Patients treated with ECT/TMS are selected from a group of diagnostic categories. Schizophrenia, schizoaffective disorder, schizophreniform disorder, bipolar disorder, major depressive disorders, Parkinson s disease, catatonia, neuroleptic malignant syndrome, postpartum psychosis. After referral proper evaluation, consultation and

informed consent procedures, patients are treated in the PACU (Post aesthetic care Unit) at the Montreal General Hospital or the Gate House (RVH-AMI) Research activity is available in inpatient services, mood disorders, TMS and ECT. In ECT particular research may focus on the schizophrenia, maintenance or continuation of treatments, anaesthetic procedures and informed consent. In TMS, research can focus on typologies of depression and related diagnosis. Publications: Opportunity for publishing a paper in the area of ECT/TMS will be provided and encouraged. Also poster presentations will be promoted for local, national, and international conferences. Publications from psychiatrists in inpatient, outpatient, and CL services are collated for fellows. Mission: The TIME Fellowship is structured to provide clinical, educational, and administrative and research experience and training. The aim of the fellowship is to produce clinicians who are fully competent to utilize ECT/TMS in a select group of patients. Also the fellow will be competent to educate medical students, residents, other clinicians and be capable of designing and administering a program for ECT/TMS in a hospital setting. How will fellowship enhance residency training? The TIME fellow will have a greater appreciation, understanding of diagnostic categories which may benefit from ECT/TMS. The fellow will have the enriched academic/clinical opportunity to function in mood program, inpatient service and CL service. The fellow will participate, present cases in clinical conferences, supervise residents and provide sophisticated care to patients and their families. A broad literature exposure pertaining to ECT/TMS is readily available (Journal of Convulsive Treatment and the Journal of ECT Libraries availability). Being a fellow on the inpatient/outpatient/cl services will enlarge the number of clinician s attending to patients and offer a stimulating academic environment. A weekly case conference will be provided for select patients, i.e. new cases, maintenance cases and ongoing cases. The major strength of this structural setup is that the fellow will have exposure to a number of clinicians and be able to follow the evaluation, case evolution and completion of treatments of a wide variety of diagnostic categories. Academic Facilities: ECTs are provided in a PACU (Post Anaesthetic Care Unit MGH 7 th floor East, Cedar Side), where psychiatrists, fellows, residents, medical students observe and participate in ECT with anaesthetists, specialized recovery nurses and other staff. On line literature access, ECT library (hardcover) and telecommunication facilities available. Daily Treatments (ECT) Weekly Case Conference Rounds Outpatient clinical responsibilities to be coordinated with Mood Disorder Program Director and contingent upon clinical load in ECT/TMS and inpatient psychiatry Role towards residents. Academic literature discussion pertaining to cases of residents rotating through on their 2 nd year (inpatient) and with other residents (Mood Disorders Program) Academic Activities: Ward Case Conference Rounds Morbidity and Mortality Rounds Weekly Case Conference Rounds (ECT/TMS) Grand Rounds Outpatient Rounds Select Ad-hoc Conference presentations Locally planned Conferences Support Staff available: Inpatient Service Administrator Ward Coordinators Clinical Nurse Specialist Biomedical Engineering Staff Proposed Scientific Meetings: Canadian Psychiatric Association: ECT course Brief (1-week) Fellowship course for psychiatry trainees Research Productivity and Publication: Topic of interest to be determined by research direction and fellow publication could ensue from this research. Curriculum: Following all patients receiving ECT: Presently 3 cases/day X 5 per week. Attending ward rounds for inpatient psychiatrists and outpatient rounds for Mood Program. Fellow will follow several patients on both inpatient and outpatient Psychiatry. Percentage of cases: Approximately 40% schizophrenia/schizoaffective disorder
