6 Mail-in Reimbursements 1. Incur eligible medical expenses totaling $100 or more. 2. Fill out a form. Attach your receipt or Explanation of Benefits (EOB). 3. Mail or fax the form and receipt to MidAmerica. 4. MidAmerica will send a check $5 fee for each reimbursement, not to exceed $30 per year.
7 Online Reimbursements 1. Incur eligible medical expenses totaling $100 or more. 2. Fill out a digital PDF form (found on MidAmerica s website). 3. Use a digital copy of your Explanation of Benefits (EOB), prescription record or scan your receipt. 4. to claims@midamerica.biz 5. MidAmerica will send a check. 6. $5 fee for each reimbursement, not to exceed $30 per year. 7
8 Recurring Claims Monthly Payments Recurring claims are expenses that you incur on a monthly basis and cost the same each month. Example: Health Insurance, Medication 1. Fill out the Recurring Claim Form found on MidAmerica s website. 2. After paying your first bill, provide a copy of the bill, along with the form, to MidAmerica either electronically or by paper mail. The bill should state name of the expense, amount and frequency. 3. MidAmerica will send a check each month. 4. Participants are required to submit monthly documentation as proof of continued payment, such as a cancelled check or receipt. 5. $5 fee for each reimbursement, not to exceed $30 per year. 8
