2 Who We Are Saguaro Group, LLC provides and supports mental health, developmental disability and substance abuse services that demonstrate a commitment to the community, put customers first, and honor our history of quality. The Saguaro Group is made up of three wholly owned subsidiary companies: Saguaro Management and Accounting Services, Inc., Triumph, LLC and Community Innovations, Inc. 3 Our mission: Triumph is committed to empowering individuals and families by delivering effective human services which encourage choice and promote wellness. Triumph provides comprehensive mental health and substance abuse services to adults, children and families. Services are evidence-based, best practice models, outcome-oriented and consumer driven. Services include: Day Treatment, Therapeutic Foster Services, Clinical Assessment, Outpatient Therapy, Psycho- Social Rehabilitation, Community Support Team, Intensive In- Home, Assertive Community Treatment Team and Walk-In Crisis and After Care. Evidence-based models include Wellness Management and Recovery, Seven Challenges, Cognitive Behavioral Therapy, Positive Action. Dialectical Behavioral Therapy and Trauma- Focused Cognitive Behavioral Therapy. Our mission: To enable individuals who we support to access services, make choices, pursue aspirations, live and interact with dignity and respect in their communities. Community Innovations (CI) supports individuals in discovering and creating what they most want in their lives. CI provides ICF/MR Residential and Day Services, I/DD Group Residential Programs, Alternative Family Living Homes, Independent Living Programs, Community-Based Services supporting persons with I/DD, Clinical Assessment and Case Management. Additionally, CI provides comprehensive child and adult mental health services including Outpatient Therapy, Psycho-Social Rehabilitation, Community Support Team and Intensive In-Home services. Full Three-Year CARF Accreditation In July 2011 Community Innovations, Inc. and Triumph, LLC achieved a three year re-accreditation from the Commission on Accreditation of Rehabilitative Facilities (CARF) for all periodic services. In addition, an initial three year CARF accreditation was awarded for 17 CI residential programs and 3 Triumph Day Treatment Programs. Both companies were commended for proactive supportive management; quality management activities; person centered treatment approaches; community integration; advocacy efforts; clean, welcoming environments; and well organized medical records. What Surveyors had to say Community Innovations: Blown away with the level of excellence collaborative, respected, pro-active, consistent in audit reviews and a model excellence in services. Triumph: Strong leadership committed to quality improvement in its business and service delivery practices well respected in community as a provider of choice with companionate staff, it s clear they care about the quality of life of the persons served.

6 Highlights Move iintto Managed Carre In 2011 the NC General Assembly passed House Bill 916 calling for the Department of Health and Human Services to restructure the delivery of mental health, developmental disability and substance abuse services through the statewide expansion of the 1915(b)/(c) Medicaid Waiver. To prepare to become a member of a managed care provider network, the companies of the Saguaro Group formed a workgroup to examine all of the tasks to be completed to ensure that we had all the necessary tools and met all the applicable standards for entrance into the network. The group established an internal website to keep current on the many changes required. The group reviewed procedures and identified needed revisions in the areas of consumer enrollment, provider enrollment, third party billing, consumer access, privacy, confidentiality, medical records and compliance. Triumph and Community Innovations staff submitted the enrollment application for entrance into the PBH, ECBH and the Sandhills networks. Since that time we have been awarded contracts and have begun providing services in PBH and ECBH. Advanced Access Prrogrrams Opened Triumph began operating two walk-in crisis programs in 2011 called Advanced Access. Both programs came to Triumph as the result of success in competitive RFP processes held by the Five County LME and pbh. As a result of the support from these LMEs and MCOs, we opened these services in Roanoke Rapids and Burlington. Both programs are staffed five days a week with psychiatrists, outpatient therapists, and qualified professionals who provide immediate assessment and treatment for individuals who present at our facilities. The facilities provide a community alternative to hospitalization and provide appointment options within 2-3 days after hospital discharge to ensure individuals receive medications and follow up care. In Burlington, at the request of pbh, the program operates with extended hours, serving the citizens of Alamance and Caswell counties from 8:00 am to 8:00 pm. Both programs have been enthusiastically received and supported by their respective communities, other providers, and the MCO. Worrkfforrce Devellopmentt Community Innovations, Inc. requested and received a Proclamation from Governor Purdue in recognition of NC Direct Support Professionals Recognition Week. This workforce development initiative is intended to raise awareness of the dedication by direct support professionals who enhance the lives and provide the essential support to help keep individuals with disabilities connected to family, friends and community. Chiilld Pllacementt Agency Liicense On June 30, 2011, Triumph became a Licensed Child Placement Agency in the state of North Carolina. This license allows Triumph to recruit, train, and oversee foster and therapeutic foster homes serving youth with serious emotional disturbances. Each home is required to meet specific criteria in order to be individually licensed by the Division of Social Services. The first home was licensed in October, and respite care is provided in this home. The first respite stay provided was a youth being served by Intensive In-Home Services in Five County.

E-prescribing by physicians implemented with support of Saguaro Management IT Culture of Quality Staff Development plan approved by company leadership Triumph personnel certified as MINT trainers Staff from several programs completed Trauma-focused CBT certification Staff focus groups to address impact of staff retention and general quality Saguaro Group participating in Community Care North Carolina (CCNC) pilot Community Innovations, Inc. and Triumph, LLC. will combine administrative and operational structures into one company. The combined entity will be known as Triumph beginning January 1, Begin implementation of an Electronic Health Record Seek enrollment in eight additional MCOs Culture of Quality Improve Access to services through quality improvement (QI) project that focuses on effective response when an individual or family seeks services, and will also focus on ensuring the individual remains in treatment long enough to see a positive outcome. A system that responds quickly to an individual s request for services can prevent a crisis which may result in more distress to the individual and more costly care for the system. Saguaro Group recognizes the importance of a fully integrated culture of quality that includes the use of Evidence Based Practice (EBP) in service delivery. This QI project is intended to improve resources and trainings related to the EBPs. National data indicate that people with mh/dd/sa issues have overall poorer heath than those who do not experience these conditions. Saguaro Group is a Community Care of North Carolina (CCNC) partner, working closely with that group in a coordinated health effort. to improve individuals physical and behavioral health integration activities. The leadership of the Saguaro Group understands the importance of strengthening the alignment between leadership and our employees. The staff development QI project promotes employee engagement in the company, resulting in actions that are purposeful and passionate and consistent with the company s mission and strategic goals.
