If you need supportive housing call: Toronto Mental Health and Addictions Supportive Housing Network Phone: 416 979 5496 Hours: 8:30 am to 4:30 pm, Monday to Friday Website: www.tosupportivehousing.ca help you apply for supportive housing in Toronto Supportive housing is for people with mental health and/or addictions problems who need help with things like taking their medicine, doing housework, and making meals. If you need help filling out the application, talk to your social worker or another member of your treatment team. Financial help Ontario Works Website: www.mcss.gov.on.ca/en/mcss/programs/social/ow/ If you have no income but have rent and bills to pay and need help, contact your local Ontario Works Support Program office. Use their website to find the office closest to you or talk to your social worker. To apply for Ontario Works financial help you will need this information: your name, date of birth and address your Social Insurance Number (SIN) and/or health card number a list of places you have lived in the last 2 years a list of places you have worked in the last 2 years a list of your monthly expenses (for example, rent, water, heat, food) 4
