PERRY COUNTY SCHOOL DISTRICT E-RATE INTERNET SERVICE RFP RFP #13-14:1 The Perry County School District is accepting quotes for eligible services as defined by Schools and Libraries Division (SLD) of the Universal Services Administration Company (USAC) -- http://www.usac.org/sl/. All inquiries will be recorded and provided to all prospective vendors who attend the required site visit. General Facts and Information: All addresses are New Augusta, Mississippi 39462. All interconnections between the Service Provider (SP) and PERRY COUNTY SCHOOL DISTRICT (PCSD) must be RJ45 jacks provided at a 100 Megabit minimum. Contract term is 3 or 5 years and will commence on July 1, 2013. Service must be fully functional by the beginning of the contract term. Circuit must provide symmetrical bandwidth. This request is also for a service level agreement. All bandwidths are to be provided as fully available at all times. For example, a 20 Megabit Internet connection is guaranteed to provide 20 Megabits (as defined by industry standards) to PERRY COUNTY SCHOOL DISTRICT. Service circuits providing service level that varies widely from the agreed to and guaranteed connection speeds will be considered in breach of contract. PERRY COUNTY SCHOOL DISTRICT will provide a firewall between the district network and the Internet. Internet Service Provider will own and maintain all equipment on the Internet facing side of the District firewall. Vendor must complete the worksheet on the following page and include with the quotation. Vendor must provide documentation detailing filtering solution if proposed. Vendor must attach an upgrade proposal as described in the worksheet document. Vendor must attend a mandatory on-site visit to see the facilities into which the vendor s equipment will be installed. This visit will be scheduled on Friday, February 15, 2013 at 10:00 AM. Vendor must provide 3 references from businesses and/or school districts with similar Internet connections to this proposal. Update information will be emailed to each service provider attending the mandatory site visit. The following services must be included as part of the INTERNET SERVICE: Internet access from circuit to be located at: PERRY COUNTY SCHOOL DISTRICT OFFICE 105 Main Street New Augusta, MS 39462 30 Available Public IP addresses for servers and other devices requiring outside access Network Address Translation Service for the entire district network DNS Service ISP must provide circuit utilization data in a timely manner upon request of the PERRY COUNTY SCHOOL DISTRICT. The contract must allow for upgrading using the prices quoted in the worksheet grid. Vendor must quote all options in the grid to be eligible for consideration. Vendor is encouraged to propose additional options according to their offerings. Option to include Internet Filtering that is CIPA compliant.
