3 %nit ed %;tat CB Cioil Serbice Retirement Spitern t is important that you, as a member of this system, thoroughly understand the benefits and privileges this membership gives and what your responsibilities are for securing these benefits. You should be fully aware, not only of what it will mean to you during your lifetime but also of the protection it gives your family upon your death. You are urged to acquaint yourself with the facts in this certificate. Then place the certificate with your other important personal papers for safekeeping. Review it from time to time so that you will be thoroughly familiar with your rights and can plan intelligently for your future. 1. How the Retirement Fund s Financed 1. Deductions are automatically made from your basic salary each pay period. The following shows the rate of deductions for various periods: 2% percent from August 1, 1920, to June 30, 1926; 3y2 percent from July 1, 1926, to June 30, 1942 ; 5 percent from July 1, 1942, to the day before the first pay period after June 30, 1948; and 6 percent thereafter. 2. The Government makes annual appropriations sufficient. to maintain the retirement fund in a sound financial condition, and to assure the payment of your annuity when you retire. 11. What' the Term "Creditdble Service.' Means All civilian service in or under the Federal Government and the District of Columbia Government (including service in temporary positions and in positions excepted from the competitive requirements of the Civil Service Act), and generally all periods of active honorable military service, are creditable. Military service means service in the i Armv., Navv., Marine Cor~s. and Coast Guard. The word "service", used hereafter means "creditable service." 111. How You May Receive Full Credit for Service for Which You Have No Money in the Fund 1. You will be given full credit, without deposit, for all civilian service performed before August 1, 1920; and for all military service, regardless of when rendered. 2. You may receive full credit for service performed after July 31, 1920, for which no deductions were taken by making deposit to the retirement fund. The amount required for deposit will be the proper percentage deduction of salary (2'75, 31/2, 5, or 6), plus interest at 4 percent to December 31, 1947, and 3 percent thereafter, compounded annually, during all periods of employment. f the deposit is not made, your annuity will be reduced by one-tenth of the amount of the payment. 3. You may receive credit for service for which you have received a refund only by making redeposit to the fund. The amount re- quired for payment will be the sum refunded, with interest. credit will be give~z rdnless the redeposit is made. 1V. When You Will Be Eligible for Annuity Benefits Five years of service as a rzvilian employee are required before you may receive annuity benefits under any condition. 1. Age retiren~ent.-you rrtust retire at age 70 if you have had 15 years of service. f you have served less than 15 years when you reach your 70th birthday, you need not retire until the 15 years are completed. 2. Optional retiren2ent.-you may retire: (a) At or after age 60 upon completion of 30 years of service; (b) At or after age 62 upon completion of 15 years of service; (r) Between the ages of 55 and 60 upon completion of 30 years of service, but on a reduced annuity. 3. Disconti~?ued-service retirement (after 25 years).-you may retire on a reduced annuity, regardless of your age, if you are involuntarily separated through no fault of your own after serving 25 years. 4. Discontinued-service retirement (after 5 years).-you may receive an annuity when you reach the age of 62 if at the time you are separated from the service you have completed 5 or more years of service. 5. Disabjlity retirement.-you may retire because of disability after 5 years of service if you are found by the Civil Service Commission on. official medical examination to be totally disabled for useful and efficient service in your position or a similar one. The disability need not be caused by your official duties but must not be the result of misconduct or vicious habits. Application for disability retirement must be made before separation from the service or within 6 months thereafter, except in cases of certain incompetent and hospitalized persons. The annuity is computed under the basic formula indicated below, regardless of the age of the applicant. V. How Much You Will Receive 1. Basic formula.-all annuities are computed under a basic formula. This formula is: One percent plus $25, or 1y2 percent, of the average annual basic salary for the highest 5 consecutive years, multiplied by the years of service. The 1 percent plus $25 is used for average salaries up to $5,000, while the 1% percent is used in all other cases. The total annuity may not exceed 80 percent of the average salary for the highest 5 consecutive years. f no deposit is made for service not covered by deductions, the basic formula annuity will be reduced as indicated in Sec The following table illustrates annuities computed under the basic formula when all deposits and redeposits have been made. 5 2. Reduced anntlilies.-the annuities for optional retirement between the ages of 55 and 60 with 30 years of service (Sec. V-2 (c) ) and for discontinued service after 25 years (Sec. V-3)- are first computed under the basic formula and then reduced by one-fourth of 1 percent for each full month (3 percent for each full year) you are under age The survivorship plans: (a) Widow survivor-annuitant.-as a husband retiring for any reason, other than discontinued service after 5 years, you may take a reduced annuity and thus provide an annuity for your widow upon your death. Your widow will receive the annuity at age 50, or immediately upon your death if she is already past age 50, and it will continue until she dies or remarries. Your rate of annuity will be 90 percent of the full rate due you under the basic formula, with a further reduction of three-fourths of 1 percent for each full year your wife is under age 60 at the time of your retirement, but it may not be 1-ess than 75 percent of the full rate. Your widow's rate of annuity will be one-half of your full rate, excluding any part of it purchased by voluntary contributions. (b) Survivor-annuztant with inszrrable 2ntere~t.-As an unmarried employee retiring for any reason, other than discontinued service after 5 years, or disability, you may take a reduced annuity and thus provide an annuity for a survivor upon your death. n order to name the survivor, you must show that you are in good health at the time you retire, and the survivor you name must have an insurable interest in you (for instance, a claim of dependency on you). The reduction in your annuity depends on the age of the survivor-annuitant in relation to your age. The annuity payable to your survivorannuitant will be paid immediately upon your death, and will be one-half of the reduced rate paid to you during your lifetime, excluding any part of it purchased by voluntary contributions. t will continue during the lifetime of your survivor-annuitant. The rates of annuity due you in relation to the different ages are: AGE OF SURVVOR-ANNUTANT N RELATON 7'0 YOUR AGE Older, same age, or less Five, brtt less than Ten, but less than Fifteen, but less than 20, years Twenty, but less than 25, years Twenty-five or more years 1 PERCENTAGE OF FULL RA:',UE V. What You May Do To Purchase Additional Annuity 1. You may make voluntary contributions. These are in addition to the regular 6 percent deductions taken from your pay. These con- tributions may not exceed 10 percent of your total basic pay since August 1, 1920, and must be made in multiples of $25 (i. e., $25, 350, $75, etc.). Application to make such contributions must be filed with the Civil Service Commission. These contributions earn interest at 3 percent, compounded annually. When you retire, they will be used to increase your annuity. f you are separated before you are eligible to retire, the amount in your voluntary contribution account will be returned to you; if you die, it will be payable to your beneficiary or estate. No voluntary contributions may be made until all refunds have been redeposited and deposit for all service after July 31, 1920, which was not covered by deductions has been made. 2. Each $100 sum credited to your voluntary contribution account will buy annuity in addition to the regular annuity in amounts increasing at 20 cents each year from $7 at age 55 to $10 at age 70 (i. e., $7.20 at age 56; $8 at age 60; $9.80 at age 69). VZ. What You Receive if You Leave the Government Service Before Becoming Eligible for Retirement on Annuity < With less than 5 years of civilian service, you may receive a refund of the amount deducted from your salary, plus interest at 4 percent to December 31, 1947, and 3 percent thereafter, compounded annually, to date of separation, except that no interest is paid if the refund period is 1 year or less. 2. With 5 or more but less than 20 years of civilian service you may receive: (a) A discontinued-service annuity at age 62 (Sec. V-4) ; or (b) A refund of the amount deducted from your salary, plus interest at 4 percent to December 31, 1947, and 3 percent thereafter, compounded annually, to date of separation. 3. With 20 or more years of civilian service, you will receive a discontinued-service annuity at age 62 (Sec. V-4). You?nay not receive a refund of the deductions taken from your salary. VZZ. What Annuity Benefits Your Widow and Children Will Receive if You Die While in Active Service After Completion of 5 Years of Civilian Service - - To be eligible for these benefits: (a) Your widow must have been married to you at least 2 years immediately before your death, or be the mother of your child born during the marriage; (b) Your child must be unmarried, and be under age 18 or incapable of self-support because of physical or mental disability. The following chart illustrates the conditions under which the survivor benefits are paid, the amount of annuity, and the periods of payment : -

To each child: (a) One-half of the annuity which would have been due the employee at time of death, (b) $1,200 divided by the number of surviving children, or (c) $480, whichever is the least. To each child: At death, marriage, 18 (except where disability bars self-support), whichever is earliest. Married woman. NO PROVSON No~~.-Upon the death of the widow or termination of annuity to any child, the annuity payments to the remaining children will be recomputed. The term "annuity" as used in column 4 means the annuity which would have been due the employee at time of death, excluding any part of it purchased by voluntary contributions. ZX. The term "annuity" as used in column 4 means the annuity received by the retired employee or, if he chose the survivorship annuity when he retired (Sec. V-3), the annuity he would have received if he had not named a survivor-annuitant. Any part of the annuity purchased by voluntary contributions is excluded.

7 X. What 1s Meant by Lump-Sum Death Payments; When and to Whom They Are Made The law requires that you designate a beneficiary to receive any amount payable in a lump sum following your death. To name your beneficiary you should execute Form 280&1 in duplicate and forward it to the Civil Service Commission (the form may be secured from your personnel oficer). The designation is not valid unless received in the Commission before your death. The duplicate will be returned to you to keep with your personal papers. t is suggested that it then be attacked to this certificate and kept in a safe place so that it may be located readily in the event of your death. You may change your beneficiary at any time and without the consent of the previously designated beneficiary by executing a new designation. t is unnecessary, however, to execute a new designation merely because you transfer to a different agency or because of a change in your position. f there is no beneficiary living to receive the lump-sum payment, it will be paid to the administrator or executor of your estate, or next of kin, in the order named. The following chart illustrates the conditions under which lumpsum death payments are made and the amount of such payments: DECEASED PERSON i Employee leaving no widow or children entitled to immediate or future annuity. NOTE.-Any unpaid annuity (full months only) due the retired employee upon his death will be paid to his designated beneficiary or his estate; any unpaid annuity due a survivor will be paid upon death to the survivor's estate. nterest is earned on the employee's account at 4 percent to December 31, 1947, and at 3 percent thereafter to death or retirement, whichever is earlier. t is compounded annually. i X. How Payment s Made X.
