II. Purpose of Addendum To approve a new full-day GSRP classroom To increase the capacity from 185 children to 197 children. III. Methodology 08/31/2015- Contact Document received- Modification request 09/01/2015- Inspection Completed-Env. Health:A 09/03/2015- Inspection Completed- Fire: C 09/15/2015- Inspection completed-onsite (additional equipment needed, repair of faucet) 09/18/2015 Contact Document received- (pictures/written statement of compliance. Request to proceed w/ 12 children instead of 16 for new room capacity. IV. Description of Findings and Conclusions The terms of the original license have been modified on August 30, 2006, September 17, 2007, September 3, 2008, October 29, 2009, July 16, 2010, August 20, 2010, September 25, 2012, and April 21, 2014 and September 18, 2015. This modification will approve the use of a new classroom #205 to approve a full day GSRP program. There is a restroom inside the classroom with one toilet and one sink. There is also an additional hand washing sink in this classroom. There are an additional total of 27 hand washing sinks, and 40 toilets which are available for use throughout the building. A limited scope lead risk assessment was completed August 28, 2009 which only included rooms 107, 100 wing hallway, 200 wing hallway, girls and boys restroom, office hallway, multi-purpose room, kitchen, and 200 wing playground. Ms. Parker, licensee designee was informed that a full lead risk assessment will be required for all approved child use classrooms, all child use areas including but not limited to the outdoor play area, and hallways by January 2, 2017. An Environmental Health inspection was completed on September 1, 2015 to approve the new GSRP room #205. An updated fire inspection was completed on September 14, 2015. Based on the violations found during the fire inspection, a temporary approval to continue to operate was granted until 11/3/2015. Ms. Parker is aware that the fire violations will need to be corrected and a re-inspection will be required prior to the 11/3/2015 deadline which is prior to the license expiration date of 12/21/2015. Ms. Parker is aware that the license will not be renewed if the center is not in full compliance with the fire safety provisions. 2

The following rooms and measurements are approved by the department and were used in determining the total capacity of the center: Room 100 is approved for children birth through 15 months of age. This room was previously measured at 728 square feet and is approved for 14 children based on 50 square feet per child. Although this space can accommodate 14 children, the maximum group size for this room is limited to 12 children per group. Room 101 is approved for preschool-age children 3 years through 5 years 11 months of age. This is a part-day program which operates MWF (4 year old program) and TTH (3 year old program). This room was previously measured at 728 square feet and is approved for 21 children based on 35 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 102 is approved for children 4 years of age. This is a part-day program which operates Monday-Friday from 9:00 a.m. until 12:00 p.m. This room was previously measured at 728 square feet and is approved for 21 children based on 35 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 103 is approved for toddlers 12 months through 2 ½ years of age. This room was previously measured at 728 square feet and is approved for 14 based on 50 square feet per child. Although this space can accommodate 14 children, the maximum group size for this room is limited to 12 children per group. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 104 is approved for the Discovery Preschool children ages 4 year through 5 years 11 months of age. This is a full day program which operates from 6:00 a.m. until 6:00 p.m. This room was previously measured at 720 square feet and is approved for 21 children based on 35 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 105 is approved for the Discovery Preschool children 2 ½ years of age through 3 years of age. This is a full day program which operates from 6:00 a.m. until 6:00 p.m. This room was previously measured at 720 square feet and is approved for 21 children based on 35 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 202 is approved for a part-day GSRP preschool program for children 4 years of age and older. This room was previously measured at 875 square feet and is approved for 25 children based on 35 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. 3

Room 203 is approved for a part-day GSRP preschool program for children 4 years of age and older. This room was previously measured at 756 (28x27) square feet which can accommodate 22 children based on 35 square feet per child. The licensee has requested to limit the capacity of this room to 12 children based on available sleeping equipment. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 205 is approved for a full day GSRP preschool program for children 4 years of age and older. This room was measured at 728 (26x28) square feet which can accommodate 20 children based on 35 square feet per child. The licensee has requested to limit the capacity of this room to 16 children. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room. Room 401 (formerly labeled music room) is approved for the school-age child care room for children 5 years of age and older. This room was previously measured at 1110 square feet and is approved for 32 children based on 35 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room at any one given time. The new total approved capacity for the center is 197 children at any one given time. The following rooms have been approved for child use, but were not used in determining the total capacity of the center: The Gymnasium is approved for large muscle activities only. This space was previously measured at 4,897 (59x83) which can accommodate 140 children, ages 2 ½ years of age and older at any one given time based on 35 square feet per child. This space can accommodate 97 children, younger than 2 ½ years of age based on 50 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room at any one given time. The cafeteria is approved as a multi-purpose room. This space was previously measured at 2,559 square feet and can accommodate 73 children, ages 2 ½ years of age and older at any one given time based on 35 square feet per child. This space can accommodate 51 children under 2 ½ years of age at any one given time based on 50 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room at any one given time. 4

The library is approved for child use. This space measured at 1100 (44x25) square feet which can accommodate 31 children at any one given time, ages 2 ½ years of age and older at 35 square feet per child. This space can accommodate 22 children 2 ½ years of age and younger at 50 square feet per child. If mixed age groups are combined in this space; ratio, maximum group size and capacity and based on the age of the youngest child in the room at any one given time. V. Recommendation I recommend modifying the license to approve room 205 and increasing the capacity from 185 children to 197 children ages birth through 12 years of age.
