The Patient Protection and Affordable Care Act (ACA) called for the creation of a government-run health insurance exchange in each state to serve individuals who do not have employer-based insurance. The public insurance exchanges launch Oct. 1, 2013. Each state was invited to launch its own health insurance exchange, and for states that did not wish to do so, the federal government will launch and maintain an exchange for them. Health insurance exchanges are web-based marketplaces that allow consumers to compare health plans, enroll and pay premiums. They are a significant part of the health reform law, and the primary way in which the law attempts to create greater accessibility to health insurance and greater transparency about insurance products. Public exchanges are designated as either State-Sponsored or Federally Facilitated or as State-Federal Partnerships in instances where the state did not want the total responsibility but wanted some input or intends to take over the exchange functions from the federal government in the future. AIS s Health Directory and Factbook also covers private-sector exchanges, which have been around for several years and have proliferated as a result of the interest created by the new law. The book features directories of both public and private exchanges, as well as the government officials, personnel, experts and vendors that have joined to implement this huge transformation in the way health insurance is sold. The ACA also created the Small Business Health Option Program () to serve the small group market. The feature mandate was delayed until 2015 for federally facilitated exchanges, but most states are offering a feature in their exchanges starting Oct. 1, 2013. Throughout spring and summer of 2013, insurers applied for and contracted with state exchanges some even using the exchanges as an opportunity to enter new markets. Previously, states had selected Essential Health Benefit benchmark plans, which carriers used to design their products and set premiums based on the metal-tier actuarial designations required by the law. States and insurers began

negotiating rates, tweaking plan designs and awarding contracts plans that will cover individuals from the uninsured and small group markets for the 2014 plan year. The following table lists the names of insurers and the public exchanges in which they will participate. While many large insurers will participate in the new marketplaces, several big names indicated reluctance to offer products, and some withdrew after submitting applications. Aetna, for example, by mid-september 2013 had withdrawn from five public exchanges, citing concerns about financial viability. The book provides details of the decision making processes and challenges that these plans (and other stakeholders) faced in bringing these new plan offerings to this brand new market. The CD edition of the full product provides details on many of the contracted carriers, including plan names, tier designations and premium rates. This is a partial list of carrier participation decisions and contracts for the 2014 enrollment season that starts Oct. 1, 2013. The list reflects contracted health plans as well as those that have applied to state exchanges.
