Request for Proposal RFP 15/16-1 Wallace Elementary/Middle School Playground Project 2015 December 11, 2015 The is currently soliciting request for proposals from qualified playground equipment companies with turnkey installation. The District is requesting proposals for playground equipment installed with fall surfaces. Proposals shall provide for three separate play areas primary, elementary and middle school. Each proposal shall clearly set forth proposed specifications. The District reserves the right to reject all proposals and to waive all informalities. A separate pricing schedule should be submitted including all costs associated with specifications including equipment, installation, warranty and training. A non-mandatory pre-proposal conference will take place Thursday, January 7, 2016 at 1:00 PM at Wallace Elementary/Middle School in Wallace, SC. Potential proposers are encouraged to attend. RFP 15/16-1 December 11, 2015 Page 2 A. Introduction The is seeking qualified providers and installers of school playground systems. This request for proposals establishes the specifications, terms and conditions governing the selection of a firm to provide those services to the. All submittals shall be in the form and format as specified in this RFP section entitled Proposal Format and Content. B. Project Descriptions Project location is in 3643 SC-9, Wallace, South Carolina. The playgrounds will be located at the newly constructed Wallace Elementary Middle School (WEMS). WEMS current population consist of around 400 students ranging from Pre-K through 8 th grade. The district is seeking three play areas with the following minimum sizes: Primary 50 X 60 Elementary 60 X 80 Middle 40 X 50 WEMS has been identified as the Fitness Magnet for. The district anticipates the award of the RFP for the prescribed services on February 1, 2016 with a following notice to proceed by February 11, 2016 with the remaining project time until substantial completion will be approximately 50 days. C. Summary of Scope of Work The services of the surface preparation and installation of equipment are required to be complete by April 1, 2016 so that students and staff can fully utilize playground after the 2016 Spring Break. District expects the project management team to be an extension of the District staff, protecting the District s interests while keeping the project on schedule and on budget
RFP 15/16-1 December 11, 2015 Page 3 The scope of the work for the playground equipment company for this project is as follows: Propose the appropriate age related equipment to meet the physical activity levels of primary, elementary and middle school children. Provide quality control and inspection services on the installation of the playground project through project completion. Propose a safe environment and design. Provide staff training within 1 week of completion of project. Report directly to the District s Deputy Superintendent. Confirm services related to final project close out has been completed and provide additional assurance of project completion. A qualified playground firm will be able to provide services listed above and identify any further services necessary to ensure a timely and cost-effective construction of the project. D. Required Proposal Format and Content A qualifying application must address all the following points, in the following order: 1. Understanding of and Approach to the Project a. Provide a summary of your approach to the project. Discuss activities during the installation process as appropriate. b. Provide an outline of in-house procedures to coordinate the work of project team members. c. Approach to technical reviews when required, equipment required for appropriate installation, development of installation alternatives, inspections, field inspections and industry standards. d. Provide a description of staffing to be used in project. Identify specific individual(s) that the District will interface with.

List of Comparable Completed Projects Provide a list and description of construction projects completed within the last three years of similar scope and complexity that your firm was an active participant in as Construction Manager and/or Inspector. 3. Experiences in Controlling Project Cost and Installation Schedule Provide a list and description of construction projects completed within the last three years indicating: a. Initial project budget. b. Installation start and completion dates. c. Name, address and telephone number of: 1. Owner representative who was directly involved during the construction phase. 2. Installation Manager of Record. 4. Project sketches and diagrams Provide project sketches and diagrams to scale for each playground area primary, elementary and middle school. Include clear specification on surface preparation, identifying type of material and depth. Include specifications on surface barrier materials. Identify model, manufacturer and other pertinent information related to equipment and corresponding durability. Provide specifications for each piece of equipment. 5. Statement of Overriding Qualifications Provide a definitive statement of qualifications and strengths the firm believes will convince the district that your firm is the best fit for this project.
RFP 15/16-1 December 11, 2015 Page 5 E. Pre Proposal Meeting and Site Visit A pre-proposal meeting will be held at 1:00 pm on Thursday, January 7, 2016 at the Wallace Elementary Middle School. A tour of the school site will be conducted following a question and answer period. Interested firms are encouraged to attend prior to submitting responses to the RFP. This meeting is not mandatory however due to limited staff no other site visits will be provided to proposers after this date. F. Basis of Award The award of the contract will be based on criteria and guidelines established by the District. The award will be recommended after a careful evaluation by the selection committee and will be based on demonstrated competence as well as the best interest of the District as determined by the committee. The successful firm must demonstrate and document successful experience with previous projects. The proposing firm should be capable of providing project management services and inspection services with special emphasis on cost, schedule and management of information. It is the District s desire to employ individuals from a qualified firm that exhibit proactive leadership and good planning and management practices. G. Insurance The selected firm will be required to provide proof of general commercial liability coverage in the amount of $1,000,000. H. Addenda Any addenda to this request for Proposals will be mailed to all potential firms who have been issued a copy of the RFP. Questions asked and not answered during the site tour will be mailed to potential firms that attend the tour.

Firm Selection Procedure A formal selection committee will be formed to review the qualifications of all firms responding to the RFP based on the evaluation criteria below. The committee will select those to interview as a part of the evaluation process. After interview process, firms will be ranked by the selection committee. The highest ranked firm will be awarded a contract by February 1, 2016.
