Terms and Conditions Governing United Overseas Bank Limited ( UOB ) Free Travel Insurance with UOB MasterCard Cards Campaign ( Terms and Conditions ) 1. Campaign 1.01 This Campaign is open to all UOB MasterCard Cardmembers who comply with the Terms and Conditions, commencing from 11 August 2014 to 28 September 2014, both dates inclusive (the Campaign Period ). 1.02 Notwithstanding anything herein to the contrary, the Bank has the discretion at any time and from time to time to determine the eligibility and/or ineligibility of any UOB MasterCard Cardmember for the Campaign and shall not be obliged to make any payment or compensation whatsoever to UOB MasterCard Cardmembers rendered ineligible for participation in the Campaign nor provide any explanation for such ineligibility. 2. Participation in the Campaign 2.01 (a) An Eligible MasterCard Cardmember will be entitled to the Free Travel Insurance if he/she complies with all the following:- (i) The Principal MasterCard Cardmember must charge at least S$800 (the Qualifying Amount ) at Singapore Airlines to the Eligible MasterCard Card account during the Campaign Period (the Eligible MasterCard Cardmember ). Amount(s) charged by Supplementary MasterCard Cardmembers will be taken into consideration for the same minimum spend threshold stated above. Only posted Singapore Airlines transaction(s) will be considered; AND (ii) Successfully activate the Free Travel Insurance coverage himself/herself by completing the travel coverage activation form via uob.com.sg/mcinsurance (the Activation Site ) any time before his/her travel date(s). Travel date(s) must be between 18 August 2014 to 1 January 2015, both dates inclusive (the Travel Period ). Successful travel coverage activation via the Activation Site will be prompted with a pop up message for the Eligible MasterCard Cardmember. (b) Upon successful travel coverage activation, the Eligible MasterCard Cardmember will be covered with a UniTravel Single Trip Plan. (c) Notwithstanding the fact that the MasterCard Cardmember has already completed the travel coverage activation and furnished all required information, the MasterCard Cardmember will not be entitled to the Free Travel Insurance if he/she does not comply with any of the Terms and Conditions. 2.02 By submitting travel coverage activation form, the UOB MasterCard Cardmember consents to the disclosure of information including, but not limited to, the UOB MasterCard Cardmembers credit MasterCard Card number to United Overseas Insurance Limited, independent telecommunication authorities or service provider or such other third party which is engaged by the Bank for the Campaign. The UOB MasterCard Cardmember authorises the Bank to disclose any information regarding the UOB MasterCard Cardmember and the UOB MasterCard Cardmember s relevant Eligible MasterCard Card account to such parties for the purposes of the Campaign. 2.03 Voluntary cancellation or termination of supplementary Eligible MasterCard Card account(s) will not by itself disqualify an otherwise Eligible MasterCard Cardmember from the Campaign. 3. Free Travel Insurance 3.01 Each Eligible MasterCard Cardmember shall be entitled to a maximum of five (5) Free Travel Insurance covers given that he/she has made the corresponding number of Singapore Airlines transaction(s) of the Qualifying Amount to his/her Eligible MasterCard Card. 3.02 Eligible MasterCard Cardmember MUST travel in the Trip along with or without his/her Family Members for the Free Travel Insurance to be effective.
3.03 The Free Travel Insurance shall cover Eligible MasterCard Cardmember and/or his/her Family Members. 3.04 The Free Travel Insurance; UniTravel Single Trip Plan policy shall only cover for the Trip(s) that commence from and end in Singapore within the Travel Period. Each Trip should not exceed thirty (30) days. 3.05 Once Eligible MasterCard Cardmember has satisfied Clause 2, the Free Travel Insurance is deemed to be effective. 3.06 Coverage and benefit limits of the Free Travel Insurance is as shown in Appendix A. 3.07 The Free Travel Insurance is underwritten by the United Overseas Insurance Limited ( UOI ). 3.08 In the event of a claim, Eligible MasterCard Cardmember consents to disclose or provide relevant information or document(s) to UOI and its service providers. 3.09 The Free Travel Insurance is not transferable or exchangeable for cash, credits, other gifts or otherwise in full or in part or refundable. The Bank assumes no liability or responsibility and shall not be responsible or liable for any injury, loss, claim or damage whatsoever or for any charges, costs or expenses of any kind whatsoever suffered or incurred as a result of the redemption or usage of the goods and services or the Free Travel Insurance or in connection with the Campaign and/or redemption or use of the Free Travel Insurance and any consequences including, but not limited to, loss of life, injury to person and/or loss or damage to property arising from or in connection with the Campaign, and/or use of the Free Travel Insurance howsoever arising. 3.10 Eligible MasterCard Cardmembers can download the UniTravel Single Trip certificate of insurance issued in conjunction with the master policy in the name of United Overseas Bank from the travel coverage Activation Site. 3.11 All Qualifying Amount(s) must be charged to the account of the Eligible MasterCard Card of the Eligible MasterCard Cardmember. The Bank is not responsible for any failure or delay in the transmission of the transactions by any party including but not limited to acquiring merchants, merchant establishments, or any telecommunication provider. 3.12 The Bank reserves the right to determine at its discretion whether: (a) an Eligible MasterCard Card is valid; (b) an Eligible MasterCard Cardmember has met all the requirements of the Campaign; and (c) charges made to the Eligible MasterCard Card qualify as Qualifying Amount(s) for the purposes of the Campaign. 3.13 If a Qualifying Amount is cancelled or reversed after the Campaign Period is over and the total amount spent during the Campaign Period falls short of the Qualifying Amount, the Eligible MasterCard Cardmember will not be considered to have spent the Qualifying Amount. 4. General 4.01 (a) The Merchant is not a participant in or sponsor of the Campaign. The Merchant of the goods or services may impose conditions for redemption and use of the goods or services or the Free Travel Insurance offered in the Campaign. (b) The Bank is neither the agent nor the principal of the Merchant and makes no representation as to the quality, condition or performance of the goods and services or the Free Travel Insurance provided. Any dispute about quality, condition, service standard or performance of the goods or services or the Free Travel Insurance is to be resolved directly between the Eligible MasterCard Cardmember and the Merchant. 4.02 (a) The Bank shall not be liable if it is unable to perform its obligations under the Terms and Conditions, due directly or indirectly to the failure of the email vendor, the telecommunication authorities or service provider or such other third party which maybe engaged for the Campaign, any machine or communication system, industrial dispute, war, Act of God, or anything outside the control of the Bank.
(b) The Bank shall not be responsible or liable:- for any failure or delay in the transmission or receipt of:- (i) evidence of sale transactions by MasterCard acquiring merchants, merchant establishments, MasterCard, postal or telecommunication authorities/provider or any other parties which may result in a charge incurred or made by the Eligible MasterCard Cardmember or the applicable supplemental MasterCard Cardmember being omitted from being posted to the Eligible MasterCard Cardmember s account and/or captured in the Bank s system during the Campaign Period; (ii) the emails to the Eligible MasterCard Cardmember which may result in the email not being received by the Bank and therefore the Eligible MasterCard Cardmember being omitted from participating in the Campaign; or (iii) the breakdown or inability to access the Activation Site or any delay in the transmission or receipt of any SMS; (iv) for any late posting of the Retail Purchases or for any failure in the Retail Purchases being transacted by the Eligible MasterCard Cards or being captured in the Bank s system; (v) for any notice or communication, email or free gifts (if any) which gets lost, misplaced, defaced, tampered with, stolen, damaged or misdirected; or (vi) for any breakdown or malfunction in any computer system or equipment. 4.03 Notwithstanding anything in the Terms and Conditions, the Bank may at any time vary, modify, add or delete any of the Terms and Conditions, including but not limited to terminating or withdrawing the Campaign or amending the duration of the Campaign without giving any reason, prior notice and/or assuming any liability to any party and shall not be liable to pay any compensation or enter into any correspondence in connection with the same and all MasterCard Cardmembers shall be bound by these amendments or variations. 4.04 The Bank s decision in all matters arising out of or in connection with the Campaign is final and conclusive. The Bank shall not be obliged to enter into any correspondence with any persons and no appeal, correspondence or claims will be entertained. 4.05 By participating in the Campaign and completing the Activation Process, each MasterCard Cardmember agrees to be bound by the Terms and Conditions. If there is any inconsistency between the Terms and Conditions and the UOB Cardmembers Agreement, the Terms and Conditions prevail only to the extent of such inconsistency. 4.06 The Free Travel Insurance is provided by the Merchant, under such terms and conditions determined by the Merchant, and therefore, the Bank will neither be responsible or liable for any defect, the quality, merchantability, the fitness for purpose or any other implied term or condition or any other aspect with respect to the Free Travel Insurance. The Bank also assumes no liability or responsibility for the acts or defaults of the Merchant. The Bank will not be responsible or liable for any injury, loss or damage suffered as a result of the redemption or usage of the goods or services or the Free Travel Insurance provided by the Merchant or arising from or in connection with the Campaign howsoever arising. 4.07 A person who is not a party to the Terms and Conditions and/or any agreement governed by the Terms and Conditions shall have no rights under the Contracts (Rights of Third Parties) Act (Cap. 53B) of Singapore to enforce any term of such agreement or any of the Terms and Conditions. 4.08 The Terms and Conditions are governed by the laws of Singapore and all MasterCard Cardmembers participating shall be deemed to have agreed to submit to the exclusive jurisdiction of the courts of Singapore. 4.09. All information is correct at the time of publishing and the Bank makes no representation or warranty whether express or implied, and accepts no responsibility or liability for its completeness or accuracy. In the event of any inconsistency or discrepancies between these Terms and Conditions and any advertising, Campaignal, publicity and other materials relating to or in connection with the Campaign, the Terms and Conditions shall prevail.
5. Definitions 5.01 Activation Period refers to the period commencing 11 August 2014 to 1 January 2015, both dates inclusive. 5.02 Activation Process refers to the activation process described under Clause 2.01(a)(ii) above. 5.03 Activation Site refers to uob.com.sg/mcinsurance 5.04 Bank or UOB refers to United Overseas Bank Limited and its successors in title. 5.05 Campaign refers to the Free Travel Insurance with UOB MasterCard Cards Campaign Campaign. 5.06 Campaign Period refers to the period commencing 11 August 2014 to 28 September 2014, both dates inclusive. 5.07 Disqualified MasterCard Cardmembers shall mean any MasterCard Cardmember, including Eligible MasterCard Cardmembers, who are disqualified from participating in the Campaign including those set out below:- (a) any MasterCard Cardmember whose single Eligible MasterCard Card account and/or multiple Eligible MasterCard Card accounts, as the case may be, is/are voluntarily or involuntarily suspended, cancelled, closed or terminated anytime during the Activation Period and Campaign Period; (b) whose Eligible MasterCard Card account(s) is/are not active, valid, subsisting or in good standing or which are otherwise determined by the Bank as being delinquent or unsatisfactorily conducted for any reasons as may be determined by the Bank at its sole discretion; (c) who are deemed mentally unsound by Order of the Courts of Singapore, facing legal incapacity or is incapable of handling their affairs, deceased, insolvent, bankrupt or have any legal proceedings or investigations by any government authority (or any threat of the same) of any nature instituted against them; or (d) who the Bank may decide to exclude, at its discretion, without prior notice at any time during the Campaign Period. 5.08 Eligible MasterCard Card shall mean an existing MasterCard credit card on personal liability which is: (a) issued by the Bank in Singapore; and (b) owned by the Eligible MasterCard Cardmember, BUT shall exclude any Corporate Business/Purchasing credit/debit MasterCard Cards issued by the Bank. 5.09 Eligible MasterCard Cardmember shall mean a UOB MasterCard Card Cardmember who has complied with all the conditions set out under Clause 2.01(a) above. 5.10 Family Members shall mean the following: (a) the Eligible MasterCard Cardmember's legally married spouse who has not been legally separated or divorced from the Eligible MasterCard Cardmember; and (b) all the Eligible MasterCard Cardmember's legally dependent children including step-children and legally adopted children, each of whom is: under the age of eighteen (18) years or up to twenty-three (23) years on his/her next birthday; studying full-time in a recognised institution of higher learning; unemployed; and unmarried. 5.11 Free Travel Insurance refers to UniTravel Single Trip Plan policy underwritten by UOI as shown in Appendix A, provided that Travel Insurance can be redeemed by the Eligible MasterCard Cardmember in accordance with the Terms and Conditions. 5.12 Insured Person means the Eligible MasterCard Cardmember or any one of his Family Members as defined herein. 5.13 MasterCard Cardmember shall mean any existing and new principal and supplementary holders of an Eligible MasterCard Card, whose MasterCard Card account(s) is/are valid, subsisting, in good standing and satisfactorily conducted as determined by the Bank in its discretion. 5.14 Merchant refers to United Overseas Insurance Limited.
5.15 Qualifying Amount refers to a minimum sum of Singapore Dollars Eight Hundred (S$800) worth of Singapore Airlines Transaction(s), charged either to the account of the Eligible MasterCard Card and/or to the account of the applicable supplementary Eligible MasterCard Card during the Campaign Period. 5.16 Retail Purchases means all transactions made by a MasterCard Cardmember using the Eligible MasterCard Card or the applicable supplementary Eligible MasterCard Card. 5.17 UOB MasterCard Cardmember shall mean a principal MasterCard Cardmember or supplementary MasterCard Cardmember but shall exclude MasterCard Cardmembers:- (a) who hold any Business Debit MasterCard Cards, Corporate MasterCard Cards (not on personal liability), Purchasing MasterCard Cards, Empire World MasterCard Card, Platinum Business MasterCard Cards, Distribution MasterCard Cards and Business Platinum MasterCard Cards; and (b) whose Eligible MasterCard Card accounts have been suspended, cancelled or terminated (excluding voluntary cancellation or termination of MasterCard Card accounts where there are more than one (1) accounts and at least one (1) account remains) for any reason during the Activation Period. 5.18 UOB Cardmembers Agreement refers to the prevailing UOB Cardmembers Agreement which is available at uob.com.sg/personal/uob Cards/credit/tnc.html 5.19 Terms and Conditions means the terms and conditions relating to the Campaign as may be amended from time to time. 5.20 Trip means the period commencing from the time you leave your place of residence or business to proceed directly to the place of embarkation in Singapore and ceases when you return to your place of residence or business or three (3) hours upon your arrival in Singapore after clearing the Immigration or the expiry of the period of insurance shown on the Certificate of Insurance, whichever is the earlier. Subject to maximum duration of not more than thirty (30) days per Trip for Single Trip Plan. 5.21 UniTravel Single Trip Plan policy refers to a certificate of insurance issued in conjunction with the master policy in the name of United Overseas Bank, where one Trip commences between 18 August 2014 to 1 January 2015 and ends by 31 January 2015. Each Trip should not exceed thirty (30) days. Eligible MasterCard Cardmember must travel in the Trip with or without his/her Family members for Single Trip Plan to be effective. 5.22 Except where the context otherwise requires, words denoting the singular include the plural and vice versa.

Follow-up Medical Expenses Incurred Upon Return to Singapore We will reimburse you for the necessary follow-up medical, hospital and treatment expenses reasonably incurred by you in Singapore within 31 days after your return to Singapore. 10,000 per Family

Trip Curtailment We will reimburse you for the proportional return of irrecoverable prepaid costs if it is necessary for you to curtail your Trip due to strike/riot, Serious Injury/Illness or hijacking of the Scheduled Public Conveyance in which you are traveling on. 5,000 per Family 7. Travel Delay If the Scheduled Public Conveyance that you had arranged to travel is delayed due to industrial action, adverse weather conditions or mechanical breakdown for at least 6 consecutive hours from the time specified in the itinerary, 500 per Family Limit for each full 6 consecutive hours 100

Baggage Delay We will pay you a cash benefit if your checked-in baggage is delayed for more than 6 consecutive hours due to misdirection. 500 per Family Limit for each full 6 consecutive hours 100 10. Terrorism We will indemnify for loss arising from terrorism excluding any act of terrorism involving the use or release or the threat of any nuclear weapon or device or chemical or biological agent.
