COMPREHENSIVE HEALTH and WELLNESS BENEFITS No Deductible Medical Plan Harris Health pays around 90% of your total monthly premium rate for the Employee Only level of coverage. 100% in-network coverage for most outpatient labs, x-rays, and surgical facility fees. 100% in-network coverage for outpatient rehab services at Quentin Mease, Ben Taub, and LBJ hospitals. 100% in-network coverage for a basic annual hearing exam. 100% in-network coverage for a medically necessary ambulance. 100% in-network coverage for medically necessary durable medical equipment. 100% in- network and out-of-network outpatient hospice care. 100% in-network coverage at Diabetes America facilities. No copay for office visits or preferred diabetic supplies, such as test strips, lancets, and syringes. $20 in-network copay for Primary Care Physician (PCP) visits with open access to in-network specialists without PCP referrals. $25 in-network copay for convenience care clinics for outpatient emergency care visits. As low as a $3 minimum copay for generic retail (30-day) prescriptions at in-network pharmacy locations. As low as a $6 minimum copay for mail order (31-90 day) prescriptions. Disability and Retirement BENEFITS SHORT-TERM DISABILITY (STD) NO COST benefit to eligible employees on approved FMLA who are unable to perform the essential functions of their jobs for a consecutive period of time due to a temporary, non-work related illness or injury. Long-TERM DISABILITY (LTD) FREE benefit offered to all full-time actively-at-work employees after a one-year waiting period. This benefit also offers access to NO COST programs and services to stay healthy and focus on the job. Life Insurance - Automatic enrollment at NO COST to all actively at work benefit eligible employees at two times base annual salary. - Additional life benefits can be purchased at low cost group term rates: Employee Optional Life, Spouse Life, Dependent Life, and Employee Optional Accidental Death & Dismemberment (AD&D) or Optional Family AD&D. 401(k) and 457(b) plans Harris Health offers simultaneous participation in two defined contribution plans, which allows employees to double their tax-deferred retirement savings compared to most other employers. - 401(k) with $1 for $1 employer match up to 5% of eligible compensation. - All eligible employees can contribute up to the annual IRS limit in each plan. - In 2013 the elective contribution limit is $17,500 and $5,500 for catch-up contributions under each plan. LIFESTYLE and FINANCIAL BENEFITS TUITION REIMBURSEMENT Harris Health supports employees who are in pursuit of a degree or licensure and who are seeking to qualify for an identified job classification within our own facilities. PAID TIME OFF (PTO) - Full and part-time employees accrue PTO hours as of first hours worked. - PTO Non-exempt 8.0 hours bi-weekly accrual, thereafter increasing incrementally to a maximum of 1040 hours. - PTO Exempt Approved Job Classifications 9.538 bi-weekly accrual, thereafter increasing incrementally to a maximum of 1040 hours. - Employees may accrue up to 1040 hours. - Accrued hours in excess of 240 may be sold. FLEXIBLE SPENDING ACCOUNTS (FSAs) Use an FSA to bring home more of your paycheck. Set aside pre-tax dollars for: - Qualified dependent care expenses under a Dependent Care FSA. - Qualified medical, dental and vision expenses under a Health Care FSA. Harris Health also offers the use of a FREE debit card for qualifying expenses. MOTHER-FRIENDLY WORKSITE Harris Health provides private rooms for lactating mothers to express and store milk during the workday. Employee Assistance PROGRAM (EAP) FREE to all employees to assist in handling personal problems concerning family, finances, health, emotional stress including: adoption assistance, childcare, daily living services, identity theft recovery services, K-12 education, legal referrals, estate planning tools, and much more. DISASTER RELIEF PROGRAM FREE individual employee assistance may be available to full-time employees after a 90-day probation period for documented financial losses resulting from a disaster. A disaster may include death of a spouse or dependent child when no insurance is available, illness of an employee, spouse or dependent child when resources have been depleted, or a natural disaster such as fire, flood, or tornado resulting in property damage.
