4 The Directory of Hospitals A national network of selected hospitals As an AXA healthcare customer, you have access to a national network of hospitals, day-patient units, scanning centres and other private healthcare facilities. These have been selected by our team of doctors, nurses and other medical professionals following a comprehensive assessment of their quality, value and range of services. All of them are listed in our Directory of Hospitals. Oral surgery If you require an oral surgical procedure, then we will provide benefit for those oral surgical procedures as set out in your membership handbook, when your dentist refers you directly to a facility indicated by the symbol in our Directory of Hospitals. lease visit our website: for further information and contact our team of ersonal Advisers before treatment. ataract Surgery If you require an eye operation involving cataract surgery, then we will provide benefit when your general practitioner refers you directly to one of the facilities indicated by the S symbol in our Directory of Hospitals. Our quality assessment and improvement programme Our initial assessment covers most aspects of the hospital s services. We only select hospitals and other facilities for inclusion in our network if they meet our strict quality criteria. This is only the beginning, however, and once accepted, hospitals must then undertake a programme of continuous quality improvement to ensure that their services continue to meet our required standards, in light of advances in medical care. In turn, we are committed to supporting the hospitals included in our network, encouraging them to invest in new facilities and services to our mutual long-term benefit. We regularly monitor their performance to assess their quality of care, which is why your feedback is particularly important. If you should need to use a facility, please complete your hospital s patient satisfaction survey form and return it to them. Standards for specialised services We also work closely with network hospitals that provide certain specialised care services such as cancer, cardiac revascularisation, critical care, paediatrics and psychiatric care. National guidelines for these have been established in conjunction with appropriate professional authorities and organisations. The benefits of our network hospitals Being treated in advanced medical facilities in a supportive, professional environment can play an important role in helping patients to recover. The hospitals included in our Directory of Hospitals will endeavour to make your stay as pleasant and comfortable as possible. You can usually have your own private room with en-suite bathroom, television and telephone. You will be given a choice of dishes for each course at mealtimes and your visitors may, within reason, come and go as they wish. 2

5 Be sure to call us first To check whether you are covered, please call us before you start treatment. We will then be able to authorise your claim in advance as described in the section Receiving treatment and making a claim in your membership handbook. There may be rare occasions when we suspend the use of a network hospital or another facility listed in our Directory of Hospitals to protect the interest of our members. Should this occur, we will indicate the suspension on our website: Day-patient treatment Advances in medical care mean that many operations are now undertaken on a day-patient basis. This is in keeping with good clinical practice. lease note that you may be cared for in an open day ward (rather than a single room) while you recover from an anaesthetic to enable nursing staff to monitor your progress more effectively. ost hospitals now offer day-patient treatment and we have included a number of purpose-built day-patient units in the Directory of Hospitals. lease check the Directory of Hospitals The map on page four shows the United Kingdom divided into eleven areas. Each entry shows the medical service available, arranged alphabetically by town and ordered in reference to the eleven areas shown on the map. Key to symbols Oral surgery arrangement in place S ataract surgery arrangement in place A triangle ( ) indicates an NHS hospital that does not have dedicated private patient facilities. (We will recognise an NHS bed nominated as a private bed but in such cases the level of accommodation may be the same as standard NHS accommodation.) Scanning centres these centres provide: RI (magnetic resonance imaging) T (computerised tomography) ET (positron emission tomography) These are usually carried out on an out-patient basis. Only the centres listed can be used for out-patient scanning.
