SUMMATIVE EVALUATION REPORT (Library Media Specialist) Library Media Specialist: School: Date: STANDARD I - The Library Media Specialist competently administers and manages the Library Media Center. 1. The LMS establishes and maintains an environment in which students and staff can work at a productive level. 2. The LMS demonstrates in selection, acquisition, circulation and maintenance of materials and equipment, keeping the needs of diverse students in mind. 3. The LMS prepares statistical records and reports needed to administer the LMC. 4. The LMS administers budgets according to needs and objectives of the LMC. s For Office Use Only: GCN-AF8.BNT Page 1

STANDARD II - The Library Media Specialist actively participates in the teaching/learning process. 5. The LMS plans and implements the LMC program of library media skills, using varied instructional strategies to address the diversity of the learners. 6. The LMS collaboratively supports classroom teachers in their instructional units. 7. The LMS promotes the development of reading skills and reading appreciation. 8. STANDARD III - The Library Media Specialist uses various forms of assessment to monitor the effectiveness of the Library Media Center program. 9. The LMS recognizes the role of information retrieval in the future of education. 10. The LMS solicits feedback from the Library Media Center (LMC) patrons to help in assessing the efficacy of the Library Media Center staff. 11. STANDARD IV - The Library Media Specialist keeps current on instructional knowledge and seeks and explores changes in teaching behaviors that will improve student performance. 12. The LMS successfully engages in professional development activities consistent with the goals and objectives of the building, district and state. 13. The LMS engages in professional growth through membership in professional organizations and attendance at their conferences and workshops. 14. The LMS appropriately uses technology as an instructional tool, integrates technology into teaching and learning activities, continues to grow professionally in the application of technology in the classroom, and successfully completes the appropriate LMS evaluation according to the district technology assessment timeline.
