Section I Curriculum A. Are the program Mission and goals congruent with faculty beliefs and reflected in the curriculum of the undergraduate program, graduate program, and post-master s programs? B. How do the curricula of programs reflect commitment to a culturally, racially and ethnically diverse community and to socially responsible standards of professional nursing? C. Is the organizing framework consistent with the program philosophy and purposes? D. Is the organizing framework operationalized to unify and sequence the curriculum? E. Do the level and summative objectives provide direction for achievement of program objectives? Criteria - Curricula of undergraduate and graduate programs are consistent with the mission and goals of the Division of Nursing (defined as Philosophy and Organizing Framework). Method for Collection - Annual faculty retreat - faculty review philosophy and framework and affirm beliefs. Process/Decision Responsibility - DA. Criteria - Consistency of program philosophy, organizing framework with selected standards of professional nursing and reflective of diversity. Method of Collection - Review standards and reaffirm their appropriateness. Faculty review course evaluations yearly. Process/Decision Responsibility - UGCr, GCr, SNCr, UGC, GC, PEC, DA. Criteria - Consistency of program philosophy, purposes and organizing framework. Method of Collection - Annual curriculum review meeting. Process/Decision Responsibility - UGCr, GCr, SNCr, PEC, DC. Criteria - Organizing concepts are used to select and organize content and learning experiences. Method of Collection - Mapping of curriculum threads, key concepts, assess course sequence. Reviewed by UGC and GC. Process/Decision Responsibility - UGCr, GCr, SNCr, UGC, GC, PEC. Criteria - Achievement of program objectives through progressive level and summative objectives. Method of Collection DC reports quarterly to DA. Process/Decision Responsibility - UGCr, GCr. Time Yearly.

F. Are individual courses effectively implementing the program s philosophy, purposes, and organizing framework and professional nursing standards? G. Do learning experiences provide an opportunity for students to achieve program and course objectives? H. How does the master s program build on knowledge and competencies of baccalaureate nursing education to provide for attainment of advanced knowledge and practice of nursing? Criteria - Course objectives are attainable and implement the organizing framework and level and summative objectives. Learning experiences provide for achievement of course objectives. Course content and learning experiences reflect course objectives, are at appropriate level of difficulty, reflect current and anticipated trends in professional nursing standards and practice as well as area diversity. Number and type of teaching strategies are appropriate to objectives, content, and level of students, and are consistent with program philosophy. Evaluation methods promote achievement of course objectives and are consistent with program philosophy and nursing standards. Method of Collection - Clinical and course evaluations. Summarized by faculty along with implications and plans for improving courses and resolving problems as well as results of efforts to solve problems and improve outcomes. Summaries reported to Program Evaluation Committee. Process/Decision Responsibility PEC reviews data. Gives data to appropriate curriculum committee: UGCr, GCr, UGC, GC, SNCr,DA, Individual faculty. Time - Every three years and as appropriate with course changes and/or identified problems occur. Criteria - Course content and learning experiences implement program and course objectives. Method of Collection - Agency evaluations, student, course, and clinical evaluations. Summarized by faculty along with implications and plans for improving courses and resolving problems as well as results of efforts to solve problems and improve outcomes. Summaries submitted to Program Evaluation Committee. Process/Decision Responsibility - PEC, Individual faculty. Time - Every three years and as appropriate with course changes and/or identified problems occur. Criteria - Evidence of advanced knowledge and practice. Method of Collection - Course faculty and Graduate Program Coordinator define desired advanced practice and knowledge outcomes and assess course descriptions, objectives, and learning experiences for abilities to achieve desired outcomes. Alumni and employer surveys assess competence in advanced practice. Results of assessment reported to Program Evaluation Committee along with implicated remedial plans. Process/Decision Responsibility - GCr, SNCr, GC, Individual faculty. Time - Every three years and as appropriate with course changes and/or when identified problems occur. Section II Required Outcomes A. Critical Thinking - Are student skills evident according to Division definition? B. Therapeutic Nursing Interventions - Are students abilities evident according to the Division definition of expected outcomes? C. Communication Abilities - Are students abilities in written, oral, and nonverbal Criteria - Evidence, including summary narratives, course exams, student exemplary papers, ATI and NCLEX. Method of Collection Faculty report to the Program Evaluation Committee, ways in which they facilitate skill development according to the Division definition and expected level skills. Faculty describes plans for changes/improvements when indicated by findings. Reports of findings and of follow up are submitted at specified time

communication evident as described in outcome behaviors? D. Graduation Rates - Are admission criteria adequate for predicting ability to achieve in the program? E. Employment Patterns - What are our graduates employment patterns? Are our graduates prepared to function? intervals. Process/Decision Responsibility - GCr, UGCr, PEC, SNCr, Individual faculty. Criteria - Students admitted to the program are able to achieve program objectives according to the curriculum plan. Students of varying ethnicity are able to achieve in the program. At-risk students are identified and assisted as necessary and as resources are available. Method of Collection - Tracking of progression of students through all programs. Course faculty report failures to coordinators (RN, LVN, UGC, GC). The demographics of students who fail and/or stop out is examined by UGC, GC, and DA for implications for changes in the programs. Admission supplementary criteria examined related to changes in student and client demographics. Process/Decision Responsibility - UGCr, GCr, SNCr, PEC, DA. Time - Progression data collected every semester summarized and reported annually. Criteria - Baccalaureate graduates can function as generalists, masters graduates with a specialized focus at a post baccalaureate level as seen in State Board examinations, employer and alumni feedback. Method of Collection - Analysis of State Board Examination results. Summary report of findings submitted to PEC and DA. Assess appropriate time intervals for evaluating this outcome. Process/Decision Responsibility- PEC, SNCr, DA. Time - Annual License Exam (BRN) reviews (NCLEX exams), Survey at graduation. Employer focus surveys every 3 years. Community Advisory meeting every year. Section lii Students Are student policies, current, published, available, nondiscriminatory, and followed by faculty? Criteria - Student policies are published and available to students and faculty. New policies are made available to current students. All policies are in accordance with those of the University. Polices are nondiscriminatory. Faculty operate by the policies. Method of Collection - UGC and GC examine student handbooks for currency, accuracy, non discrimination. Process/Decision Responsibility - When either committee adds/revises policy, students are advised. UGC, GC, UGCr, SNCr, GCr, SA, DA. Time- On-going and every two years along with catalog changes. Section IV Faculty A. Is performance of nursing faculty consistent with program purposes and program professional standards? Criteria - Compliance with evaluation criteria for retention, tenure and promotion Method of Collection - Evaluation of lecturers; evaluation of probationary faculty for retention or tenure; evaluation of faculty requesting promotion; and evaluation of tenured faculty. WPAF, TFEC: Self report and peer report. Process/Decision Responsibility - FA-D-RTP, D C,/School

Dean, School RTP, University President. Time - Every five years for tenured faculty, annually at request for promotion. B. Are academic and experiential qualifications of faculty consistent with professional standards and university expectations? C. Do faculty vita reflect appropriate community service? D. Do faculty vita reflect appropriate teaching excellence, scholarship, and university services? E. Do faculty vita reflect continuing faculty development? Section V Resources A. Is the nursing skills lab designed and operated to support the instructional goals of the program? B. Do agencies utilized for clinical experiences support the instructional goals of the undergraduate program and the graduate program? C. Are the library resources adequate for needs of students and instructional needs of faculty? D. Are technology services and equipment adequate to support the instructional requirements of faculty? Criteria - Appointment criteria, consistent with BRN, NLN, Univ. Method of Collection - Record review. Process/Decision Responsibility - Appointment committee. Time - Annually in search process. Criteria - Appropriate community service as defined by the Division service variables. Method of Collection - Faculty reports reviewed by Faculty Affairs, report to PEC, DC. CV and WPAF yearly. Process/Decision Responsibility - FA, DC. Time - Every year. Criteria - Adequate access to students for independent activity. Sufficient equipment and supplies to allow demonstration and return demonstration. Adequate space for independent student activity. Adequate amounts and variety of up-to-date materials and supplies. Method of Collection - Student and faculty input. Reviewed by UGC. Process/Decision Responsibility - Instructional assistant(s), course faculty, UGC, DA, DC. Time - Annually and on-going as needed. Criteria - Standards of practice are congruent with professional nursing practice. Presence of positive role models for professional nursing practice. Adequate census and staff: patient ratios. Periodic planned communication between faculty and agency staff. Objectives of the course achievable in the setting. Method of Collection - Observation and discussion with agency staff. Regional planning meetings and other agency meetings. Report from faculty using Faculty Evaluation of clinical agency and Collaborative Faculty-Agency Evaluation. Student clinical evaluation of agencies. Faculty provide summary report to the PEC. PEC makes summary recommendations for change/remediation to UGC, GC. Process/Decision Responsibility - PEC, DA, UGC, GC, SNCr, DC. Time - On-going with new agencies and at one year intervals to coincide with course evaluation. Criteria - Annual additions to the collection as needed to meet learning and instructional needs. Method of Collection - Assessment of holdings by library coordinator and librarian. Faculty offer ongoing requests at UGC meeting. Process/Decision Responsibility - UGC, GC, UGCr, GCr, SNCr, DA, Library Coordinator. Time - Ongoing Criteria - Adequacy of equipment and services, including computing and other technology. Adequate production services and assistance in development and effective use of

media and materials. Method of Collection - Faculty assessment and report to UGCr, graduate faculty to GCr and SNCr. Process/Decision Responsibility - Instructional assistant, course faculty, UGCr, GCr. Time - Annually E. Is the media collection sufficient to meet the instructional goals of the program? F. Do student support services function effectively and promote attainment of program purposes? G. Do program support services function effectively and promote attainment of program purposes? H. Are the physical facilities adequate to meet the needs of the program? Criteria - Adequacy of collection. Method of Collection - Faculty assessment and undergraduate report to LC, graduate faculty to GC. LC/GC report to UGC and GC. Process/Decision Responsibility - Instructional assistant, course faculty, LC, UGC, GC. Time - Every three years and on-going. Criteria - Adequacy of student services. Method of Collection - Progression survey. Analysis of survey results by PEC with follow up recommendations on findings. Process/Decision Responsibility - PEC, DC, DA. Time - Every three years and on-going. Criteria - Quality, quantity of work production is satisfactory. Adequate support of scholarly development of faculty. Method of Collection - Faculty input at annual meeting. Process/Decision Responsibility - DA. Criteria - Needs for current and projected student enrollment and number of faculty and staff. Method of Collection - Institutional planning process, faculty discussion at annual meeting. Process/Decision Responsibility - DA. Section Vl Structure and Governance A. Is the chairperson carrying out responsibilities at a high level of performance and productivity? B. Does the governance/organizational structure of the program function effectively and promote attainment of program purposes? C. Do program coordinators function effectively and promote attainment of program purposes? Criteria - Accomplishment of goals and performance objectives of chairperson in key areas of responsibility. Method of Collection - Performance evaluation tool completed by faculty, chairperson s self-evaluation. Process/Decision Responsibility - DA, Chairperson s Administrative Advisory Committee. Time - Every three years during the second year of threeyear chairperson term (08, 11, 14, etc.) Criteria - Efficient policy development process. Clearly defined roles and responsibilities. Involvement of faculty, Chairperson, students, and staff in policy formation. Method of Collection - Committee minutes, annual faculty meeting. Process/Decision Responsibility - DA, UGC, GC, Administrative Advisory C, DC. Time Every three years and on-going. Criteria Meet program goals and purposes? Method of Collection - Survey of faculty and DC. Process/Decision Responsibility - DC. Time Every three years at time of Chair s evaluation.

D. Are policies written, implemented and nondiscriminatory? Criteria - Policies of the University, School and Division are written and implemented as written. They are non discriminatory. Method of Collection - Policies are published, operationalized and support educational equity-provide evidence. Process/Decision Responsibility - DA, UGC, GC, Chair, Affirmative Action Committee. Time Every three years and on-going. Section Vll Overall Program Evaluation A. Does the overall program evaluation adequately address all program components? B. Are program evaluation components addressed according to plans? C. Are program evaluation findings used for development, maintenance, and revision of the program? Criteria - All state and national accreditation criteria are addressed in the program. DC Method of Collection DC and PEC assess plan and make recommendations for changes/improvements. Process/Decision Responsibility - DC and PEC. Time Annually and as needed for accreditation changes. Criteria - All elements of the program evaluation plan are implemented according to timelines, or reasons for changes are noted. Method of Collection - PEC facilitates and assesses implementation of program elements. PEC assures data collection, either through program coordinators or directly, and submits a summary of findings and recommendations resulting from findings. Report to DC and PEC. Time Annually or as indicated in the Overall Program Evaluation Plan. Criteria - Data from Program Evaluation is used by DC and DC s designees, such as Program Coordinators, for brainstorming, leading to development, maintenance, and revision. Method of Collection - PEC facilitates implementation of activities through communication with DC. Process/Decision Responsibility - PEC, UGCr, GCr, SNCr, UGC, GC, DC. Time Annually and as needed.
