HOW TO: ROLE: CONTRACTOR SIGNATORY Contract Signature by Contractor You will receive email from DSHS communicating there is a Contract/Renewal ready to be signed. 1. Login to egrants in your role as the Contractor Signatory. 2. Select My Administration link from the top purple bar. 3. My Administration page is displayed. 4. Select Contractor esignature (under the Document Administration heading). 5. The Contract Signature Page is displayed. Page 1 Updated: 5/27/2015
6. Choose Contract Ready for Signature from the Signature Status drop down list. 7. Select the search button. 8. The signature search results are displayed that include all contracts ready for signature. 10. Before signing the contract, review the associated contract specific forms (e.g. budget) and complete any necessary declarations. You can do this by clicking on the Contract. Note: Screenshot above is a representation of the forms you will see for your renewal. The actual forms seen will change depending on the type of contract. You may choose to review the forms in all sections except Contract Execution Forms at this time. 14. In the Contract Execution Forms section, these are your actionable forms to enable the esignature process. The Contract Execution Section Forms include: Contract Declarations: o Contract Declarations must be completed. o Depending on the type of organization you are & type of contract that is being renewed, you will have at least 1 declaration (FFATA) and up to 5. o The declarations are no different than what you have completed in prior years. They are the same form in electronic format. If they apply to you, you complete the form and click-save. If they do not apply to you, select Not Applicable on the form and click-save. o Contract Declarations could include: Certification Regarding Lobbying Certification Regarding Professional Services Contracts Not Hiring Former or Retired State Agency Employees Child Support Certification FFATA Disclosure of Ownership and Control Interest Statement Family Planning Abortion Attestation Page 4 Updated: 5/27/2015

This will open a document that you may print. Contract Print: These are the details of the contract for the renewal you are signing. You may click on the link. This will open a document that you may print. 15. To esign the Contract, Select My Administration link from the top purple bar. 16. My Administration page is displayed. 17. 20. Choose Contract Ready for Signature from the Signature Status drop down list. 21. Select the search button. 22. The signature search results are displayed that include all contracts ready for signature. 23. Check the check box next to the contract(s) you are selecting to sign. 24. Click the CLICK HERE TO E-SIGN SELECTED CONTRACTS button. Save icon will display to confirm esignature. Page 6 Updated: 5/27/2015
