RENTAL APPLICATION GUIDELINES Sedona Elite Properties Management Inc. welcomes all applicants and supports fair housing. We do not refuse to lease or rent any housing accommodations or property nor in any way discriminate against a person because of sex, race, religion, familial status, handicap, color or national origin. $35.00 Application Fee Per Person Every adult, age 18 years and older, must personally fill out and sign an application if they are going to be a resident of the property. A valid government issued ID card is required for any Residents over 18yrs of age. A photocopy of such is required for each Resident. No application will be processed until the application is completed in full. Any questions that do not pertain should be marked N/A or marked as none. Leave NO blank spaces. Missing or incomplete information may delay or prevent verification, permitting another application to be approved before your application. Require Proof of Income All income must be lawful and verifiable. Acceptable forms of verification include, but are not limited to the following: o Employees: Request for Income Verification form completed by the employer, original pay stubs, not copies. Cash or letters from your employer are not verifiable income. o Self-Employed: IRS 1040 (first two pages of the last two year s tax returns) and Schedule C (profit and loss statement of the last two year s tax returns) o Direct Deposit: bank statements for the past two months, which must list the source of the deposit. o Child Support: filed stamped court order. Must show history of payment received for a minimum 12-month period. o Award letter. o Court Documentation. DISCLOSURE / AUTHORIZATION / CONSENT The undersigned declares that the information on this rental application are true and correct and that I have not knowling withheld any fact or circumstances, which would, if disclosed, affect my application unfavorably. Applicant(s) hereby verifies that falsification of any information provided on the rental application is grounds for rejection of this application before move-in. If falsification is discovered after move-in, the Lease and all Addendums will be terminated and collect from me any damages incurred including reasonable attorneys fees resulting there from. I authorize this verification with my signature, the investigation and release of all information on all statements contained herein, but not limited to, the release of all public records, credit history, criminal and rent history, employment history/verification and any other verification required by Sedona Elite Properties Management Inc. whether by fax, photocopy or original signature. I further authorize Sedona Elite Properties Management Inc. to inquire into my character, general reputation, personal characteristics and mode of living, and I release all concerned from any liability in connection with any information they give. I further understand that all persons or firms named may freely give any requested information concerning me and hereby waive all right to action for any consequences resulting from such information. I also understand and agree that Sedona Elite Properties Management Inc. will rely upon this Rental Application as an inducement for entering into a rental agreement or lease. Acceptance of this application and the Deposit to Hold in the amount of $ does not constitute the lease and the offer to lease is not binding until approved by Landlord. If your application is approved and you fail to enter into a rental agreement or fail to take possession under the terms of your rental agreement, if one has been signed, you understand and agree that the entire Deposit to Hold shall be forfeited as a liquidated damages. If your application is NOT approved you will receive a full refund of your Deposit to Hold. Sedona Elite Properties Management Inc. is in receipt of your Deposit to Hold for the purpose of securing the rental property located at and having it removed from availability, SEPMI will no longer advertise the property and will consider it as rented. If said property cannot be rendered to me as agreed upon due to circumstances beyond the control of Sedona Elite Properties Management Inc. I will not hold Sedona Elite Properties Management Inc liable in any way. This application is preliminary only and does not obligate Sedona Elite Properties Management Inc and/or Owners to execute a lease or deliver possession of the proposed premises. This application may be rejected by Sedona Elite Properties Management Inc. without reason or explanation. I further release Sedona Elite Properties Management inc., all persons, agencies, its principals, firms and/or the owner of any property which I am applying to occupy from any liabilities resulting from providing such information.
