COMES NOW NuVox Communications of Oklahoma, Inc. and respectfully states : 1. PARTIES AND INTEREST S The Applicant is NuVox Communications of Oklahoma, Inc., ("NuVox"). Its business address is Olive Blvd., Suite 430, St. Louis, MO No parties are being named as respondents to this cause. However, a copy of this Notice and Filing will be mailed or delivered to the Commission's Office of General Counsel and to the Attorney General of the State of Oklahoma. 2. FACTS a. By Order No , dated September 13, 1999, this Commission granted Gabriel Communications of Oklahoma, Inc. ("Gabriel") a Certificate of Public Convenience and Necessity to provide local exchange telecommunications services in the State of in Oklahoma within the service territories of Southwestern Bell Telephone Company and GTE. Gabriel's initial General Exchange No. 1 was approved by Commission Order No , dated January 4, b. By virtue of proper filings with the Delaware Secretary of State, the corporate name of Gabriel Communications of Oklahoma, Inc. was changed to NuVox Communications of Oklahoma, Inc. On March 9, 2001, NuVox filed with this Commission notice of this corporate name change reflecting the name change in Cause No. PUD The revised NuVox Oklahoma General Exchange Tariff No. 1 was filed with the Commission and approved, effective March 9, ~' s w ru a ~ m h ~r 4 ~...~ T u c. 2 NUVOX COMMUNICATIONS OF OKLAHOMA, INC. PAGE 2 existing O.C.C. General Exchange TariffNo. 1. Specifically, NuVox proposes to add a new feature to its existing VoxIP product. This new feature, SIP Trunking, allows existing and new VoxIP customers to have new functionality with their VoxIP service. The SIP Trunking feature is optional for new and existing VoxIP customers. VoxIP SIP Trunking uses Session Initiation Protocol technology to establish a single physical connection to the NuVox VoIP Network. This connection can be used for inbound/outbound local and long distance voice communications, as well as for data and Internet access. SIP Trunking is designed for termination on a customer-owned IPPBX system and is offered subject to technology availability and CPE compatibility. Other VoxIP services may be bundled with SIP Trunking contingent upon availability and compatibility and subject to the monthly recurring and nonrecurring pricing as described elsewhere in the pertinent section. f. A copy of the existing tariff sheet (2"d Revised Page 21.4) with the changes denoted is attached. A new sheet (Origina121.5) is also being added to the end of the VoxIP section. A copy of each page is attached hereto as Exhibit "A". NuVox requests approval of the proposed tariff revision, to be effective on or before August 11, NuVox reserves the right to seek emergency approval of these tariff revisions. 3. AUTHORITY Jurisdiction over the subject matter of this Application exists with the Corporation Commission by virtue of Article 9, Sec. 18 of the Oklahoma Constitution. This Application for tariff approval is authorized by OAC 165 : (b) (6), and Notice was provided pursuant to OAC 165: (c) (2). 4. RELIEF REQUESTED : NuVox respectfully requests that the Commission review and approve, without suspension, the proposed tariff revisions, to be effective on or before August 11, NuVox reserves the right to seek expedited approval prior thereto. I ORIGINAL PAG E VoxIP 21.8 VoxIP Session Initiation Protocol (SIP) Trunkin 2 (co ntinued) VoxIP SIP Features Direct Inward Dialing (DID) : DID service is available to host and remote locations that are in the NuVox footprint. Remote site DID service requires that a physical address be associated with each DID for the provision of local calling and 911 emergency routing service. VoxIP NuFeatures DID pricing applies to SIP Trunking. Call Paths : Customers can choose between I and 96 Call Paths. Pricing will be calculated based on the number of simultaneous call paths that are specified on a per customer basis. Monthly Recurring Char g e $25.00 Per Call Path SIP Compression : This optional feature increases simultaneous calls and call path capacity by condensing message size prior to sending messages across the network. SIP Compression MRC applies in addition to the Call Path MRC. Monthly Recurrin g Charg e SIP Compression MRC is 40% of the total Call Path monthly recurring charges. Long Distance : Pricing in VoxIP LD and LD Bucket sections applies to SIP Trunking. Additiona lly, SIP Trunking customers may qualify for LD pricing determined on an ICB basis.
