Dear Student: Dear New or Returning Emory Undergraduate, Graduate or Professional Student: Welcome to Emory University. I hope that your time here will be happy, healthy and always intellectually stimulating. As you know, Emory University requires all degree-seeking and international students to either have health insurance or purchase the University-sponsored plan. In order to waive enrollment in the Emory-sponsored plan, a student must show evidence by August 24, 2011 of enrollment in a United States domiciled health insurance plan that meets waiver criteria. The waiver web site opened on April 27, 2011. In order to meet this requirement, you must either enroll in the Emory Student Health Insurance Plan (offered by Aetna Student Health, with a 2011-12 annual premium of $2,360) or successfully complete the waiver process on-line at www.opus.emory.edu. If you do not waive out of the Emory Student Health Insurance Plan by July 1st, you will be automatically enrolled in the Emory/Aetna plan for 2011-12 and billed by Emory Student Financial Services. However you will still have until August 24th to complete a waiver and have the insurance enrollment and charge reversed. Please note that the 2011-12 Aetna Student Plan includes 100% payment of all covered expenses (after $25 specialist and $50 ER co-payments) at Emory Healthcare out-patient and in-patient facilities. The Plan will pay benefits in accordance with any applicable Georgia insurance law. The Emory University Student Health Plan may not cover all your health care expenses. The plan excludes coverage for certain services and contains limitations on the amounts it will pay. Please read the Emory University brochure carefully before deciding whether this plan is right for you. While this document and the Emory University brochure tell you about some of the important features of the plan, other features may be important to you and some further limit what the plan will pay. If you want to look at the full plan description, which is contained in the Master Policy issued to Emory University, you may view it at Emory University Student Health Insurance office or you may contact us at (877) 261-8403. This plan will never pay more than $250,000 in a coverage year and no more than $2,000 for prescription drugs. Additional plan maximums may also apply. Some illnesses may cost more to treat and health care providers may bill you for what the plan does not cover. Where Can I Go for Service? First Stop, Student Health Services. When you need care, consider Emory University Student Health Services (EUSHS) as your first stop. They can provide many of the routine health services you need. If you need care they can t provide, they ll refer you to a doctor or other health care provider who belongs to Aetna s Emory Core or Preferred Provider* networks (doctors, specialists and facilities). You also may visit any licensed health care provider directly for services as long as you receive a referral from EUSHS first, except for specific Plan restrictions on certain services. However, when you visit Student Health Services first, you ll generally pay less out of your own pocket for your care. How And When Do I Enroll in the Plan? All degree-seeking and International Students who do not submit proof of comparable coverage through the Online Waiver system by the deadline date and who are automatically charged for the insurance, will have an effective date of coverage as indicated in Coverage Effective Dates. If you have other coverage and wish to waive enrollment in the Student Health Insurance Plan as listed above, please submit proof of comparable coverage through the Online Waiver system by the waiver deadline dates of August 24, 2011 for Fall Semester, January 19, 2012 for new students enrolling for the Spring Semester, and May 19, 2012 for new students enrolling for the Summer.

To Waive Online: Login to OPUS (https://www.opus.emory.edu) using your Network ID and password First time users should select Obtain Network id and Password and follow the prompt. A completed waiver must be submitted by the deadline date. Waiver Deadline Dates Fall August 24, 2011 Spring/Summer January 19, 2012 Summer May 19, 2012 Waiver submissions may be audited by Emory University, Aetna Student Health, and/or their contractors or representatives. You may be required to provide, upon request, any coverage documents and/or other records demonstrating that you meet the school s requirements for waiving the student health insurance plan. By submitting the waiver request, you agree that your current insurance plan may be contacted for confirmation that your coverage is in force for the applicable policy year and that it meets the school s waiver requirements. Please make sure you understand your school s credit hour and other requirements for enrolling in this plan. Aetna Student Health reserves the right to review, at any time, your eligibility to enroll in this plan. If it is determined that you did not meet the school s eligibility requirements for enrollment, your participation in the plan may be rescinded in accordance with its terms. Aetna Natural Products and Services discount program: Access to reduced rates on services from participating providers for acupuncture, chiropractic care, massage therapy and dietetic counseling. While only your doctor can diagnose, prescribe or give medical advice, the Informed Health Line nurses can provide information on more than 5,000 health topics. Contact your doctor first with any questions or concerns regarding your health care needs. Discount programs and other programs above provide access to discounted prices and are NOT insured benefits.
