NOTICE TO CM/GC Companies Facilities and Construction Management Jefferson County School District R-1 (Jeffco) is pre-qualifying Construction Manager/General Contractors for CM/GC services for a new K-6 school facility located in the Candelas Development within the City of Arvada, CO. Project: New K-6 School at Candelas Construction Budget: To Be Determined. Anticipated Capacity: 625 Students Construction Start: Anticipated April 2016 Approx. SF: 60,000 Construction Final Acceptance: Aug 1, 2017 Services Required: CMGC Services Architect: Hord Coplan Macht To Pre-Qualify: 1. Mail or hand deliver one (1) hard copy and one (1) DVD or flash-drive with an Adobe.pdf file of all information in hard copy of the completed pre-qualification packet to ensure it is received no later than 4:00PM, Friday, October 23, 2015. See CM/GC Request for Qualifications for format and information required. 2. This Notice is for a specific project. All submittals will be reviewed and evaluated and candidate CM/GC teams will be selected for a short-list or interview process; a single CM/GC will be selected from the short-list or interview process. This is a pre-qualification process only; no pricing or fee information is requested at this time. 3. CM/GC REQUEST FOR QUALIFICATIONS A. Instructions to the Statement of Qualifications 1. Interested CM/GC Companies are invited to submit a comprehensive Statement of Qualifications (SOQ) by responding to the items below. Responses must be in sufficient detail to provide for a thorough evaluation and assessment. 2. Information presented in the SOQ should conform to the following: a. A letter of introduction and interest. b. The SOQ shall be limited to a maximum of forty (40) single-faced pages of information. c. Information submitted must be arranged in the same order in which it is requested in section B Submittal Components. d. Submittals are to be bound or stapled. e. Font size is not to be less than 10 point. B. Submittal Components 1. Cover - The cover will show the following: Company s name Company s address State 'CM/GC Statement of Qualifications and the date of the submittal Name of Project: New K-6 School at Candelas The name of the person(s) authorized to make representation for the firm and that person's address and telephone number. 2. Letter of introduction and interest 3. Business Organizational Information in the format identified as Organizational Information included in this Request for Qualifications. 4. Company information, including principals and proposed staffing and organizational chart with duties/roles for the project. Include all proposed Consultants, if utilized, and their roles, including key personnel that will be assigned to the project. 5. If a joint venture is proposed, indicate the scope of work to be done by each company and the address of each company directly responsible for the performance of the work identified by phases, if appropriate. Indicate where and how coordination, management, organization, quality control and pre-construction services will be performed between joint venture partners. 6. If the Company has multiple offices, identify the role(s) of each office and which proposed staff members are at each office. 7. Resumes of personnel listed in the Company information that will have an active role in the project. 8. Relevant Projects - Examples of K-12 type projects of similar size, scope and equivalent in value. Utilize the format of the "Examples of Relevant Projects" form included. 9. Information on the Company s pre-construction services, including scheduling, specific expertise and proposed roles and responsibilities to the Architect and Owner. 10. Experience with CM/GC projects including types of projects, philosophy on management of sub-contractors, self-performed work, and responsibilities and coordination on maintaining GMP pricing during the construction phase(s). Include

project delivery information with multiple bid packages and phased construction. Discuss experience with GMP established at DD and CD phases. 11. Additional, special, or unique experiences pertinent to project and/or qualifications - Identify any special or unique experience and/or qualification of the Company that should be considered in the pre-qualification process. telephone number, name of project, brief project description, the name of the architect and telephone number, location of project, original contract amount, current contract amount, percentage of completion, and scheduled completion date. 11.0 Resumes: Provide on separate sheets of paper, resumes/experience for key staff in your firm including project managers, superintendents, estimators, schedulers or others that will work on Jeffco projects. For Project Manager and Superintendent include how long employee has been with your firm and project reference contact information for past five years projects. 12.0 Provide a list of individuals in your company who have received training in Stormwater Management Planning. Attach a copy of the certificate for each individual. 13.0 Does your firm have a company-wide safety program? 13.1 Is a copy of the safety program available to Jeffco for examination? 13.2 Attach a sample site specific safety plan. 13.3 Does your firm have a safety officer? If so, attach a resume. 14.0 What is your current experience modification rate (EMR) for Worker's Compensation? 15.0 Does your firm provide a bona fide health benefit & retirement plan for your employees? 15.1 What percentage of your current employees is covered by your plan? 16.2 Is a copy of your health benefit and retirement plan available to Jeffco for examination? 16.0 Does your firm have a pre-hire criminal background program? 17.0 Does your firm have a pre-hire drug-testing program? 17.1 Does your firm require that subcontractors working on your project have a pre-hire drug-testing program? 17.2 Does your firm have a policy to insure a drug free work place? 17.3 Is a copy of the procedure to enforce this policy available to Jeffco for examination? 18 Does your firm participate in an affirmative action program? 18.1 Is a copy of the Affirmative Action Program available to Jeffco for examination?
