FOREWORD This logbook is designed to serve as an aide-memoire for doctors to record their CME activities which have to be submitted as self-claims using the SMC Online System. Doctors are required to submit self-claims for Categories 1C, 2, 3A and 3B (for Specialists) CME activities. Some important points concerning CME activities under each of these categories are highlighted in the table below. Doctors are advised to check their CME points periodically by logging into the SMC Online System. Note Members of the Academy of Medicine (AM) or Singapore Medical Association (SMA) may request these bodies to submit the CME claims electronically on their behalf. Members of the College of Family Physicians Singapore (CFPS) may request the College to assist them in submitting Category 3A claims for reading The Singapore Family Physician journals. Family Physicians who participate in the Category 3B Distance-learning Programme conducted by the CFPS need not submit their claims as the College will be submitting claims on their behalf. Upon successful submission of CME credit claims (Category 1C, 2, 3A and 3B), a unique transaction ID number will be assigned and displayed on the acknowledgement screen of each submitted claim. Please take note of the transaction ID number which should be quoted when seeking clarification regarding a submitted credit claim.
