Complete a VWCC non- credit college application online at: http://www.virginiawestern.edu/studenthelp/student/applyonline.php. 2. Provide your assigned Student EMPLID #. Please make a note of your Student EMPLID#. This will be used throughout your studies at VWCC. 3. Complete placement testing at the VWCC Main Campus, Library basement (visit http://www.virginiawestern.edu/ltc/ for details). Attach a copy of your test results to this form. If your test scores fall below acceptable limits, you may re- take the test up to two additional times. Student Information for Pharmacy Technician Program Spring 2013 Program Dates: Mondays and Wednesdays, September 8, 2014 through November 12, 2014 from 6:00 pm until 10:00 pm in Anderson Hall on the Main Campus of Virginia Western Community College. Requirements: Students must be 18 years of age and have a high school diploma or GED; however, current high school students may be considered for the program under some circumstances. Students must be computer literate and have access to a computer with Internet access, an email address and printing capabilities. Students must complete Virginia Western s Placement Tests and show the ability to read, write and do math equivalent to the 9 th - 10 th grade level. While no specific limits are prescribed by this program, students are advised that the State and National Pharmacy Technician Certification Board and many employers will have restrictions and limits on: Individuals who have had disciplinary action against their pharmacy technician license, certification, or registration in any other jurisdiction, or have been prohibited from performing the duties of a pharmacy technician by any other state, or prohibited by a health regulatory board of any state or by any federal agency from practicing, or assisting in the practice of, any health profession Individuals who have been convicted of, pled no contest to, or have charges pending of any felony, or any crime involving moral turpitude, or a violation of any federal, state, or local drug law. Individuals who have been physically or emotionally dependent upon the use of alcohol or drugs or treated by, consulted with, or been under the care of a professional for any substance abuse within the last two years.

PHARAMCY TECHNICIAN PAYMENT & REFUND POLICY Tuition for the Pharmacy Technician program in the amount of $1,599.00 is due on September 3, 2014. Tuition payments can be made after the following process has been completed by the Administrative Staff of Virginia Western Community College: Payment can be made at any of the Virginia Western locations, i.e. Main Campus, Greenfield Education and Training Center or the Franklin Center. Your payment may be made by check, money order or credit card (NO CASH except at the Main Campus Cashier s office location). No financial assistance for the Pharmacy Technician program is available through Virginia Western Community College s Financial Aid office. Students who are unable to complete the Pharmacy Technician program will be considered for refunds using the schedule shown below when a written request for refund is received by Virginia Western s Workforce Development Division Office; Attention: Dan Semones (PO Box 1400, Roanoke, VA 24038). Attendance and continuation requirements will be discussed on the first day of class. REFUND SCHEDULE Students who withdraw in writing before September 15, 2014: Refund amount $1,400.00 Student who withdraw in writing before September 22, 2014: Refund amount $1,200.00 Students who withdraw in writing before September 29, 2014: Refund amount $720.00 No refunds will be provided to students who withdraw on or after September 29, 2014.
