Employee Declaration Form NORTHSTONE (NI) LIMITED [GF1 FORM] MOTOR FLEET POLICY EMPLOYEE DECLARATION FORM 2009 (This form should be completed by any employee who receives a car allowance and/or other payments from the Company in respect of the use of their own vehicle on Company business). All employers have a legal duty to ensure, so far as is reasonably practicable, the health, safety and welfare of all employees. This duty extends to the use, management and control of vehicles on Company business and includes vehicles owned by employees and used on Company business. I have read section 5 of the Northstone Motor Fleet Policy (copy attached) and understand my legal obligations in respect of the use of my vehicle on Company business. I hereby declare that I will: maintain my vehicle in a roadworthy condition at all times. ensure that my vehicle is regularly serviced in accordance with the manufacturer s recommendations. regularly carry out basic maintenance checks as recommended by the vehicle manufacturer which include, but are not restricted to, tyre pressure and tread, mirrors and fluid levels. ensure that my vehicle has a current MOT certificate, if required, at all times. have a current and valid license to drive my vehicle. effect suitable and sufficient motor vehicle insurance, including business use cover, for my vehicle. advise the Company should there by any changes in my circumstances which affect my capability to drive my vehicle on Company business. Specific Vehicle Checks APPENDIX 3 SPECIFIC VEHICLE CHECKS COMMERICAL VEHICLES Authorised Drivers, in addition to the checks detailed in section 2.2. of the Motor Fleet Policy, must also undertake the following specific vehicle checks:- (1) Tipper Lorries tail gate securing mechanisms build up of material preventing proper closing of tail gates hydraulic oil leaks at tipping rams and pipes access steps and grab handles to cabs and vehicle bodies (2) Truck Mixers spillage on discharge chutes prior to leaving the plant and customer sites (3) Block, Pipe and Tile Offloaders mounted crane warning indicators side gate securing mechanisms jack legs uplifted before moving off block and tile bale binding straps (4) Waste Disposal Skip Trucks net coverage of waste in skip jack legs to be uplifted before moving off lifting chains to be secured after skip has been offloaded visible defects in lifting chains and hooks skip tipping hooks on bed of lorry (5) Cement Tankers proper securing of hoses walkways and guardrails (if fitted) along top of tanks pressure gauges and safety valves hydraulic oil leaks at tipping rams and pipes guarding of drive shafts
