The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA). The plan has contracts with Life Insurance Company of North America and Wellspring Family Services EAP to pay life insurance, AD&D and EAP claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2013 were $272,778. request. The items listed below are included in that report:

For Group Health Benefits for Faculty, Admin. & Staff This is a summary of the annual report of the Group Health Benefits for Faculty, Admin. & Staff, EIN 91-0565006, Plan No. 505, for period January 01, 2013 through December 31, 2013. The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA). Seattle University has committed itself to pay certain wellness program claims incurred under the terms of plan. The plan has a contract with Group Health Cooperative to pay health and prescription drug claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2013 were $3,976,118.

For Washington Dental Service Dental Plan for Faculty, Administration and Staff This is a summary of the annual report of the Washington Dental Service Dental Plan for Faculty, Administration and Staff, EIN 91-0565006, Plan No. 506, for period January 01, 2013 through December 31, 2013. The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA). The plan has a contract with Life Insurance Company of North America to pay long-term disability claims incurred under the terms of the plan.
