Description: For 2014-2015, the will award a $500 Scholarship each to 3 Female and 3 Male Hayfield student-athletes. Total scholarships awarded will be $3,000. Award: Scholarship winners will be announced at the Varsity Sports Reception in June. The 2014-2015 scholarships will be awarded to 3 Female and 3 Male Hayfield student-athletes. The scholarships will be paid directly to the college, university, community college or trade school the student-athletes designate. Payments will be made per Hawk Athletic Boosters, Inc. accounting practices. Eligibility: Applicants must be a graduating senior of Hayfield Secondary School who will be attending a college, university, community college or trade school for the Fall semester/quarter of the 2015-16 school year. Applicants must be a Varsity team member of a Hayfield Secondary School sport or sports for the 2014-2015 school year. Applicants must have lettered in a Varsity sport this year. Applicant must have a minimum GPA of 2.5. Applicant must be or have been in good standing throughout the school year. Applicant must be or have been in good standing at all times in all sports and activities in which the athlete participated. A completed essay response is required. Applicants must not be receiving a full scholarship or appointment to any military academy. Applicants who receive an appointment to one of the US Military Academies (or affiliated preparatory schools) or who are awarded a full scholarship at any accredited four-year college or university are not eligible to receive funds from this program. A completed Hayfield Secondary School 2014-2015 Scholarship Application (attachments are acceptable if more space is needed) and a letter of recommendation from one (1) of the student-athlete s varsity head coach is required for the application to be considered for review. Deadline: Submit all required documentation to the Career Center by Tuesday, May 12, 2015.
Selection: The selection will be made by a committee of three (3) members of the Athletic Boosters Club that do not have student-athletes applying for any of the scholarships. Scholarship Committee members will be appointed by the President of the and approved by the Director of Student Activities (the administrator overseeing the ). Scholarship Committee members may be substituted with other athletic booster members if a conflict of interest arises. Applications will be considered based on equal weight given to each of the requirements detailed above. Within these requirements, the committee will balance consideration of applicant differences in 1) years of participation, 2) level of athletic and academic achievement, 3) essay quality, 4) leadership, and 5) comments from the coach s recommendation. Award: Each scholarship will be made payable to the student-athlete s account at the college, university, community college or trade school of choice (following confirmation of acceptance) via standard Hawk Athletic Boosters, Inc. accounting practices. The reserves the right to modify this application at any point prior to the beginning of a season.
