Appendix 4: Guidelines for Prescribing and Administering Drugs: A midwife may prescribe and administer the following substances in accordance with the guidelines approved by the Board. This list indicates the routes of administration and where appropriate, dosage limits for drugs which may be prescribed and administered by a midwife. Antibiotics for prophylactic treatment of Group B streptococcus: Drug Route: Dosage: Penicillin G IV 5 million units initial dosse; then 2.5 million IV every 4 hours Cefazolin IV 2g initial dose then 1g every 8 hours Clindamycin IV 900 mg every 8 hrs (no pharmacy substitution on dose) Erythromycin IV 500mg every 6 hrs until delivery Vancomycin IV 1g every 12 hrs Antibiotics for treatment of Mastitis: Cloxacillin PO 500mg QID X 7-10 days Cephalexin (Keflex) PO 500mg QID X 7-10 days Antibiotics for treatment of Simple Cystitis and Asymptomatic Bacteriuria: Amoxicillin PO 500mg Q8H x 7 days Ampicillin PO 500mg q6h X 10 days Cefexime (Suprax) PO 400mg daily X 7 days Cephalexin (Keflex) PO 500mg q 12 h X 7 days Macrobid PO BID X 7 days Macrodantin PO 50mg QID X 7 days Antifungal agents considered safe in pregnancy and for newborns: Nystatin Local Clotrimazole local Miconazole Local Fluconozole (Diflucan) PO Mother 400 mg as initial dose then 100 mg BID Baby 6 mg/kg initial dose then 3 mg/kg daily

Antiviral drugs for prevention of genital herpes: Acyclovir (Avirax, Zovirax) PO 800 1200 mg per day in divided doses such as 200 mg QID or 400 mg TID. Initiate at 36 weeks gestation and continue until birth. Valacyclovir Hydrochloride (Valtrex, Apo-Valacyclovir) PO 1000 mg daily Carboprost (Hemabate) IM/IMM.25mg q 15 min to a maximum of 2.0 mg Antiemetic Dimenhydrinate (Gravol) IM 25-50 mg Q3-4 h Doxylamine succinate-pyridoxine PO 2 tab @ hs, 1 tab in am and 1 tab at mid-day hydrochloride (Diclectin) Promethazine (Phenergan) IM 25 mg Q3-4 h Antihistamine Diphenhydramine hydrochloride IM 10-50 mg Domperidone (Motilium) PO Up to 40 mg QID Entonox Inhalant PRN; patient administered Epinephrine hydrochloride: Pediatric 1:10,000 solution ET or IV.1 mg/kg given rapidly, may be repeated in 3 minutes if no response Adult 1:1,000 solution IM.3 mg into the lateral thigh Ergometrine maleate IM.25 mg Q2-4h Erythromycin ophthalmic ointment topical Folic Acid PO According to current recommendations Hepatitis B immunoglobulin IM According to client indicationis Hydrocortisone topical 2

Oxytocin: IM 10IU IV 5IU bolus for prophalaxis; IV 20-40 IU of oxytocin may be placed in 1000 ml of Normal saline or Ringer s Solution and given at a rate sufficient to maintain uterine tone. Drug: Route Dosage: Intravenous fluids IV Normal saline or Ringer s solution; and 2/3 Dextrose, 1/3 normal saline Lidocaine hydrochloride with or without epinephrine local infiltration Misoprostal (cytotec) rectally 800-1000µg PO 400-600 µg Naloxone (Narcan) IM/IV.1mg/kg Phytonadione (Vitamin K) IM 1mg RhD immune globulin IM/IV --300 µg at 28 weeks for prophylaxis against Rh isoimmunization in the Rh negative woman where the father is Rh positive on PRN for antepartum bleed -- 300 µg postpartum if indicated by results of neonatal ABO typing and DAT Therapeutic oxygen inhalant 3

The following drugs may be carried by a midwife for emergency use and administered following consultation with a physician. i) Magnesium sulfate Intravenous/ Intramuscular ii) Calcium gluconate: 100 mg/ml Intravenous Prophylaxis against eclampsia requires 4 mg bolus followed by a solution which delivers 2 gm per hour thereafter. (20 mg/1000 cc in Ringer s solution at 100 ml/hour). calcium gluconate is given in 100 mg increments when necessary to reverse any toxic effect of magnesium sulfate iii) Hydralazine Solution (5-10 mg) according to blood pressure Intravenous / Intramuscular The following drugs may be obtained on a physician s prescription, relating to a particular client, and administered by a midwife. i) Analgesic - Meperidine Intramuscular injection 75-100 mg Q3/4 hourly - Morphine Sulfate Intramuscular injection 10-15 mg Q3/4 hourly - Fentanyl Intravenous 25-50mcg Dilute 2ml (100mcg) into 8ml N/S to obtain 10ml (10mcg/ml) May repeat q10min for total dose 200mcg/hr i) Acetaminophen with codeine (15-30 mg)/asa with codeine (15-30 mg) Oral 4
