2 OBJECTIVES Overview While every module in the BSN Completion Program has obvious course objectives and, less obviously, personal objectives, the Community Health module incorporates a third group: program objectives. We have identified certain abilities that define the expanded role of the BSN graduate, and we need to document if those abilities are being evidenced by our students. With this in mind, students are asked to write weekly summaries of their individual and group ALP activities. They need to identify which program, course and personal objectives they have met through the activities described. The objectives are listed as follows: Course objectives: Upon completion of this course, each student should be able to: 1. Integrate the concepts of community, community health, community health nursing, and primary health care in a variety of community settings. 2. Analyze the impact of historical, current sociopolitical, economic, cultural, and religious factors which impact community health and health services including community health nursing. 3. Apply the nursing process to promote the health of individuals, families, and communities. 4. Differentiate the roles of the Community Health Nurse in a variety of settings. 5. Utilize principles of education and change theory to promote the health of individuals families, and communities. 6. Act as an advocate for individuals, families and communities in relation to health issues. 7. Utilize principles of Christian philosophy and values in the practice of community health nursing. Program objectives: The expanded role of the BSN graduate will include the ability to: 1. Base their practice on current knowledge, theory and research. 2. Assume responsibility and accountability for practice. 3. Form partnerships with patients and with other health care professionals. 4. Communicate, collaborate and negotiate. 5. Practice across a variety of settings with diverse populations. 6. Access, assemble and evaluate health information. 7. Teach patients. NURS 4534 Preceptor Pkt. 1

3 Personal objectives Students will need to identify their personal objectives for this course. They may choose to define overall objectives, weekly objectives, or a combination of the two. There is no specified number of personal objectives required. They are asked to just be honest. What do they want to learn from this course? Meeting the objectives Please note that from weeks 2-7 of this module a student s weekly ALP summaries must show or describe how his/her ALP activities met the above objectives. They must show evidence of meeting all objectives in some form in order to pass the ALP portion of this module. Weekly forms The following page is a blank form students are to copy and use weekly. 5 INSTRUCTIONS TO PRECEPTORS Overview The assignments and Applied Learning Project (ALP) experiences for this course are designed to help students apply the nursing process and the Neuman theory for the promotion of health for individuals, families, groups and communities. This is accomplished through primary, secondary and tertiary prevention methods in the community setting local, national and international. Students are challenged to consider the conceptual base for community health and nursing, the diversity of roles for the community health nurse, and factors impacting the health care system and thus nursing. Special emphasis will be placed on major community health problems. The ALP experiences will require application of the nursing process to individuals and groups in the home, school, industry and other community settings. There are no set in stone rules for accomplishing the ALPs. Students are free to set personal goals for this course, using their own creativity and ideas to meet course objectives. Community and Public Health Nurse Observation/Participation Assignments (Individual Assignments) Students are to choose one area of community health nursing, and one public health nursing site. Some suggested areas of community health nursing are: school nursing, occupational health, correctional nursing, home health, case management, rehabilitation, parish nursing, environmental health, health policy, epidemiology, and community education. If students currently work in a community health setting, or have in the past, they must select an area outside their experience. Students are to design experiences that will expose them to the various nursing roles in these areas. Probably the most effective type of experience is a combination of observation and participation. Once they have had an opportunity to see what nurses in these various roles do, they are to write up their experience, keeping in mind the overall course objectives and evaluation criteria. Small group ALP component Each group will need to select a community to assess. You may recall that in the ALP for the Health Promotion module, you were asked to select a group or community from your immediate circle of influence. For this class your community will need to encompass a larger area, but please do not select the entire city of Tulsa or Oklahoma City. That only happens at the graduate level! At the baccalaureate level, common definitions of community would include a local neighborhood or a defined aggregate within a certain geographical location. NURS 4534 Preceptor Pkt. 4

6 Notebook Each group will prepare an assessment notebook for this project, which contains the following sections: Written Report (Notebook) A. Assessment ( See Table 19-1 in text. Determine data needed and collect data. Interpret data and identify needs. Set goals based on needs.) 1. Familiarization or Windshield Survey (Write about your experience) 2. Community Profile Inventory (See Table 18-4 in text. Answer the community assessment questions for each variable that are applicable to your community). a) Location variables b) Population variables c) Social variables 3. Data Collection (Choose two or more of the following. This will assist you in evaluating information about your community s health status to discover existing or potential needs as a basis for planning future action. See pages in text). a) Surveys b) Descriptive epidemiologic studies c) Community forum/town hall meetings d) Focus groups e) Healthy People 2010 f) Other Sources (WHO, U.S. Public Health Service, National B. Analysis and Design (Diagnosis) Institutes of Health, local health department, etc.) 1. Analyze findings and set specific objectives 2. Design alternative solutions 3. Examine costs and benefits of proposed solution. 2. Judge the potential outputs, outcomes, and impact of plan. 8 RESPONSIBILITIES STUDENT RESP ONSIBILITIES The student will locate a Preceptor outside his/her current area of practice, and arrange the details of the practicum experience to fulfill course requirements. The student will perform appropriate practicum services as agreed upon and monitored by the Preceptor. The student will make relevant entries into clients records as appropriate to the experience. The student will be required to submit, in class, a weekly summary of experiences. OKLAHOMA WESLEYAN UNIVERSITY RESP ONSIBILITIES OWU will be responsible for the educational programs of students assigned to the Preceptor. OWU will provide upon request A letter/proof of RN licensure in the state of residence. A physical form on record with proof of Rubella immunization A negative TB Mantoux test or chest x-ray Hepatitis B immunization completed or in process OWU will be responsible for the proper conduct of the student during the practicum experience with the Preceptor as governed by the rules and regulations of Oklahoma Wesleyan University. P RECEP TOR RESP ONSIBILITIES The cooperative relationship between the Preceptor for the RN-BSN Completion student and the Oklahoma Wesleyan University Division of Nursing is agreed to for purposes of education and practicum experience. Oklahoma Wesleyan University, through the RN- BSN student, will provide limited clinical services to attain specific and limited goals to be defined with the Preceptor. If at all possible, the Preceptor must be a Baccalaureate-prepared RN with current practice/licensure in their state of residence, and will retain full responsibility for the care of all clients. The Preceptor will Provide, insofar as possible, a practicum experience for the RN-BSN Completion student which will enable them to fulfill their requirements for the course. Provide necessary orientation, supervision, and guidance in the practicum activities of the student. NURS 4534 Preceptor Pkt. 7

9 Maintain administrative and professional supervision of the student insofar as his/her presence affects the operation of the College and/or the direct or indirect care of clients. Provide, without cost to Oklahoma Wesleyan University or the student, required practicum experiences. Service provided to clients by the student will accrue to the Preceptor's total service record. Complete an evaluation of the student, mailing the completed evaluation to the faculty member. NURS 4534 Preceptor Pkt. 8
