1 Adding Roles to Your COI Smart Profile Log in here: smart.com/login.php Your Login ID is your six digit Aurora Employee Number. If you forgot your password, please use the, Forgot Login ID or Password? link. Upon logging in you will see the Home Page of COI Smart. On the left hand side you will see your Menu Bar. Click on My Questionnaire, and then click on My Profile.
2 The next page you will see is your profile page. From here, you will add any additional roles you serve within Aurora Health Care. Additional roles include: Board DO NOT USE! This role is solely for our members serving on Aurora Health Care s Board of Directors. Caregiver Everyone within Aurora Health Care has this role automatically. Do not uncheck this role from your profile. Compliance DO NOT USE! This role is solely for individuals who work in Aurora Health Care s Compliance Department. Continuing Medical Education If you are participating in any continuing medical education presentations/courses within Aurora Health Care, you will want to add this role to your profile. HCCS Administration DO NOT USE! This role is solely for our vendor who created COI Smart. Human Subject Research If you are participating in any human subject research/clinical trials/research studies within Aurora Health Care and are considered a Key Personnel (or above) you will want to include this role. Malpractice Insurance Enrollee If you are a physician or certified registered nurse anesthetist (CRNA) you will need to include this role on your profile. PHS Funded Research If you are participating in any research funded by a public health service (PHS), you will need to include this role on your profile. *** Adding additional roles means you will have additional questions to answer that are specific to the department overseeing that activity. *** To add a role, click the checkbox in front of the role you will need. Do not touch anything in the Departments field. These are pre set for you. Once you are done, click, SAVE & GO TO MY QUESTIONNAIRE
3 After you click your questionnaire, wait for the page to load, and the questions in your questionnaire will appear at the bottom of the page, in order. (Pictured below)
4 Some of the questions will say Complete under the column, Status. These are questions you have already completed. However, if you have additional roles within Aurora Health Care throughout the year such as Human Subject Research with the Insitutional Review Board (IRB), or Public Health Service funded research with the Sponsored Programs Office, Continuing Medical Education or dislosures for Malpractice Insurance Coverage (Physicians and CRNA s only) you will have additional questions that you will need to answer to satisfy those specific department s Conflict of Interest reporting requirements. Those additional questions will apear with the word, Pending under the status column. (Pictured below) You have the option to Revise previously answered questions if you have an update in your disclosures. If you simply have additional questions you need to complete, you can click on Answer next to those specific questions. To complete your additional questions, please click Answer to complete the questions with the status of Pending.
5 At the end of your questionnaire, a new window will pop up asking for you to confirm your updated responses and to type revise in the text box to submit your revisions. (Pictured below) Once you type revise in the red textbot and push the red button, Submit Revision to this Question you will have the following screen pop up. If you click on the Adobe PDF icon, you can save a hard copy/printable version of your complete questionnaires & disclosures to keep for your own records.
