6 ACCOMMODATION: Accommodation is arranged in the Hotel Flora, (single and double-rooms with bathroom, toilet and TV set are available), November 29 December 1, 2012 (2 nights), contact to the Hotel Flora ( Tel.: ). In case of the hotel requirements - please make your own reservations. Please note, that the reservation at Hotel Flora is only preliminary and deadline for reservation is October 31, 2012, if you contact hotel Flora, there is necessary to mention that you are participant of the Dyskinesias BOARD: Dinner willl be given on November 29, 2012 Restaurant Vila Primavesi, Univerzitní 7, Olomouc (close to the meeting venue). Lunch is on December 9, 2011 at the Center of Arts of Palacký University, Univerzitní street 3 5, Olomouc. CANCELLATION AND REFUND OF REGISTRATION FEES: In case of cancellation, please contact the Conference Centre, Palacký University, Olomouc. The cancellation deadline for full registration fee refund is 31 October Refunds will not be considered for requests received after this deadline date. All approved refunds will be issued after the Conference.
