3 Selfmanagement is not a question but a fact for everyone and especially for those with a chronic illness. Hours yearly awake. Decides over his own life, own nutricion, movement and goals. Support of selfcare means for the patient: Better understanding of the healthcare process More control and setting personal goals with respect to health, lifestyle and behaviour Better balanced process with demand of the patient Higher level of therapy follow up Reduction of burden of disease and better quality of care.
