Monash Health Self Service Password Reset \ Account Unlock via a Non-Monash Workstation 1. Open your web browser and type in the following web address: Https://selfservice.monashhealth.org 2. To reset your password click on the Self Service button 3. Enter your User ID (Employee Number)

4. Select Unlock Account OR Reset Forgotten Password (depending on your requirement) and Select CONTINUE 5. You are now presented with two methods to reset your password. Either by answering 4 Security Questions OR via an SMS code

OPTION1: 4 Security Question 6.1 Answer the above questions and then select Continue. NOTE: The above Answers can be changed via the following link: [ Resetting your 4 Security Questions] OPTION 2: SMS code 6.2 After pressing this option you will be sent an SMS text message with the code. NOTE: If you do not receive the message then please contact the IT Service Desk on 9594 47255 who can assist you over the phone.

Once successfully identifying your identity, you are now able to reset your password. NOTE: Your password MUST meet the minimum requirements with 3 of the following 4: At least one capital letter At least one lower case letter At least one number At least one non-alphanumeric character (eg - %, @, *,!) The password must be a minimum of 8 characters in total. For example: Wednesday123, Gre@twork,!Monash1 The previous 3 passwords will not be allowed The password cannot contain your name The password will expire every 90 days

Your password has now been successfully reset and you re welcome to login.
