International Undergraduate Application (see Appendix I) 2. U.S-China Joint DVM Program Application Cover sheets (see Appendix II) Copy of official TOEFL score report from the ETS website Copy of official General GRE report from the ETS website Copy of official transcripts for all college work (English and Chinese) One page (250 words) essay explaining why applicant desires to receive a CSC position One page (250 words) essay describing applicant s career goals Two teacher recommendation letters WES translation of the Chinese transcript (optional; if accepted, the candidate will have to provide the WES translation before June 1, 2016) Deadline for all materials: January 15, 2016 for mail and February 1, 2016 for electronic submission. Appendix I: International Undergraduate Application Thank you for your interest in attending! *** To apply for Fall 2016 semester, the application deadline is February 1, 2016 for the CSC-KSU prevet-dvm Scholarship Program. *** APPLICATION INSTRUCTIONS: The following is a list of the materials required when applying for International Undergraduate admission: 1. A completed K-State application. To apply on-line: https://www.k-state.edu/admit/apply/apply.html 2. An $80 non-refundable application fee must be included with the application. ***According to university policy, the Office of International Admissions will NOT be able to begin processing your application until the application fee has been paid.*** The application fee can be paid by personal check, bank draft, money order or credit card. The best way to pay application fee is on line when you apply. If you prefer to mail personal check or money order, the submission deadline is January 15, 2016. 3. Official transcripts, translated into English if necessary. Applicants should request official transcripts from all universities they have attended. Official transcript is generally sent directly from one university to the other, not passing through students. Official transcripts have the university seal on the transcript, itself, and should be mailed to the following address in an envelope with a seal over the back of the envelope. ***Please make sure your name is on ALL documents. We CANNOT accept scanned and emailed documents. *** The submission deadline is January 15, 2016 4.
