Note: Please supply as much information as possible on all of the items above. After this application has been submitted, no additional items may be included without the express consent of the appropriate department. *No petrol-fuelled generators are permitted. Generators are generally not permitted on the highway unless authorised by Middlesbrough Council. *Regulated entertainment includes; the performance of a play, exhibition of a film, an indoor sporting event, boxing or wrestling entertainment, performance of live music, any playing of recorded music, performance of dance, or similar entertainment to live music, recorded music or dance. **If you are intending to provide your own catering facilities you must advise the Events Manager at least one month before the event takes place. There is permanent catering facilities available within MIMA, if you wish to use this facility please liaise directly with the Café Manager 01642 222784. *The location and direction of any PA/sound systems should be highlighted and submitted on the site plan and Noise Management Scheme. CENTRE SQUARE FOUNTAIN Your event will be subject to a risk assessment in relation to the fountain which may be required to be fenced off, the charge for this will be approximately 600. BIG SCREEN MIDDLESBROUGH Middlesbrough has a Big Screen in Centre Square. Events can be linked to the screen either by showing films/stills or using the screencam. If this is something you would like to consider please contact the Events Team on 01642 729135. A formal Traffic Regulation Order is required, then please allow at least three months notice. Please provide details of the number, weight and size of any delivery vehicles and/or participating vehicles:. Note: There is no car parking available in the Town Hall Quadrangle unless prior arrangements have been made. TOILET FACILITITES You will be required to ensure that the toilet facilities are adequate. Please submit details of your proposals to include method of disposal and if toilets are hired, the name and address of the hire company: CLEANSING Please identify the method to be used in order to maintain the area free of litter and refuse: Note: The event organiser should ensure that the site is regularly litter-picked during the event and at the end of each day to ensure that the council's obligations under the Environmental Protection Act 1990 - Code of Practice on Litter and Refuse are discharged. If the event organiser fails to do this then the council reserves the right to carry out the works in default and charge the event organiser the cost incurred. 3
It is the event organiser s responsibility to arrange removal of all rubbish from the site. You will not be permitted to use any council skip/litter bins etc for disposal. The Council reserves the right to remove any unauthorised advertising and to recover the cost incurred from the events organisers. SECTION THREE - INSURANCE Event Organisers are required to hold a current policy of Insurance in respect of Public Liability or Third Party risks (including products liability where appropriate). The relevant limit of indemnity shall be an amount approved by the council s Risk and Insurance Section and Legal Section. Under no circumstances shall this be less than 5 million and the Council reserves the right to require a higher limit if deemed necessary. Organisers are to be responsible for ensuring that any individual participant in the event has an adequate up to date Public Liability Insurance Policy. COPIES OF RELEVANT INSURANCE DOCUMENTS MUST BE SUBMITTED TO THE FESTIVALS AND EVENTS TEAM PRIOR TO THE EVENT TAKING PLACE. SECTION FOUR - NOISE At least 28 days before the scheduled event, the applicant shall submit a noise management scheme indicating how noise from the event and preparations and ancillary activities surrounding the event, will be controlled to meet the following guidance. The noise management scheme shall be based on the Noise Council Guide "Code of Practice on Environmental Noise Controls at Concerts". The music noise level (MNL) shall not exceed the designated level, when measured over at 1metre from the facade of the nearest noise sensitive premises, in accordance with the Noise Council's Guidance, 'Code of Practice on Environmental Noise Controls at Concerts'. The designated level shall be determined by the total number of annual events held at each location in accordance with the following table: Concert Days per year Venue Category Designated Level 1 to 3 Urban stadia or The MNL shall not arenas exceed 75 db(a) over 1 to 3 Other urban or The MNL shall not rural venues exceed 65 db(a) over 4 to 12 All venues The MNL shall not exceed the background noise level by more than 15 db(a) over The applicant shall keep a detailed log of all events held at each venue, to include as a minimum: Details of any noise complaints received, investigations carried out, results obtained and any action taken. Details of the times of rehearsals, sound checks, performances etc and any other activities likely to generate noise. SECTION FIVE - EMERGENCY SERVICES Depending on the size of the event, you are required to inform various emergency services 28 days in advance of the event, if it is a licensable event. Booking of the event will not be confirmed until all relevant information has been received, including insurance documents as required, any changes prior to the confirmed event must be agreed with the Festivals and Events Team. Failure to comply may result in the council refusing to grant permission for the holding of the event. 4
