Okeechobee, Florida 34974 DESCRIPTION OF PROJECT: The Seminole Tribe of Florida Housing Department (STOFHD) is soliciting proposals from qualified vendors for full-service Contractor to work in Brighton, Florida.
SECTION 1: INTRODUCTION AND INSTRUCTIONS 1.0 INTRODUCTION This RFP provides prospective firms with sufficient information to enable them to prepare and submit proposals to the Seminole Tribe of Florida Housing Department (STOFHD) for The STOFHD is initiating this competitive RFP to qualified Vendors who are in the business of providing General Contractor Services, capable of completing numerous job functions/services in a timely manner and therefore, seeks to enter into an Agreement with that Vendor. 1.1 INSTRUCTIONS This Request for Proposal (RFP) contains the instructions governing the proposals to be submitted and the material to be included therein; as well as any mandatory requirements that must be met to be eligible for consideration. Okeechobee, Florida 34974 A mandatory pre-bid/site meeting at the Housing Department conference room is scheduled for Thursday, 11/19/2015 starting at 10:30 am BID SUBMISSION AND OPENING Bidders shall submit one (1) signed original sealed hard copy bid and three (3) copies by November 30, 2015 (3:00pm EST) to: Seminole Tribe of Florida Housing Department Attn: Lambert Hazellief, Project Coordinator 650 East Harney Pond Rd. All submissions must be by mail, express-mail, courier service, or hand delivery. Submissions cannot be faxed or emailed. Bidders are solely responsible for following all instructions and submitting all completed bid documents on or before the submission deadline in order to be considered responsive and responsible. A public bid opening will occur on November 30, 2015 (4:00pm EST), at which time bid submissions received by the deadline will be publicly opened and recorded. The STOFHD hereby notifies all bidders that if your proposal is selected, you will be required to enter into a formal contract with the Seminole Tribe of Florida for the services you are bidding. You will also be required to meet all company vendor, insurance and certification requirements. The STOFHD reserves the right to reject any or all proposals, to waive formalities or informalities, and to award contract, either in whole or in part. The right is also reserved to select minority vendors and/or vendors with tribal member ownership. All proposals submitted become the property of STOFHD upon receipt. The content of all proposals shall be kept confidential until an award is made, after which such content will no longer be kept confidential, except as noted therein. 2.1 SCOPE OF WORK (SOW) SECTION 2: GENERAL REQUIREMENTS The Scope-of-Work ( SOW ) contained in the Attachments describes the services expected for a Contractor to provide the STOFHD. Respondent must ensure that all such Services contained in the SOW are provided for in every particular and shall meet or exceed the level of quality, availability, features, functionality, frequency, accuracy, and performance. The silence of specifications or requirements as to any detail contained in this document or any of the attachments or Exhibits, or the apparent omission from any specifications or requirements, or a detailed description concerning any point, shall be regarded as meaning that only the best commercial practices shall prevail and that only material and workmanship of the finest quality are to be used. All interpretations of the Scope of Work shall be made on the basis of this statement
2.2 IMPORTANT DATES RFP Release Date: - Tuesday, 11/17/2015 Pre-Bid Meeting: - Thursday, 11/19/2015 Inquiries and Questions Deadline: - Wednesday, 11/25/2015 Proposal Due Date Monday, 11/30/2015 2.3 NOTIFICATION Each Respondent submitting a proposal in response to this RFP will be notified in writing as to acceptance or rejection of their proposal. Release of notification letters should be within sixty (60) days of the proposal submission date. STOFHD reserves the right to delay this action if it is deemed to be in the best interests of STOFHD. 3.0 OBJECTIVE The objective of this RFP is to provide sufficient information to enable qualified Respondents to submit written proposals. This RFP is neither a contractual offer nor a commitment to purchase any products or services. All information contained in the proposal must be legible. Any and all corrections and or erasures must be initialed. Each proposal shall be signed in ink by an authorized representative of the Respondent. The contents of the proposal submitted by the successful Respondent of the RFP may become part of any contract awarded as a result of this solicitation.
Scope of work RFP STOFBR103015 Water Damage Repairs 1495 N. Tucker Ridge Rd. Okeechobee, FL 34974 Living Room 1. Supply all labor and materials. 2. A/C return wall- remove existing base. Wallboard to be cut 2 up from visual damage or floor. 3. Replace/Repair any damaged wall framing, while wall is opened. 4. Treat exposed area with microbial solution (if mold is found in amounts above threshold allowance, remediation must adhere by licensed remediation company). 5. Contact Housing Department for inspection, prior to beginning re-construction and during construction process. (863-801-5118) 6. Install new wallboard and base. Install new return grille. Texture wall, prime and paint to match existing walls and base. (use Sherwin Williams paint, minimum of two finish coats) (caulk base before painting) Laundry Room/Kicthen/Bedroom 1. Supply all labor and materials. 2. A/C Plenum Remove existing air handler and plenum. 3. Remove existing base, all laundry room wallboard shall be cut 2 up from visual damaged area or floor.
4. All wallboard on opposite sides shall be cut 2 up from visual damage or floor. (bedroom, bedroom closet and kitchen) 5. Replace/Repair any damaged wall framing or door framing/trim while walls are opened. 6. Inspect all plumbing in exposed wall for any leaks or damage. (Repair/replace if needed) 7. Treat exposed areas with microbial solution (If mold is found in amounts above threshold allowance, remediation must adhere by licensed remediation company). 8. Contact Housing Department for inspection, prior to beginning re-construction and during construction process. (863-801-5118) 9. Install new wallboard and base. Install new metal frame plenum. (With three sides to be enclosed on the inside and outside with purple wallboard. Tape and finish all inside joints before painting and use corner bead on the outside joints, finish, texture and paint with Sherwin Williams semi-gloss to match existing walls). 10. Clean evaporator coil with coil cleaner while removed. 11. Re-install air handler and evaporator coil. Install drain line float switch if there isn t one. (Make sure to glue drain line connections, if not already glued) 12. Remove kitchen base cabinets on adjacent side of laundry room wall, to replace base and wallboard 2 up from visual damaged area or floor. Inspect cabinets for water damage, replace if damaged. 13. Clean/sanitize all a/c duct work throughout home. 14. Clean up & remove all debris from the job site. 15. Contact Housing Department for final inspection, prior to leaving the job site. It is the contractor s responsibility to provide a schedule and obtain any and all permits required to perform the task listed in the above Scope of Work. All change orders shall be approved by the Housing Department prior to any work being completed that is outside the scope of work.
