Structured Home Visits Asthma or Diabetes 1. Participant completes a baseline call with primary health coach (PHC) 2. PHC schedules a home visit based on the following triggers: recent utilizations new medication delivery devices need for an environmental assessment difficulty with telephonic communications 3. In-home specialist completes assessment Respiratory Therapist or Certified Diabetes Educator family members encouraged to participate review of participant s problem solving skills understanding of self-management plan assessment of living environment physical assessment of participant; spirometry screening 4.
