INSTRUCTIONS FOR ONLINE OPEN ENROLLMENT Open Enrollment runs from November 2 through November 20, 2015, with changes effective January 1, 2016. This year you can newly enroll in or change your medical, dental and/or vision coverage and/or enroll in Flexible Spending Account(s) through MyBentley. This step sets up the necessary payroll deductions for coverage. If you want Flexible Spending Accounts for 2016, you must enroll in those accounts online via MyBentley. No separate FSA form is necessary. If you do not make changes to your medical, dental and/or vision plan, your current plan(s) will automatically be continued for 2016. ACCESSING ONLINE OPEN ENROLLMENT To access Online Open Enrollment, please follow the steps below: 1. Open a web browser and go to MyBentley at http://my.bentley.edu, then click the Sign In button. 2. Type your network shortname (i.e. ssmith) in the Username text box. 3. Type your network password in the Password text box, then Login. If you do not know your login information, click the Need help with your username and password? link and follow the prompts. 4. IMPORTANT STEP: Click the HR Information tab at the top of the page. Under the Faculty/Staff Self-Service link in the left-hand navigation area, click on Online Open Enrollment. This brings you to the Open Enrollment Summary page.

VIEWING BENEFITS COSTS At any time, you may view your monthly medical, dental, vision and/or Flex Spending costs by clicking the View Changes button on the Open Enrollment Summary Page. This opens the Open Enrollment Cost Detail page, which displays a comparison chart with the costs for the benefit(s) you currently have and those you are selecting for 2016 (please note these are monthly amounts), as well as a Net Change amount. HEALTH, DENTAL AND VISION If you do not make any changes to your medical, dental and/or vision plan, your current plan(s) will automatically be continued in 2016. If you do make changes, you must submit these changes online via MyBentley to set up the necessary payroll deductions as well as on a Harvard Pilgrim, Delta Dental and/or EyeMed Vision Care form (available in Human Resources, Rauch 201, from Sandy Smith or on the Bentley Open Enrollment webpage). Note: Changes for eligible domestic partners and their dependents must be processed through Sandy Smith. 1. To begin, click on Open Enrollment at the bottom of the Open Enrollment Summary page (or Restart to clear changes made to date). 2. To enroll in or change medical, dental or vision plans, click the Health link under Group to display your choices. If applicable, the name of your current plan(s) will appear (radio button is selected). 3. If you wish to add or switch plans or change your coverage type (Individual to Family or vice versa), select the desired plan, then click Change Dental, Change Medical or Change Vision. Make your addition/change by clicking in the appropriate "My Choice" column, then click the Submit Change button to return to your Group choices. The effective date of the new plan will be January 1, 2016.

4. If you wish to stop your current dental, medical or vision plan altogether, click Change Dental, Change Medical or Change Vision, then click the Stop Benefit button to return to your Group choices. 5. Repeat Steps 2-4 to make additional changes to your medical, dental or vision plan. 6. Click the Return to Open Enrollment Main Page button to return to the main page. 7. If you do not want to make any other plan changes or enroll in Flexible Spending Accounts, click the COMPLETE button. Your changes will not take effect unless you click the COMPLETE button. On the next screen, you will see a message stating "You have successfully completed the Open Enrollment process. Here is a summary of your selections." You will also receive an e-mail confirmation within 10 minutes stating you have successfully completed the Open Enrollment process with a summary of your selections. (Be sure to also complete the necessary paper enrollment forms.) FLEXIBLE SPENDING ACCOUNTS Existing Flexible Spending Accounts CANNOT be continued for 2016. Therefore, if you want Flexible Spending Accounts for 2016, you must enroll in those accounts online. NO SEPARATE FSA FORM IS NECESSARY.

To enroll in a Flexible Spending Account: 1. Click the Flex Spending link at the bottom of the Open Enrollment Summary page to display your Flex Spending Group choices. 2. Click the Dependent or Medical Flexible Spending Account link to display the Open Enrollment Choice Detail page. 3. Enter the total annual amount (minimum: $200; maximum: $2,500 for medical or $5,000 for dependent care) in the Annual Dollar Amount box. Do not enter a $ sign or comma. Your per pay deduction amount for each payroll will be automatically calculated. 4. Click the Submit Change button to establish your Flex Spending Account deduction. 5. Repeat Steps 2-4 to enroll in another Flexible Spending Account. 6. Click the Return to Open Enrollment Main Page button to return to the main page to make any other changes.

WHEN YOU ARE FINISHED When you are finished, click the Complete button on the Open Enrollment Summary page. Your changes will not take effect unless you click the COMPLETE button. On the screen you will see a message stating: "You have successfully completed the Open Enrollment process. Here is a summary of your selections." At this point, you may log out by clicking Exit in the top right-hand corner of your browser. You will receive an e-mail confirmation within 10 minutes stating you have successfully completed the Open Enrollment process with a summary of your selections. If applicable, you will receive a letter from Crosby Benefit Systems by mail confirming your Flexible Spending Account selection(s) for 2016. If this statement is incorrect, please contact Sandy Smith in Human Resources at (781) 891-2817 or ssmith@bentley.edu. NOTE: If you have a Crosby medical debit card, please retain it for continued use in 2016. STARTING OVER If at any time before Open Enrollment ends (November 20, 2015) you want to make additional changes to your benefits, return to the Open Enrollment Summary page and click the Restart button. NOTE: This will clear any changes made to date. You will have to make all elections again. NEED HELP?
