Together with Brigid Ide, Executive Director of Patient Safety & Quality, and her team, the Enterprise Analytics team collaborated to provide you the Quality Dashboard, QualDash version 2. As many of you are aware, the QualDash is an interactive tool to provide transparent information. Based on your feedback, this new version has some added features as well as some new metrics. Please continue to provide feedback so that we can best meet your quality reporting needs. Below are the basic changes. Accessing QualDash: URL / Web Address: Enter QualDash into URL or click http://qualdash.ucsfmedicalcenter.org UCSF Campus or Medical Center network/vpn connection REQUIRED Chart Review allows for access to patient detail information ONLY if QualDash is first opened within APeX s BI & Analytics Dashboard. See Attachment. Key Updates on QualDash: New Landing Page Layout O The landing page now has all placeholders populated with metrics O In addition, the Sepsis metric section (Sepsis) now has a link that will take you to the Sepsis dashboard New Metrics Added O HAPU Incidence All & Stage 2+ These metrics display the Rate of HAPU incidence (HAPUs per 1,000 patient days) and also divides out the Pressure Ulcers categorized as Stage 2 and above. By selecting this metric you will be taken to the Metric Detail page. If you then select the Patient Details tab, the pressure ulcer stage will be shown in the Results column. Included is a HAPU Incidence process metric. O Falls All & w/injury Incidence Metrics These metrics display Rate of All falls and falls with injury incidence (falls per 1,000 patient days). By selecting this metric you will be taken to the Metric Detail page. If you then select the Patient Details tab, the fall injury severity type will be shown in the Results column. O Restraints Utilization Metrics These metrics display the rate of utilization of non-violent restraints (Restraint-days divided by the number of patient-days). Included is a Restraint process metric. O HAC Metrics These are the metrics for the Hospital Acquired Conditions (HAC) obtained from UHC data. The individual metrics can be seen in detail on the Metric Detail page. There is no composite target for this metric. O AHRQ (Adults/Peds) Metrics These are the metrics for the Agency for Healthcare Research & Quality (AHRQ) quality indicators obtained from the UHC data. The individual metrics can be seen in detail on the Metric Detail page. There is no composite target for this metric.
O UHC Mortality O/E Metrics These metrics display the number of observed deaths divided by the number of expected deaths as defined by UHC O CHG Bathing Central Line This metric displays the APeX documented Daily CHG Bathing for Patients with Central Lines. O Sepsis Care Bundle Metric This metric displays Total Sepsis Bundle Compliance. In addition, the complete detailed sepsis dashboard can be accessed by clicking on the wordsepsis. Metrics Details Tab Format Change O There has been an addition to the metric detail page of five buttons at the top which link to the unit level targets for many of the Nursing Sensitive Indicators. As the dashboard by design does not have the capability of unit level or metric detail target, this solution was added to meet the needs of those who will be working with those metrics. From this dashboard, you can select the following: Analytics Dashboard Orientation: My Analytics Dashboard Lists all QlikView dashboards that a given user can access Hyperlinks to Medical Center and Campus Account Request Forms (ARFs) if access to another dashboard is desired.
