3 Selfmanagement is not a question but a fact for everyone and especially for those with a chronic illness. Hours yearly awake. Decides over his own life, own nutricion, movement and goals. Care provider Interaction Commisioning Patient None of the parties can do it on its own. Selfcare Supported! combines efforts in the triangle patient, care provider and insurance company. Only when implementation elements will be implemented in a coordinated way selfmanagement can be successfully executed. Long term consistent execution of regular processes.
