1 A. Instructions for the Request of Qualifications 1. Interested professional services are invited to submit one (1) comprehensive Statement of Qualifications (SOQ) by responding to the items below. Responses must be in sufficient detail to provide for a thorough evaluation and assessment. 2. Information presented in the SOQ should conform to the following: a. A letter of introduction and interest. b. The SOQ shall be limited to a maximum of forty (40) single-faced pages of information. c. Information submitted must be arranged in the same order in which it is requested in section B Submittal Components. d. Submittals are to be bound or stapled in upper left-hand corner. e. Font size is not to be less than 10 point. The font is to be Times New Roman A pre-qualification committee will be formed to review submittals and to qualify firms for further consideration according to their capabilities and experience. The Committee reviews all materials for each firm and completes a qualification for further consideration assessment sheet. References are checked focusing on quality of work, maintenance of schedule and budget control. Once Firms are selected to be qualified for further consideration list the District will shortlist firms to participate in the solicitation phase for specific projects. Development of the shortlist is based on qualifications including: specific requirements of the project, past performance (based off performance evaluations), the firm s capacity to complete jobs of similar size, and the current workload of the firm. f. Note Just because a Firm is considered for further consideration does not guarantee that they will be shortlisted to submit bids or proposals for specific projects. g. Firms selected to be qualified for further consideration will be posted on the Denver Public Schools website at the link below. 20%201).xls B. Tab 3 If a joint venture is proposed, indicate the type of work to be done by each firm and the address of each firm where it will be performed. Indicate where and how management, organization, quality control and design coordination will be performed between joint venture partners or prime consultant and sub-consultants. Tab 4 Resumes of personnel listed in the firm information. Tab 5 Relevant Projects - Examples of projects of less than $5 million in value. Complete the "Examples of Relevant Projects" form at the end of the SOQ. One of these should be completed for each project. (Limit of 5) Examples of projects over $5 million in value. (Limit 3). (Please see attached form please make additional sheets so you can put 1 project per sheet) Tab 6 Experience with different project delivery methods and phased construction. Tab 7 Experience with site-based management teams (Design Advisory Groups) at K-12 schools. Special or unique experiences pertinent to project and/or qualifications - Identify any special or unique experience and/or qualification of the firm. If more than one office, define the relationship of roles for each office. Tab 8 Additional information which the firm feels may be appropriate for evaluation.
