EVALUATION PLAN OVERVIEW 2013-2014 PROBATIONARY TEACHER Teacher Responsibility Complete Self-Assessment; due by August 23rd Goals - Develop goals for instruction, student achievement and PUSD outcomes; due by September 27th First Semester Administrator Responsibility A minimum of 2 classroom observations A minimum of 1 scheduled observation (see definition of scheduled observation) Mid-Year Review (see definition of Mid-Year Review) Teacher Responsibility Complete reflection for each goal: instruction, student achievement, and PUSD Exit Outcomes; due no later than May 16 th Complete Data Component; date to be announced All items are to be completed prior to December 1. ACCESSING THE PERFORMANCE REVIEW SYSTEM 1. From any computer or ipad go to the Peoria Unified School District public home page, click on Staff Links, then choose Performance Review. NOTE: The Performance Review System can also be accessed from the Employee Portal home page (left menu). 2. Log in using the same username and password that you use to logon to a district computer. Important to keep in mind: while the Performance Review System can be opened on any device that has access to the Internet, (ipad, workstation, laptop, home computer, etc ) it has a built-in Auto Save that saves approximately every minute. This means you should NOT have the performance review system open on multiple devices at the same time. Therefore, ALWAYS close the browser when you are finished working in the system so that it does not over-write and cause you to lose your work. On the ipad, that means closing the tab, using the x in the upper left corner of the tab. IMT Technology Services & Training 3
The Performance Review System will appear slightly different on mobile devices. When accessing from an ipad, you will not see the graphic banner that is seen when accessing from a PC. However all content will be the same. PC View ipad View Note: The links on the navigation bar reflect the tools you will be using. HOME 1. The system opens with the Home page displayed. 2. The home page displays your evaluator and the status of your self-assessment and goals. Click the Go There Now link that is located after Self- Assessment or Goal to open the item. NOTE: You can also access your self-assessment and goal page by selecting the My Performance navigation link. MY PERFORMANCE Your My Performance page is where you will record your personal Self-Assessment and Goals. After your evaluator has completed your personal observations or evaluations, you can view this data in your My Performance section by clicking the My Performance tab. SELF-ASSESSMENT 1. Click the Self-Assessment tab to bring the form forward for completing. 2. Select a rating for each domain component and enter a summary for each domain into the domain s summary text box. Click the Information icon to view the description for each component s rating. NOTE: If your evaluator has advised you that a summary for the domain is not needed, enter an NA into the summary text box. 3. The Performance Review System will autosave every minute, however, it is strongly recommend that you occasionally select to Save while working in the system and that you ALWAYS select to Save before exiting the online system. You will find the Save option at the bottom of the page. 4. When you have completed your Self-Evaluation form, click the Finalize Self Assessment button to submit to your evaluator. NOTE: A message will appear if you have not completed all Self-Assessment fields. Click the OK button on the message to return to the Self-Assessment page and complete. IMPORTANT: Saving (auto or manual) will not process if you have lost your Internet connection. If you lose your Internet connection while working in the system, do not close the system or exit the page, leave the page open while you restore your Internet connection. Immediately after your connection is restored, click the Save option to save your recent changes. Spell Check: There is no Spell Check tool in the Performance Review System, however you may type your entries into a Word document, utilize Spell Check, and then copy and paste your entries into the corresponding Performance Review System text field. GOALS 1. Click the Goals tab. 2. Review the information at the top of the Goals page. 3. Enter a Goal for Student Achievement. 4. Enter Indicators of Success for your Student Achievement goal. 5. Click the New Indicator button to enter new text fields for additional Indicators of Success, as needed. NOTE: You must include a minimum of 2 indicators of success. 6. Enter a Goal for Instruction. 7. Enter Indicators of Success for your Goal for Instruction. 8. Click the New Indicator button to enter new text fields for additional Indicators of Success, as needed. NOTE: You must include a minimum of 2 indicators of success. 9. CHECKING GOAL STATUS After you have finalized your goals, you can return to the Goals page to check the status of your goals. Waiting for Response will appear before your evaluator has reviewed your goals. After your evaluator has reviewed your goals, Approved or Not Approved will appear as assigned by your evaluator. If your evaluator has not approved one of your goals, the goal text field will open for editing again. With guidance from your evaluator, re-enter your goal and again select to Finalize Goals. OBSERVATIONS & EVALUATIONS After your evaluator has completed your observation or evaluation report, you can click the Observations or Evaluations tab to view your reports. HISTORY 1.
