Exceptions to the foregoing may be granted or permitted in writing to the Chair of the Formulating Group by TIA s Senior Vice President, Standards and Special Projects on a case-by-case basis. 1 Administrative 1.1 Call to order The meeting of the Subcommittee on Optical Fibers and Cables, TIA TR-42.12, was called to order by the Chairman, Mike Kinard, at 13:04 on 3 October The Chairman offered a brief memoriam of two past members of the Subcommittee Art Hudson, DoD representative, and Eric Loytty, Siecor representative. A moment of silence was observed. 1.2 Attendance Verbal introductions were made and the Chairman reminded the attendees to use the electronic attendance registration system using the meeting code. A total of 32 individuals representing 25 organizations were in attendance (Table 1). Quorum was established with all 15 quorum members present (Table 2). Two organizations, Huber+Suhner and General Cable, requested quorum member status and will be added as quorum members for subsequent meetings.

4 Pg. 3 of Agenda review and approval The agenda for the meeting was considered and the following modification was proposed: 1) Addition of Disposition of SP RV2 to be ANSI/TIA B under agenda item The modified agenda was approved by acclamation. 1.4 Intellectual property rights (early disclosure policy) The Chairman reviewed TIA s patent disclosure policy. TIA s patent policy can be found in the TIA Engineering Manual (5 th edition). Participants in the work of the TIA Formulating Group are urged to review this policy. Participants are encouraged to notify TIA of any patents of which they are aware that are related to the practice of a TIA publication early on in the development of the TIA document to reduce the possibility for delays in the development process and increase the likelihood that the draft TIA publication will be approved for publication. Patent searches are not required to comply with the TIA IPR policy. Moreover, the Engineering Manual describes the need for members disclosure of patents that are essential to practice TIA publications and for members to provide an indication of their willingness to license any known patents. 1.5 Chair s report; general items Items from the Opening Plenary Mike Kinard summarized the following items of significance discussed at the TR-42 Opening Plenary: 1) Receipt of a liaison letter by the TR-42 Committee from INCITS T11.2 Committee (Fibre Channel) regarding an improved 50/125 m laser-optimized multimode fiber to be considered for use within the (Fibre Channel) Physical Interface-6 (FC-PI-6) standard under development. Refer to agenda item 1.8 below. 2) Review of the TIA 568-D Task Group activities. 3) Review of the significant changes to the TIA website intended to improve usability and traceability of the standards development process. The August 2012 Teleconference TR Meeting Report was considered and approved without opposition by acclamation. 1.8 Liaison letters received The Chairman noted that a liaison letter from INCITS T11.2 had been received by the TR-42 Subcommittee. To provide context, Steve Swanson spoke to the Fibre Channel Liaison Report, contribution TR , and then to the letter itself, contribution TR The letter indicated interest in an improved laser-optimized 50/125 μm multimode fiber inasmuch as it may provide benefits in reach and margin for the developing 28 GBaud Fibre Channel standard. It was discussed and agreed upon that the Subcommittee would like to respond by the meeting closure so that it can be forwarded to the TR-42 Committee for review and release to INCITS T11.2. Paul Kolesar drafted a response with the help of several other interested Subcommittee members. Upon review there were some objections. As the discussion progressed there was a motion to assign an ad hoc team of all those interested to finalize a response with the authority of the Subcommittee. This motion was put forth by Pat Van Vickle and seconded by Mike Kinard; passed via acclamation. The ad hoc group developed a mutually agreeable liaison response that was forwarded to the TR-42 Committee for review. Refer to contribution TR Old Business 2.1 Reports TR Optical fiber cable technical review The Working Group did not meet since the last meeting and so no report was given Task Force on Laser-Optimized Fiber Specification (BIMMF) Mike Kinard noted that the Task Force planned to meet on the morning of 4 Oct 2012 and invited interested Subcommittee members to attend. Dave Mazzarese, Chairman of the Task Force, deferred discussion to agenda item The usual web link and call-in number was noted, for the information of all IEC SC86A summary Mike Kinard provided a brief verbal summary of the most relevant activities within the IEC SC86A Subcommittee. Refer to contribution TR The most discussed issues included: 1) BIMMF, particularly the synthesis of the second core diameter and numerical aperture round robin, and 2) possible modifications of IEC (macrobend loss test method) ITU-T summary Peter Pondillo provided a verbal summary of the most relevant issues within ITU-T. These issues included 1) multipath interference (MPI), 2) proposed revisions of bend insensitive SMF, G.657 category B3 and 3) proposed addition of another category

7 Pg. 6 of 19 fiber, category D, to dispersion shifted fiber specification, G.654. Aleksandar Opacic noted work on splicing and uni-directional measurements. Also, fiber-copper hybrid cables for 3G systems is being worked. The next meeting will be in July 2013, in Geneva ICEA summary Mike Kinard provided a brief verbal summary of the most relevant activities within the ICEA standards body. Refer to contribution TR Mike highlighted 1) the new MDU cable standard, ICEA S had published, 2) the indoor/outdoor cable standard and the drop cable standard, ICEA S and ICEA S respectively, both closed successfully and will publish prior to year end, and 3) the opening of a project on DAS antenna cables. 2.2 Old items to review PMD measurement adopt , status of adoption at FOTP-122, status of FOTP-113 and FOTP-124, factory testing and field testing-any outstanding issues To address the addition of the agenda item to disposition of SP RV2 to be ANSI/TIA B, Mike Kinard put forth a motion to authorize the publication of SP RV2; Steve Swanson provided the second. This motion passed unopposed via acclamation. Peter Pondillo advised that IEC is still slowly progressing through the standardization process within the IEC SC86C WG1 and therefore the Subcommittee should continue to wait to rescind FOTP-113 and FOTP-124 as previously agreed. Refer to Action Item # FOTP-28-D on fiber strength and fatigue testing, revision Previously it was agreed by the Subcommittee to adapt IEC David Mazzarese reported that no progress had been made on moving this forward. Refer to Action Items #13 and # FOTP-7, Numerical aperture of step-index fibers As agreed to previously, David Mazzarese advised that this action is on hold until the sister IEC document, IEC , stabilizes so that these documents may be harmonized. FOTP-177 will also be affected; agenda item Refer to Action Item # TIA-455-C, FOTP umbrella document Brett Lane reported that he still needs to add the expanded environmental conditions that will harmonize this standard with ICEA and IEC standards prior to submitting this document for ballot. Refer to Action Item # FOTP-204, revision to support OM4 David Mazzarese advised that he still needs to issue the re-ballot needed due to insufficient ballot response. Refer to Action Item # FOTP-80 and patch cord wavelength, adoption of IEC

8 Pg. 7 of 19 Marianna Kramarikova reported that FOTP-80-C was the most recent version on file. Mike Kinard made a motion to open a new project to adopt IEC as the revision of FTOP-80-D; Brett Lane seconded. The motion passed unopposed via acclamation. Refer to Action Item # TIA-598, color coding revision Mike Kinard advised that the ANSI ballot closed with many comments and two negative votes. He led the comment resolution. Refer to contribution TR and TR Upon conclusion of the resolution of comments, the Subcommittee authorized a second ballot. Refer to Action Item # Rescind TIA-492C-set: TIA-492C000 (1998), TIA-492CA00 (1998), TIA-492CAAA (1998), and TIA-492CAAB (2000) in favor of adoption of IEC as TIA-492C-series document to be determined (TIA-492C000-n). Refer to item 4.1 and Action Item # Bend Insensitive Multimode Fiber Issues Mike Kinard recommended that his item be deferred to the related Task Force meeting the following day, 4 October FOTP-220, DMD measurement; revision, status It was reported that the current plan is to adopt IEC , once its revision is completed TIA B, cable generic specification; revision; status Refer to agenda item TIA-472C000-C/ANSI/ICEA S , indoor cable, adoption-revision; status Refer to agenda item FOTP-177, graded-index NA; adopt IEC ; status David Mazzarese advised that the new IEC document on NA, IEC , will also cover the methods described in FOTP-177. Therefore, it was advised to wait until this document stabilizes and then disposition FOTP-177. Refer to agenda item FOTP-58, MMF core diameter measurement; adopt IEC ; status It was discussed that the IEC document on MMF core diameter measurement, IEC , is currently under revision to accommodate BIMMF and that upon completion, FOTP-58 will be harmonized with the IEC document Status of document maintenance items from February 2011 Mike Kinard reported that no progress has been made. 2.3 Action item review

The Action Items were reviewed and updated. 2.4 New document maintenance needs Refer to Attachment 2. 3 New Business 3.1 Presentations Proposals for TR action on the TIA-472-series cable specifications Mike Kinard spoke to contribution TR which outlined the unfavorable disparity between the revenue TIA realizes based upon the sale of the TIA-472-series and the royalty fees due to ICEA since these documents are adoptions of ICEA standards. Mike outlined 3 possible courses of action that were vigorously discussion. Finally, Mike Kinard moved to rescind the TIA-472 sectional specification; Brett Lane seconded. The motion passed unopposed via acclamation. Additionally, Mike Kinard moved to revise the unpublished TIA B to align with the previous motion. Brett Lane provided the second. This motion also passed unopposed via acclamation IEC TR and JWG on hybrid cables; status, intent, involvement Mike Kinard gave an overview of a new initiative within IEC on hybrid cables designed to carry optical fiber, twisted pairs/quads and current-carrying electrical conductors. They have issued a call for experts, IEC 86A/1464/AC, to participate in a newly formed Joint Working Group, SC46C/SC86A JWG8, Hybrid Cables, to address technical issues. The first meeting is 16 October 2012, at the TC46 Plenary meetings in Tampa, Florida; this will be a WebEx meeting. The scope of the JWG has not yet been determined IEC TR and JWG on End-Of-Life issues for cable Mike Kinard reported that liaison letters between IEC and ITU-T SG15/Q7, have been exchanged regarding the intent of ITU-T Recommendations and IEC specifications on the subject of End-of-Life (EOL) versus Beginning-of-Life (BOL) requirements. Some parties in ITU-T, along with some in IEC, are pushing the idea that fiber and cable specifications and/or requirements should be EOL specifications. The IEC response from SC86A is that IEC fibre and cable specifications represent BOL performance under defined criteria, and also have certain defined lifetime simulations that are intended to guide users to make conclusions about the applicability of cables over a lifetime. It further emphasizes that writers of specifications cannot make realistic judgments in a general way about the lifetime environment of any particular class of cable. Mike Kinard emphasized that this is an essential issue for fiber and cable specifications, and is expected to be ongoing for some lengthy time.

10 Pg. 9 of Document revisions Revision of TIA , cable jacket shrinkage, to harmonize with National Standards. Mike Kinard reported that significant changes have been made in the National Standards regarding the testing of cable specimens for jacket shrinkage. Specifically, the National Standards now specify testing whole-cable specimens, rather than jacket coupons or rings. This may require a revision of FOTP-86 to maintain harmony with the National Standards. After some discussion, it was decided to hold this item until the next meeting. 3.3 Other Determine upcoming meeting schedule February, June, October The Chairman advised that the next Subcommittee meeting will be during the first full week of February 2013 in Palm Springs, CA. Also, due to the usual conflict between ICEA and TIA TR-42 meetings during the first full week of June, the Chair of TR-42 has proposed moving the meeting to the second week in June. There was some discussion on this proposed move and finally consensus that the Subcommittee would support this move TIA-440 Editorial Group Mike Kinard reported that the TR-42.5 Subcommittee has closed a reaffirmation ballot on TIA-440 B. The ballot received a number of comments some of which require the expertise of TR to resolve the comments. Given that the ballot was for reaffirmation, if the comments are accepted in any manner, the document must be revised. TR-42.5 has proposed setting up an Editorial TG to deal with revision of TIA-440. Mike will send out an advisory as this gathers steam and action is required of the Subcommittee. 3.4 Output documents; letters; approved standards publications Consideration of documents for circulation as a Standards Proposals (SP). The Subcommittee authorized the following documents for circulation of as Standards Proposals (SP). Also see agenda item 4.2.
