SELF-EVALUATION ECCLESIAL LAY MINISTRY PROGRAM Diocese of Lafayette-in-Indiana Applicant: Directions: Please read each statement and evaluate this statement based on the following levels of satisfaction: A = I am very satisfied (which does not mean that there is no room for continued growth). B = I am satisfied (which means the area is not a problem to you but, on the other hand, you don t feel it s as good as you d like it to be). C = I am dissatisfied (which means that, in all honesty, you feel that things could be significantly better in this area). D = I am very dissatisfied (which means that a good deal of improvement is needed in this area).
