History San Luis Sports Therapy & Orthopedic Rehabilitation was acquired by Jim Glinn, Jr. in 1999 in San Luis Obispo, CA. Jim s vision was to create a collaborative, community involved team that prioritized patient care above all else and also had fun working together to change the lives of those they treated. At that time, a fundamental philosophy was established regarding the business and practice of physical therapy. These efforts led to growth from one clinic in 1999 to nineteen successful clinics today. Along the way, some of our physical therapists became shareholders and in 2014 Team Movement for Life of icially became the largest employee owned outpatient private practice group in the country. As San Luis Sports Therapy grew, so did its identity! We are known as Avid Physical Therapy, Athlon Physical Therapy, Central Valley Physical Therapy, Delta Physical Therapy, Score Physical Therapy, Specialized Physical Therapy, and collectively as Team Movement For Life. Our Administrative and Billing teams are based out of San Luis Obispo, CA and provide support to each clinic location. Within the Administrative team, departments include Accounting, Human Resources, Marketing, and Education & Development. Each region has its own Business Development Director who coordinates marketing and community involvement in concert with the local Clinic Directors. The company s expansion has provided opportunities for team members and other professional colleagues who recognize the value and vision of our team to deliver the same level of service within our communities. About Team Movement For Life We are an employee owned private practice with outpatient clinics in California, Arizona, and North Carolina that rewards individuals for their contributions. Our Company Model Each of the Movement for Life clinics is run by a physical therapist Clinic Director. We have developed a unique management approach that allows Directors to focus on patient care as well as on building their local clinic team. Our administrative team of professionals exists to support the needs of the clinics by handling administrative tasks, maximizing ef iciency, reimbursements, and reducing inancial and regulatory risks. Community Involvement We are very involved in the communities we have the opportunity to serve. We believe that healthcare is local and should be a collaborative effort. We see ourselves as an integral part of each patient s medical team and are committed to communication, team work, and support of every patients care. Being a part of the community is something we are very passionate about. Annually, Team Movement for Life employs Certi ied Athletic Trainers to local high schools and shares in the costs of these services totalling several hundred thousand dollars each year. These programs provide daily on-site medical coverage for student athletes at practices and games, with ongoing support from our local clinic staff. Our Certi ied Athletic Trainers also oversee the use of ImPACT, a valuable program used in the management of sports concussions. The Athletic Training program allows local schools to ensure the health and safety of their student-athletes. This is just one way we reach out. Throughout the year, each of the clinics collects money for local causes and participates in local events such as the MS Walk, the Heart Walk, Go Red for Women, START!, Miracle Miles for Kids, Relay for Life, Parkinson s Walk, Breast Cancer Awareness, and many more. Our team also inds great pleasure in sponsoring local athletic teams and organizations, and providing community lectures and free backpack screenings for kids in all clinics. 7

Clinic Director Learning Modules Business Management Course Business Principals and Role of the Directors, basic economics, inance, human resources, leadership, decision making, strategy, and planning in the context of a Physical Therapy clinic. Peer Group Discussions Regular meetings with others in training as well as those who are just starting their careers as clinic directors. Meetings will be facilitated by one of our Clinic Directors and often have guest speakers who have experience or expertise in various areas of the business. Mentoring Each participant will be assigned a Mentor who currently is running a clinic and will share their experiences as well as some hands-on experience running a practice. Director Retreat Weekend Participants will join Movement for Life Clinic Directors in their annual retreat where they will receive training, feedback, and join in the discussions that shape the future of the organization. Administrative Training Training is provided to help future clinic directors understand the ins and outs of the front of ice, billing, and back of ice processes. Clinic Visits and Consultations Each participant visits other clinics within the Movement for Life Family to have hands-on access to different management styles and environments. The participant will then act as a consultant to those clinics, providing observations and recommendations. This experience encourages both critical thinking and an understanding of the different ways to approach similar challenges. External Business Training Participants will attend external training courses to gain different perspectives and expand their business knowledge. 10

Clinical Education The Movement for Life team strives for clinical excellence. Our organization is committed to using evidence based practice (EBP) as the cornerstone of our physical therapy care and place emphasis on clinical education. In-House Courses & Clinical Webinars for CEUs: Up to 3 weekend courses per year hosted by TM4L Course topics are determined by staff suggestions, feedback from past courses, as well as new programs the practice is introducing Monthly Clinical Webinars review evaluation, treatment, and clinical reasoning in various areas of PT practice Unlimited access to MedBridge s online courses Continuing Education Courses Outside of the Company We pay for course tuition for team members to attend one clinical course per year with Clinic Director s approval; additional courses can be approved by our Clinical Development Committee. There is lexibility in this bene it to allow the Clinic Director and each physical therapist to create an individual plan for career development to compliment the clinic and individual s goals/vision. Transitional DPT We offer tuition assistance to support tdpt programs to PT s desiring to obtain their transitional doctorate. Physical Therapy Student Clinical Education Acting as a mentor is a valuable opportunity to demonstrate your ability to articulate and demonstrate physical therapy knowledge. Following one year of clinical experience, Physical Therapists are given the opportunity to mentor PT students. The practice hosts 45+ students per year from around the country, so opportunities to be a clinical instructor to the next generation of PTs are abundant. 11

Orthopedic Physical Therapy Residency The Movement for Life Team has partnered with the University of Southern California to create an exceptional Orthopedic Residency Program. Accelerate your professional development and work in a collaborative and engaging environment. Receive an excellent salary and bene its package as you gain advanced knowledge and skills in orthopedic physical therapy. This APTA credentialed program combines expertise from one of the premier Physical Therapy programs in the country with the our local team of Board Certi ied Orthopedic Clinical Specialists. The Residency is a 12 month program of post-professional clinical and didactic education for Physical Therapists designed to advance preparation as a provider of patient care services in orthopedic physical therapy. Residency Format: Two, 8 day intensive sessions at the USC LAX Learning Center in Los Angeles. Learning occurs in the small group classroom and lab settings Supervised independent study, integrated into the coursework, following the Orthopedic Section of the APTA s curriculum 150 hours of mentoring by a Movement for Life board certi ied clinician on an ongoing basis, with 1:1 clinical supervision and mentoring while treating patients Training in evidence-based guidelines and advanced clinical practice Focused skill building on examination, treatment and clinical reasoning aspects of orthopaedic physical therapy patient care Preparation for the OCS examination For more details on the Residency program, please contact Kelly Sanders at kelly@movementforlife.com. 12

Acquisitions, Mergers & Partnerships The Movement for Life Team is always looking for like-minded professionals to work with. We are open to exploring opportunities of all kinds and if we believe there is a good it among people, approach, and goals then we look to move forward with developing an ongoing relationship. Our approach includes ive key factors to bring two teams together successfully: 1. Fit While we look at the strategic it, opportunity, and viability of a possible relationship with an existing practice. The leadership and the team is what makes a practice succeed. Without the right people, it won t work. 2. Mutual Bene it Once we ve found a potential partner, we then explore the possibilities. We take time to make sure the relationship will indeed be a good it, which includes meeting staff and understanding what their goals and desires are. 3. Local Management In all cases we will have identi ied a local director to run the practice. In some cases the director may be the existing director/owner, in other cases another employee in the practice, or we may have an existing team member to take on the role. 13

Consistency Our intent is to keep things as consistent as possible with a new partnership, being careful to not disrupt the aspects that drew our initial interest. Usually this centers around the team of individuals that built the practice to what it is. We make every effort to treat employees of new partners as our own long-time team members from day one. This includes providing opportunity and working with staff to achieve their own goals while also holding all team members to our high standards for both service and growth. 5. Integration We recognize the value and experience employees of a practice bring. We offer the same training, resources, and bene its to all employees. Typically, employees will be grandfathered into our bene its program recognizing their time with their previous employer. We integrate a new partner practice into our existing back of ice and administrative systems while leaving day-to-day patient service and staff management to be handled locally. The bene it of this solid and established model goes beyond improved processes and ef iciency by providing the security of a larger organization and the lexibility and responsiveness of a small, locally managed practice. 14

Frequently Asked Questions What type of patients do you treat? As a practice, we primarily treat orthopedic cases, but also provide care in the areas of cardiopulmonary physical therapy, vestibular rehabilitation, and balance & fall prevention. We are always interested in exploring new areas of practice with team members who are passionate about advancing their education and heading up new programs. What are the typical hours for a therapist? This varies by therapist and clinic needs. Most of our team therapists work 3 long days (9-10 hours) and 2 short days (5-6 hours). Therapists schedules are coordinated between the therapist and clinic director. Are CA, AZ, & NC direct access states? Arizona & North Carolina are full direct access states for physical therapy. California also has direct access but does require a plan of care be signed by a physician after 12 visits or 45 days of care. Do you use manual therapy? Yes! We follow Evidence Based Practice and thus have a movement-based treatment paradigm that is complimented with manual therapy. Do you hire new graduates? We welcome new graduate applicants and invite many to join our team each year. We also have a very active PT student program with physical therapists at various stages in their careers throughout the practice. 15

Frequently Asked Questions Is the residency program only available to employees? Yes, residents must be employed full-time with Movement For Life. How does the cost of the residency program affect my salary? Movement for Life contributes a portion of the cost, while the resident is responsible for the remaining $8,000 tuition. Base salary does not change, but the resident can either pay their portion upfront or agree to a 12 month pay reduction. Do you have a mentoring program? Yes, all new hires have access to a 1-3 month mentoring program, dependent on their experience. They are assigned a senior PT team member who will orient them to the practice, mentor in areas of clinical care, and lead them through the Movement for Life PT Training Program. Can you start the Clinic Director Training as a new grad? Yes, many have done this! The program is designed to be lexible and can run approximately 12 months with the duration being based on your level of management experience. The program can also be done simultaneously with our new hire mentoring program or the residency program. 16
