(the White Sox ), Chisox Corporation, Levy Premium Foodservice Limited Partnership, Illinois Sportservice, Inc., Illinois Sports Facilities Authority, At Your Service LLC, At Your Service Management Corp., CWS Maintenance Company, Roclab Athletic Instruction LLC, Jerry M. Reinsdorf, Jerry M. Reinsdorf Revocable Trust, and each of their respective officers, directors, employees, agents, owners, members, partners, representatives and attorneys, and any and all of their respective subsidiaries or affiliates and all Major League Baseball entities, affiliates and member baseball clubs (together, the Released Parties ). Recitals: The undersigned (1) desires the Participant to participate in, attend and/or observe a White Sox Major League Baseball Game on the date written above (the Game ), including the Game and all related activities at U.S. Cellular Field, 333 W. 35th Street, Chicago,; (2) understands there is a risk and attendant danger of exposure to peanuts, peanut products, and peanut residue (e.g. an allergic reaction of any kind and to any extent, anaphylaxis, or any related symptoms); (3) understands that execution of this Release is a condition to Participant s participation in, attendance and/or observation of the Game. NOW, THEREFORE, in consideration of the premises, including the Participant s participation in, attendance and/or observation of the Game: 1. The undersigned, on behalf of himself/herself, Participant and Participant s family, estate, heirs, executors, administrators and assigns, hereby (i) releases all Released Parties from any and all claims, liability, obligation, loss and causes of action for personal injury (including, without limitation, any injury arising from an allergic reaction to peanuts), property damage, or other loss of any kind, whether known or unknown, suspected or unsuspected, asserted or not asserted, arising out of Participant s participation in, attendance/and or observation of the Game, whether caused by the negligence of Participant or another person, including one or more of the Released Parties and (ii) agrees to indemnify and hold harmless the Released Parties from any loss, liability, injury, damage or costs including reasonable attorneys fees, that the Participant or any Released Party may incur directly or indirectly due to Participant s attendance at the Game, whether caused by the negligence of Participant or another person, including one or more of the Released Parties. 2. This Release shall be governed by the laws of the State of Illinois. The undersigned agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Illinois and that if any portion hereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.. 3. The undersigned, on behalf of himself/herself and Participant, further agrees that by signing below, and in consideration of the opportunity to participate, attend and/or observe the Game, he/she authorizes Major League Baseball Properties, Inc., the Office of the Commissioner of Baseball and all of the Major League s (including the White Sox) and other affiliated entities to depict, in perpetuity, his/her and Participant s likeness, images, name, words, voice and biographical information (the Images ) in photographic or other works appearing in any and all media now known or hereafter created for purposes of promoting, advertising, or marketing current or future events related to baseball, and he/she further agrees that any and all such Images may be used by them for the foregoing purposes without compensation. 4. The undersigned represents that he or she has read this Release, fully understand its contents, and hereby executes it on behalf of himself/herself.
