2 About Who we are Coeliac Queensland (CQ) is a not-for-profit organisation established in 1974 to assist and support people who are diagnosed as requiring a gluten free diet for life. CQ is Queensland s leading support resource with over 4000 members*. We are a member-based organisation and part of the national organisation, Coeliac Australia. CQ receives no government funding and relies on membership fees, advertising, fundraising and donations to provide services to its members. Why cater gluten free? People eat gluten free for a variety of reasons. For people with coeliac disease, eating a strict gluten free diet is not a choice, but a medical necessity. When people with coeliac disease eat gluten, an inappropriate immune reaction causes inflammation and damage to the small bowel. This ultimately leads to nutrient deficiencies and a number of serious health consequences such as osteoporosis and certain forms of cancer. People with gluten intolerance do not have the same small bowel damage and related health consequences; however they do have physical symptoms upon consumption of gluten including nausea, diarrhoea, fatigue and other gastrointestinal symptoms that may last several days. Whilst at least 1 in 70 people have coeliac disease, it is estimated that between 10% and 20% of people in Australia now follow a gluten free diet for gluten intolerance or other medical conditions. Why become a member? All restaurant members receive a new member kit upon joining that contains the following: Ingredient List booklet - easily identify if an ingredient is gluten free at a glance. Over 1000 Page 2 ingredients listed. Fact sheets - all you need to know about gluten free catering including reading labels, cross contamination and tips for front-of-house staff. Advertising discounts - receive discounts on advertising in our bi-monthly e-newsletter, Against the Grain and on inserting brochures into our New Member Kits. Gluten Free Food Only waterproof sticker set - perfect for labelling your gluten free product storage containers Window sticker and membership certificate - replaced yearly upon renewal - let customers know that you re a member of Coeliac Queensland and have gluten free options available. Ongoing benefits include Online restaurant directory listing (see page 3) Quarterly Professional Member E-newsletter Quarterly Australian Coeliac magazine and bi-monthly Against the Grain newsletter Opportunity to participate in our Member Discount Program (see page 8) *As at 1 April 2014

3 Online Restaurant, Cafe and Food Stockist Directory Coeliac Queensland has launched an online Restaurant, Café and Food Stockist Directory. The listing previously consisted of restaurants and cafes recommended by members and was only available to current members of Coeliac Queensland. The online directory is now accessible via our website and is available to the general public, not just members. This will be an invaluable resource for people living in or travelling to Queensland, who require a gluten free diet. Visit to view the directory. Each listing includes full contact details, the company logo and website, opening hours, cuisine, average mains price and more. Listings are connected to Google Maps, making it even easier for customers to find you. Coeliac Queensland will be regularly promoting the online directory via our bi-monthly member e-newsletter, Against the Grain and Coeliac Queensland Facebook page. All restaurant members of Coeliac Queensland receive a listing as part of their membership. Please note that there will only be one listing per membership. There is a package deal for franchised stores. Costs Membership is only $75 per year and runs from 1 July to 30 June. A renewal reminder will be sent out in May. If you are a franchise, please [email protected] for further information on franchise packages. How do I join? Simply fill out the Joining Form and sign the Catering Agreement and Terms and Conditions on the following pages and return them to Coeliac Queensland via or fax. Once we have received payment we will post out your new member kit and add your listing to our online directory. Page 3

4 Terms and Conditions Definitions Website : CQ : Coeliac Queensland Restaurant : the company (including all its employees, agents and contractors) that has been accepted by CQ to be added as a Restaurant Member. Website Listing The Restaurant agrees to provide accurate information regarding the restaurant to CQ to be listed on the website. Should any details change, the restaurant agrees to notify CQ as soon as possible so that the listing can be updated. Only one listing will be published per membership. Gluten Free The Restaurant agrees to offer a good selection of gluten free items on their menu. The definition of a good selection is at the discretion of CQ, but a guide is at least two items per menu course. The Restaurant agrees to abide by the catering terms and conditions, particularly with regards to contamination. If the Restaurant no longer offers a good selection of gluten free options, they are to notify the CQ immediately so that their listing can be removed. No refund will be given if a listing is removed. Terms of Payment The listing will not be published online until 2015/2016 Restaurant Membership fees have been received in full. Renewal of Memberships All memberships and website listings are valid until 30 June 2016 at which time they will need to be renewed. A membership renewal form will be sent out in May each year. This form must be completed and all fees paid by the due date for the listing to remain on the website. Voting rights Please note that Restaurant Members are not entitled to a vote at the Annual General Meeting as they are classified as Associate Members under the Coeliac Queensland constitution. 5 Catering Agreement Please tick the following boxes to confirm that you agree to these terms and conditions. I will offer a good selection of gluten free options. I agree to notify Coeliac Queensland if I no longer offer a good selection of gluten free options. I will take all care to prevent cross-contamination whilst preparing gluten free meals. I will ensure all staff are familiar with the gluten free options offered and understand the importance of gluten free requests. I will regularly review gluten free food preparations. All of my products marked gluten free DO NOT consist of any ingredients containing gluten and are not exposed to cross-contamination. I understand that 1/100 of a slice of bread is enough to cause damage to the small bowel of a person with coeliac disease. I understand that oats are not gluten free in Australia, even if marked wheat free or contamination free, and are not suitable for someone with coeliac disease. I understand that my restaurant may be removed from the website at Coeliac Queensland s discretion in the event of member complaints. No refund for membership will be provided in such circumstances. 8 Member Discount Program About Coeliac Queensland has introduced a discount program for our members as both a benefit for them and a way for restaurants to promote their gluten free menus. A listing on our website as a participant of our Member Discount Program A link to our Member Discount page on the Coeliac Queensland website which is promoted in our monthly member e-newsletter. How do I join? To participate, all you need to do is provide discounts or special offers to our members on presentation of their Membership Card. (Note: There is no fee for participation in this program) You may choose to offer a percentage discount (e.g. 10% off all purchases), buy one get one free or some other special offer.
