NHAIS and FHS Services GP Payments Total payments for 07/08 of 7.2b Cervical Cancer Screening 3.4m women called for screening Labs examined 3.7m samples Saving approx 1,300 lives per year and preventing up to 4,000 cancers UK service acknowledged as the best in the world Breast Cancer Screening Over 2m women invited for screening Over 1.63m women screened Over 13,500 invasive cancers were detected Survival rate now over 80%

NHAIS and FHS Services Registration 56.9m patient registration records managed across England and Wales per annum. 38,000 GP s in 8,500 practices linked to NHAIS 12.3m transactions from GP practices per annum. 5.5m patient registrations 6.0m amendments 750,000 deductions (removal from GP list) Feeds PDS Advance search against PDS National Blood Service Over 1m demographic updates provided to NBS in 2008 Helped to improve donor attrition rate from close to 50% to almost 90%

NHAIS Stakeholders Developments are commissioned and managed by a variety of stakeholders including: Department of Health NHS Connecting for Health NHS Information Centre National Cancer Screening programme Strategic Planning Group (SPG) Strategic Planning User Group (SPUG) Special Interest Groups (e.g. Ophthalmic Payments, Open Exeter etc) National Blood Service UK Transplant

Registration Developments Primary Care Registration Migration Currently undergoing an end of programme phase review. The outcome of this review will impact on the scope and timescales of changes to the existing registration systems and processes as described below. NHAIS/PDS Stage 4 Developed and implemented over the next 2 years. NHAIS will receive demographic updates from other NHS Organisations via the PDS? Facilitate the closure of CHRIS as the National Back Office (NBO) migrate to the Demographic Spine Application (DSA)? FHS services will begin to migrate some business activities away from NHAIS and onto the PDS (e.g. Medical Record tracking)? Compliant GP systems will begin to drop the link to the local NHAIS system and carry out registrations directly on the PDS? Demographic Spine Application (DSA) Already in use by the National Back Office. Undergoing a pilot with NHAIS Agencies prior to full rollout.

GP Payments Developments Planned Developments Changes for Tiered Contributions implemented 1 st February. Input facility for DES/LES claims via Open Exeter at pilot. Single Payment Phase 2 migrates PMS payments into the New GMS applications due to be implemented at the start of the new financial year. Microgen NHS Gateway service for BACS payments will close at the end of March. Improvement s to the security and audit of the UNIX download BACS option are in development. Potential Developments Global Sum changes for the 2010/11 pay round. Procurement of a replacement for QMAS.

Cancer Screening Developments Breast Cancer Screening Higher Risk Surveillance. Introduction of a national surveillance programme for younger women who have been assessed as being at higher risk. Age Extension. The normal screening age is to be extended from 50-65 to 50-70. This will involve significant changes to the selection process and a new Vital Signs report (VSA09) to monitor progress towards full coverage. KC63 statutory returns will be amended to report on higher risk surveillance activity and age extension coverage. Cervical Cancer Screening 14 day turnaround. A new Vital Signs report (VSA15) to monitor against the 14 day turnaround from the date of test to the date the result letter is received. HPV Sentinel software. Introduction of a new infection code of U (Unknown). Existing Change Requests are to be reviewed by the National Screening User Group.

Cancer Screening Developments Strategic Plans for a National Cancer Screening System The NHS Cancer Screening Programme team and NHS CFH are currently scoping a potential new National Cancer Screening system. The feasibility study has recommended a phased approach to future developments. The proposal represents a significant programme of IT and Business change, to be delivered over a 6-7 year period. The next step is to develop a Strategic Outline Case before the development of a full Business Case and procurement.

System Platform Developments Multiple Database Environments The NHAIS system is currently limited to running a single database on a UNIX server. FHS Agencies could be running 6 or more separate databases. SSD are currently investigating the possibility of running more than one database on a single UNIX server. This could reduce hardware support costs. Outline project costs to be presented to the SPG for approval to proceed. Hosting The SPG have requested SSD to investigate the options for providing a hosting service. This could provide the opportunity for NHAIS organisations to make savings on hardware ownership and maintenance costs.

Migration Plans for NHAIS Services Primary Care Registration The migration strategy is currently under review. Before the review started, it was expected that the full migration of the registration process would not complete until 2013. Cancer Screening A Strategic Outline Case is in production. A full Business Case and procurement process will be required. It is considered to be a series of IT and Business changes over a 6-7 year period. GP Payments There are no migration plans for GP payments at this point in time. There is no planned date for the decommissioning of the NHAIS system.

Related Services Ophthalmic Payments System Currently in use at all but 7 PCTs Maintained to meet payment regulations. Produces required statistical returns. Provides Fraud Prevention and Detection tools. Electronic links established for GOS 1, 3, 4, 5 and 6 Currently linked to 192 stores. Open Exeter Provides controlled, authorised web browser access to NHAIS data. Full transaction recording for Caldicott Guardian audit. Portal to other applications e.g. Home Oxygen and HPV vaccinations.

Related Services Secure File Transfer Service Provides a simple web-based interface enabling users to select a file, recipients and a password prior to uploading a file. Handle files up to one (1) GigaBytes (GB) in size, with the facility to accommodate larger files following testing and agreement with SSD. Only available over N3. Only permits files to be loaded where both the initiator and the intended recipients email address are verified as existing on the NHSmail directory. Bowel Cancer Screening Aim to cut deaths by 15% in people aged between 60 and 70 (second biggest cancer killer). More than 4m test kits issued. More than 39,842 positive test results. More than 3,711 cancers found and treated (to Aug 08).
