used. If their work address matches our records, they will receive an from Verizon Wireless with instructions on how they may begin to take advantage of the discounts for which they are eligible. Some of my employees don t have a work address, how will they get the discount? Go to and click on the words that read, Don t Have a Work Address? See above. They will be prompted to fill out an on-line form and then fax in their work verification information. (A paystub within the last 60 days or a employee ID 7
8 badge). The information will be reviewed by Verizon Wireless and if approved, the discount will appear within 1 to 2 billing cycles. Employees qualify for the free text messaging. How do they obtain this? To activate the employee 250 free text messaging package, each employee will need to call customer (This can NOT be done on the Internet or at the Verizon Wireless stores). The employee will need to provide the name of his/her organization and have company ID. Tell me about the employee 250 text message package? Employee subscribers, upon request, activating new service or renewing an existing line of service are eligible to receive 250 Text Messages per month at no charge. The overage rate is $.20 per message, per address (sent or received). Prevailing messaging rates apply to all other messages. This messaging package may not be combined with other Text messaging packages, offers or promotions. How can I determine which Verizon Wireless store will work with my employees? Go to and in the upper right hand corner, click on Store Locator. Follow the prompts. While attempting to electronically sign the Corporate Verizon Wireless Member Agreement to enable my company and employees to participate in the program, I get a message from Verizon Wireless that our company does not qualify, what should I do? Contact Premier Solution at SolutionCenter@PremierInc.com and validate that your membership number and zip code are matching what you entered on the online form. I want to access the Premier website to gain access to the Verizon Wireless Member Agreement so that my company and employees are able to take advantage of the discount program. I understand an authorized representative from my company must first electronically sign the agreement. Instructions are outlined in Step 2 above, on page 3 of this document. ATTACHMENT A MEMBER PARTICIPATION: APPROVED CLASS OF TRADES BY GROUP AND PRIMARY SERVICE The following are the only approved Primary Service classifications within the Purchasing Partners Membership Roster pursuant to the Member definition in Section 1.5 of this Member Agreement. No changes to this list will be allowed except by formal written amendment by Verizon Wireless and Primary Customer. 8
9 Each Member must also meet all of the requirements set forth in Section 1.5 in order to be eligible to execute a Member Agreement. Those additional requirements are that each Member must: must (i) have an active membership agreement with Purchasing Partners, current in all its obligations and membership criteria, (ii) appear on Purchasing Partners Membership Roster as an active member, and (iii) be a healthcare organization. GROUP PRIMARY SERVICE DEFINITION OF PRIMARY SERVICE First Responders First Responders Ambulance Services/Paramedic/EMT/Fire Rescue specialty trained medical technician licensed to provide emergency services before and during transport to hospital. Ambulatory Care 9 Ambulatory Care Center Behavioral Health Outpatient Community Health Center Dialysis Center Oncology Center Prison Correctional Health Public Health Department Student/Employee Health Service Substance Abuse Treatment Center Wellness Facility/ Fitness Center Outpatient services beyond the normal physician office visit. May include imaging, laboratory, rehab services within one service center. Must derive more than 50% of their revenue from the ancillary services. Adult or child outpatient psychiatric services or services for the mentally or physically disabled. A community clinic that may be designated as a Federally Qualified Health Center, is required to provide an array of primary and preventive care services including: pediatrics, health screenings, immunizations, mental health, dental, family planning, medications, substance abuse services, as well as services to increase access to care, transportation, translation, case management, health education, and home visitation. These clinics are local, non profit, community-owned organizations and are primarily funded via Medicaid Outpatient Dialysis treatment center An outpatient center that primarily provides specialty oncology services such as chemotherapy or radiation therapy. Diagnostic, therapeutic and preventive health services provided for inmates in a correctional facility. Provides legally mandated services, enforces the state health and safety code, and administers other policies, statutes, regulations and programs which promote quality of life, community health, and well being. Outpatient health facility for school or employer in the campus community. Outpatient facility for the treatment of substance abuse. A facility owned by a healthcare system or facility that can offer one or more programs for wellness, fitness, and medical treatment programs all under one roof. Excludes commercial fitness centers or health clubs such as Gold's Gym, and Bally's Total Fitness. Dentistry Dentistry One who is licensed to practice the prevention, diagnosis, and treatment of diseases, injuries, and malformations of the teeth, jaws, and mouth. Includes oral surgeons and orthodontia. Durable Medical Equipment Home Care DME & Supply Dealer Home Health Care Agency Durable medical equipment dealer resells equipment and supplies directly to patients. An agency that provides medical services in a home setting. Services may be provided by a nurse, occupational, speech or physical therapist, social worker, or home health aide. Includes visiting
10 nurses that participate in the Visiting Nurses Association. Home Infusion Infusion therapy services provided in the home. Infusion Therapy is defined as the way that liquid solutions or liquid medications are administered directly into the blood stream through an intravenous catheter inserted in a vein in the body. Infusion therapies can include total parenteral nutrition, antibiotics or other drugs, blood, and chemotherapy. Must have a valid DEA certificate and pharmacy license. Hospice (home care) A program of palliative and supportive care services providing physical, psychological, social, and spiritual care for dying persons, their families, and other loved ones provided in the home. Hospital Children s Medical Facility providing acute inpatient based care for children under the age of 18. Licensed as an acute care facility. May include outpatient 10 General Medical and Surgical Long Term Acute Care Specialty Hospital care. Facility providing diagnostic, treatment, and surgical services for patients with a variety of medical conditions. These hospitals provide medical and nursing care for more than one category of medical discipline (for example, general medicine, specialized medicine, general surgery, specialized surgery, and obstetrics). May include outpatient care. Licensed as an acute care facility or hospital. Long-term care hospitals, in general, are defined as hospitals that have an average inpatient length of stay greater than 25 days. These hospitals typically provide extended medical and rehabilitative care for patients who are clinically complex and may suffer from multiple acute or chronic conditions. Services typically include comprehensive rehabilitation, respiratory therapy, head trauma treatment and pain management. Specialty Hospitals, such as psychiatric, tuberculosis, chronic disease, rehabilitation, maternity, and alcoholic or narcotic, provide a particular type of service to the majority of their patients. May include outpatient care. Licensed as an acute care facility or hospital. Imaging Centers Imaging Center MRI/CT/Radiology/Imaging Center free standing clinics, mobile services (Provides on-site imaging services for other health care providers including, but not limited to, acute care, long term care, home health care, medical practices, and private industry), or center of Radiology specialty that provide outpatient examination services which may include magnetic resonance imaging, ultrasound, and other forms of x-ray examination procedures. Laboratory Free Standing Long Term Care Facilities Freestanding Healthcare Laboratory Adult Day Care Center Assisted Living A healthcare laboratory is operated independently and provides services to referring clients that may include physicians, long term care facilities, hospital, and other facilities. A community based program offering structured activities and meals. Some health services may be offered for an additional fee. Transportation may be provided. Most programs operate during the week and can be attended full or part-time. Senior housing or residential care facility with group living arrangements that are designed to meet the needs of people who cannot live independently, but do not require nursing facility services. These homes offer a wider range of services than independent living options. Most provide help with some of the
11 Behavioral Health Inpatient Continuing Care Retirement Communities (CCRCs) Hospice (inpatient facility) Independent Living Rehabilitation Center Skilled Nursing activities of daily living and offer 24 hour on site staff and congregate dining. Inpatient non-acute psychiatric services or services for the mentally or physically disabled. Housing communities that provide different levels of care based on the needs of their residents -- from independent living apartments to skilled nursing in an affiliated nursing facility. Residents move from one setting to another based on their needs, but continue to remain a part of their CCRCs community. Typically CCRCs require a significant payment (called an endowment) prior to admission, then charge monthly fees above that. A program of palliative and supportive care services providing physical, psychological, social, and spiritual care for dying persons, their families, and other loved ones in an inpatient setting. Typically for seniors or those with special needs providing independent living and scheduled meals. The cafeteria or restaurant is not open for walk-in commercial business. Inpatient non acute rehabilitation center which may include substance abuse. Provides 24-hour nursing care for chronically-ill or short-term rehabilitative residents of all ages. Pharmacy Closed Pharmacy Provides pharmaceuticals to nursing homes, hospice, skilled nursing facility, sub-acute facility, group homes, correctional facility or other facilities that are sold and billed to end users or their agent. A Closed Pharmacy has no sales to the general public. Physicians Physician(s) Physicians (Physician s Practice) physicians that provide patient care and services in an office or office building. (Physician DEA) Surgery Centers Ambulatory surgery refers to previously scheduled surgical and nonsurgical procedures performed on an outpatient basis in a freestanding ambulatory surgery center's general or main operating room(s), satellite operating rooms, cystoscopy rooms, endoscopy rooms, cardiac catheterization labs, and laser procedure rooms. These facilities typically have one sterile operating suite. Procedures performed in locations dedicated exclusively to dentistry, podiatry, abortion, pain block, or minor procedures are not included. University and Colleges University and Colleges Only those parts of Universities and Colleges that are hospitals and medical schools may participate under this Agreement. Veterinary Veterinary Various kinds of veterinary hospitals, clinics, house call practices or mobile units. Among the types of practices there are also specialty facilities such as day practices, emergency medicine practices (open at night, on weekends & holidays or some are open 24/7), referral practices, surgery facilities, ultrasound specialties, and oncology facilities. The intent of the Primary Service name is to use a common descriptive name for a primary service or market segment. Classifications can vary from state to state. The general definition should be used to classify an organization into a primary service. For example, assisted living is a generally accepted primary service line, but some states classify this as residential care. 11
