B i o g r a p h i e s William Corr Deputy Secretary Department of Health and Human Services. On May 6, 2009, Bill Corr was unanimously confirmed by the Senate as Deputy Secretary of the Department of Health and Human Services. As Deputy Secretary, he is responsible for the operations of the largest civilian department in the federal government. Bill Corr s policy expertise and management experience will be invaluable as we work together to manage the Department and pass and implement comprehensive health reform, Secretary Sebelius said. Bill knows our department inside and out, and I look forward to partnering with him in the years ahead. Bill Corr most recently served as executive director of the Campaign for Tobacco-Free Kids. Previously, Corr served for 12 years as counsel to the U.S. House of Representatives Subcommittee on Health and the Environment. Additionally, Corr served as Chief of Staff for the Department of Health and Human Services. Corr is a graduate of the University of Virginia and the Vanderbilt University School of Law.

Mary Wakefield, Ph.D., R.N. Administrator, Health Resources and Services Administration Mary Wakefield, Ph.D., R.N., was named administrator of the Health Resources and Services Administration (HRSA) by President Barack Obama on February 20, 2009. HRSA is an agency of the U.S. Department of Health and Human Services. HRSA works to fill in the health care gaps for people who live outside the economic and medical mainstream. The agency uses its $7 billion annual budget (FY 2008) to expand access to quality health care in partnership with health care providers and health professions training programs. Dr. Wakefield joins HRSA from the University of North Dakota (UND), where she was associate dean for rural health at the School of Medicine and Health Sciences, a tenured professor, and director of the university s Center for Rural Health. As a nurse, a Ph.D., and a leading rural health care advocate, Mary Wakefield brings expertise that will be instrumental in expanding and improving services for those who are currently uninsured or underserved, President Obama said in announcing her appointment. Under her leadership, we will be able to expand and improve the care provided at the community health centers, which serve millions of uninsured Americans and address severe provider shortages across the country. Dr. Wakefield brings experience on Capitol Hill to her post at HRSA. In the 1990s, she served as chief of staff to two North Dakota senators: Kent Conrad (D) and Quentin Burdick (D). She also has served as director of the Center for Health Policy, Research and Ethics at George Mason University in Fairfax, Va., and worked on site as a consultant to the World Health Organization s Global Programme on AIDS in Geneva, Switzerland. Dr. Wakefield is a fellow in the American Academy of Nursing and was elected to the Institute of Medicine (IOM) of the National Academies in 2004. She served on the IOM committee that produced the landmark reports To Err is Human and Crossing the Quality Chasm. She also cochaired the IOM committee that produced the report Health Professions Education, and chaired the committee that produced the report Quality through Collaboration: Health Care in Rural America.

In addition, she has served on the Medicare Payment Advisory Commission, as chair of the National Advisory Council for the Agency for Healthcare Research and Quality, as a member of President Clinton s Advisory Commission on Consumer Protection and Quality in the Health Care Industry, and as a member of the National Advisory Committee to HRSA s Office of Rural Health Policy. At UND, Dr. Wakefield also was director of the Rural Assistance Center, a HRSA-funded source of information on rural health and social services for researchers, policymakers, program managers, project officers and the general public. In addition, the Center for Rural Health administered a $1.6 million award from HRSA under the Critical Access Hospital Health Information Technology Implementation program. Dr. Wakefield is a native of Devils Lake, N.D. She has a bachelor of science degree in nursing from the University of Mary in Bismarck and master s and doctoral degrees in nursing from the University of Texas at Austin.

B i o g r a p h i e s Tom Morris, M.P.A. Associate Administrator, Office of Rural Health Policy Health Resources and Services Administration Tom Morris is HRSA s associate administrator for rural health policy. In that role, he oversees the administrative and policy activities of the Office of Rural Health (ORHP) with its $174 million FY 2008 budget. HRSA works to fill in the health care gaps for people who live outside the economic and medical mainstream. The agency uses its $7 billion annual budget (FY 2008) to expand access to quality health care through an array of grants to state and local governments, health care providers and health professions training programs. Morris coordinates ORHP s response to policy issues, especially the impact of Medicare and Medicaid regulations on the rural health workforce and service delivery. As office director, he is responsible for 16 grant programs that focus on building rural health care capacity at the state and community level. Morris came to the federal government in 1996 as a Presidential Management Intern, which led to details on a Federal Intra-Agency Telemedicine Committee, in the U.S. Senate, and at the Centers for Medicare and Medicaid Services. Morris joined ORHP full time in 1998 and has worked in several capacities there, including his prior position as deputy director, and as policy coordinator and grants project officer. Prior to joining the government, he was a reporter and editor covering health care and other issues at two daily newspapers in North Carolina. Morris received his bachelor s degree in journalism from the University of North Carolina at Chapel Hill and a master s in public administration with a concentration in community health from East Carolina University.

B i o g r a p h i e s Rebecca J. Davis, Ph.D. Executive Director National Cooperative of Health Networks Rebecca is the Executive Director of NCHN and has served in this role since July 2006. Prior to this position, she served as the Executive Director of both the Virginia Rural Health Resource Center and the Virginia Rural Health Association. She was the founding Chair of the Council for Rural Virginia, was appointed to the Board of Trustees for the Center for Rural Virginia and serves as the Center s first Secretary. Dr. Davis has served on numerous boards related to rural health issues over the past eight years, including the Governor s Work Group on Rural Obstetrics; Advisory Board of the Rural Health Research Center at the University of Virginia; Virginia Tele-Health Network; Virginia Healthcare Workforce Advisory Committee; Virginia State Health Planning Grant Advisory Committee; Jefferson College of Health Sciences - Physician Assistant Program Advisory Board; and most recently the National Group of Rural Health Professional Membership Organizations Executive Directors. She has presented at both state and national meetings on rural health and rural development topics. Dr. Davis and her family resides in Hardinsburg, KY.

B i o g r a p h i e s Terry J. Hill, M.P.A. Executive Director, National Rural Health Resource Center Terry Hill has more than 25 years of professional experience working with rural health care providers and their communities. He is currently executive director of the National Rural Health Resource Center, based in Duluth, Minnesota. In that capacity, Hill leads five federally funded rural health initiatives and teaches management and health policy at the University of Minnesota Duluth Medical School and the college of St. Scholastica. He is the current president of the National Cooperative of Health Networks and former president of the Minnesota Public Health Association. Hill has spent the past dozen years working with state offices of rural health, state hospital associations and rural hospitals and clinics throughout the United States, and has made more than 100 presentations at state and national conferences during that time. He has also provided technical assistance in strategic planning, network and community development, quality improvement, health information technology and performance measurement. Hill founded and directed the Minnesota Center for Rural Health, the Northern Lakes Health Consortium, the Arrowhead Regional EMS Program and the Northeast Minnesota Telecommunications Network. These organizations collectively implemented eleven national demonstration projects in the areas of quality improvement, emergency medical services, community development, health network development, telecommunications and health provider recruitment and retention. Mr. Hill has authored numerous articles and papers on a wide range of rural health topics, has chaired or facilitated more than a dozen national meetings on rural health, and has testified before U.S. Congressional committees on seven occasions. Terry Hill was raised in a small town in central Alaska, and has had a longtime commitment to the well being of rural communities. He currently lives in Duluth, Minnesota, and is an avid runner, triathlete and cross country skier. 10

B i o g r a p h i e s Martha McLeod, M.O.E., R.D., L.D. Executive Director North Country Health Consortium, Inc. Ms. McLeod is the Executive Director of the North Country Health Consortium, Inc, (NCHC) a not-for-profit rural health network located in Northern New Hampshire. As Executive Director, she provides leadership to member agencies for collaboration and integration of health and human services to improve the health status of the residents of New Hampshire s North Country. Since joining the network in 1999, Ms. McLeod has worked with state, federal and private foundations to provide member agencies with the resources and technical assistance to carry out community needs and health status assessments, collaborative planning opportunities, grant writing, project management, workforce education and training, local public health capacity enhancement, and community-based disease prevention and health promotion activities. Ms. McLeod s past experience includes working at the community level providing nutrition services to young families and for the NH Division of Public Health in the administration of federal nutrition programs. Martha McLeod is a former member of the New Hampshire House of Representatives representing Grafton County District 2. She was assigned to the House Finance Committee where she served as the Vice Chair of Division III, working on the Health and Human Services budget. She previously served on the House Commerce Committee and chaired the Task Force on Expanded Health Coverage. Ms. McLeod was a candidate for the NH Senate from District 1 (North Country) in 2008. Ms. McLeod is on the Executive Boards of the NH Health Plan, NH s high risk pool and the New England Telehealth Consortium; and serves as the Governor s appointment to the Technology Advisory Board of the Department of Resources and Economic Development. Ms. McLeod holds a Bachelor of Science (BS) in Nutritional Science and a Master of Occupational Education (M.O.E.) in Adult and Vocational Education from the University of New Hampshire. She is a Registered Dietitian licensed to practice in New Hampshire. 11

B i o g r a p h i e s Chris Tilden, Ph.D. Director, Bureau of Local and Rural Health (BLRH) Kansas Department of Health and Environment Chris Tilden, Ph.D. is the Director of the Bureau of Local and Rural Health (BLRH) in the Kansas Department of Health and Environment. He joined the state health department in 2001 as the Program Director of the State Office of Rural Health and was promoted to bureau director in 2003. As Director of BLRH, Dr. Tilden is has administrative oversight of the state office of rural health, state primary care office, a statewide 330-funded Migrant Farmworker Health Program, public health workforce and leadership development programs, and the state trauma program. He also serves on the Kansas Primary Care Collaborative Coordinating Committee, the Kansas Health Information Technology Technical Working Group, and the Kansas Rural Health Options Project. Nationally, Dr. Tilden serves on the board of the National Organization of State Offices of Rural Health and the federal Department of Transportation s National Emergency Medical Services Advisory Committee. He currently serves as the chair of the Joint Committee on Rural Emergency Care, a collaboration of NOSORH and the National Association of State EMS Officials He has served as a grant reviewer and as a chair of grant review panels for HRSA and other federal grant programs. He is an invited speaker at many state and national conferences and serves on the adjunct faculty of the Department of Health Policy and Management at the Kansas University School of Medicine. 12

B i o g r a p h i e s Alan Morgan Chief Executive Officer Phone: 202-639-0550 E-mail: morgan@nrharural.org Alan Morgan joined the NRHA staff in 2001. He serves as a policy resource for the national media, as well as a presenter at both national and state health conferences. He has more than 17 years experience in health policy development at the state and federal levels. He has worked for former U.S. Congressman Dick Nichols and former Kansas Gov. Mike Hayden. Alan s health policy articles have been published in multiple health journals. Alan earned an M.P.A. from George Mason University and a B.S. in journalism from University of Kansas. 13

9:00 a.m. 10:15 a.m. Independence BCDE Keynote Address Title: Motivational Interviewing and UB-PAP (Ultra-Brief Personal Action Planning): A Comprehensive Approach to Patient Activation This plenary presents a unified model for encouraging adaptive patient behavior change. Clinicians learn to initiate dialogues with patients about maladaptive behavior by relying on the three core questions of UB-PAP. For patients who do not or cannot develop action plans by themselves, clinicians can selectively back-fill the intervention by utilizing the 5As of self-management support as well as core interventions from the motivational interviewing repertoire. Dr. Cole will discuss the conceptual integration of self-management support, stage of change theory, and motivational interviewing. He will demonstrate UB-PAP and the audience will practice using the tool in small groups.
